สาเหตุ
1.) ร่างกายสร้าง glucocorticoid
มากเกินปกติ
- มีความผิดปกติที adrenal
gland
- มีความผิดปกติที pituitary
gland มีเนืองอกหรือ tumor ทําให้
มีการหลังฮอร์โมน ACTH มากกว่า
ปกติ จะเรียก Cushing's
syndrome จากสาเหตุนีว่า
Cushing’s disease หรือ
Pituitary Cushing
- มี ectopic ACTH secretion
2.) ร่างกายได้รับฮอร์โมน
glucocorticoid มากเกินปกติ
สาเหตุทีพบได้บ่อย ได้แก่ การใช้ยา
corticosteroid
อาการอืนๆ เช่น
-สูญเสียมวลกระดูก -ความดันโลหิตสูง
-มีนําตาลในเลือดสูง -เปนเบาหวาน
-หงุดหงิดง่าย ซึมเศร้า -ติดเชือต่าง ๆ ได้ง่าย
-มีประจําเดือนผิดปกติ
เปนกลุ่มอาการทีเกิดจากมี
glucocorticoid โดยเฉพาะ
ฮอร์โมน cortisol มีระดับของ
ฮอร์โมนในกระแสเลือดสูง
ผิดปกติ และสูญเสีย
ลักษณะของ diurnal
rhythmicity ของ
cortisol secretion
นอกจากนีผู้ปวย Cushing’s syndrome อาจแบ่ง
ตามฮอร์โมน ACTH เปน 2 ชนิด คือ
1.) ACTH dependent เช่น Cushing’s disease
(พบบ่อยสุด), ectopic ACTH-secreting tumor
2.) ACTH independent เช่น adrenoma,
carcinoma, nodular adrenal hyperplasia,
adrenocortical rest tumor
Cortisol helps regulate your blood pressure, reduces
inflammation, and keeps your heart and blood
vessels functioning normally. Cortisol helps your body
respond to stress. It also regulates the way your
body converts proteins, carbohydrates and fats in
your diet into energy.
