This document outlines a continuum of care model for aging adults that aims to improve quality of life and independence. It discusses how care needs may change over time from total independence to some dependence on others. The model proposes targeting all aging adults and providing various support services within their residence or at senior living communities to help them maintain independence and quality of life as they age. Examples of services include assistance with activities of daily living, wellness programs, social activities, and rehab sessions. The goal is to focus on capabilities rather than disabilities and create a "slippery slope" where residents naturally progress to higher levels of care seamlessly as needed for a win-win-win solution.
