This document discusses accident reporting, investigation, and analysis. It defines different types of incidents from near misses to severe accidents and outlines appropriate response and investigation based on incident level. Level 1 incidents involve minor injuries or damage while level 3 are most severe, involving fatalities, serious injuries, major damage or production loss. The appropriate personnel to investigate and responsibilities for remedial actions increase according to incident level. The goal is to learn from incidents to prevent recurrences and improve safety.
This document provides information on report writing, including the typical structure and elements of a report. It discusses that reports are objective accounts of events that are written for a specific purpose. While reports can contain opinions, they are based primarily on collected facts and findings. The document outlines the key sections of a report, including an introduction, body, recommendations, and conclusion. It also provides examples of report elements like the title page and language conventions. Finally, the document includes a sample report and cover letter as an example of a formatted accident investigation report.
1. An assistant engineer visited TCT Engineering to learn about optical fiber systems as an alternative to improve his company's copper cable transmission system which suffers from interference issues.
2. He learned that optical fiber has a much higher capacity than copper cable and can transmit all types of data simultaneously without electrical interference. It also allows faster transmission while using lighter and less expensive materials.
3. Based on the knowledge gained, the engineer believes optical fiber will increase their competitiveness and recommends senior staff visit TCT to learn more about implementing such a system as Malaysia aims to fully digitalize telecommunications.
An accident investigation aims to improve safety by exploring the causes of events and identifying remedies. All accidents, regardless of severity, should be investigated to some degree to understand root causes. A thorough investigation involves collecting evidence from the scene, documents, and witness interviews without blame. The investigation process determines immediate causes like unsafe acts or conditions, as well as underlying causes involving management systems. The results are recorded and analyzed to identify corrective actions and prevent future occurrences.
This document provides an introduction to accident and incident investigation. It discusses the importance of reporting accidents and incidents, identifying immediate and underlying causes, conducting interviews and investigations, and developing action plans to address failings and prevent future occurrences. The goals are to understand why accidents happen using methods like the "why-because" model of root cause analysis and to respond appropriately after an incident.

Bab 1 penyiasatan saintifik
1. Bab 1 Penyiasatan Saintifik
Sains ialah ilmu tentang alam sekeliling kita. Untuk mengkaji tentang alam sekeliling
kita, penyiasatan saintifik hendaklah dilakukan.
Langkah-langkah penyiasatan saintifik mengikut turutan :
• Mengenal pasti masalah
• Membuat hipotesis
• Merancang penyiasatan
• mengawal pemboleh ubah
• Mengumpul data
• Menganalisis data
• Membuat kesimpulan
• membuat laporan
Terdapat 3 Jenis pemboleh ubah (PU) dalam penyiasatan saintifik.
a.PU yang dimalarkan-apakah perkara yang disamakan?
b.PU yang dimanipulasikan-apakah perkara yang di ubah-ubah?
c.PU yang bergerak balas-apakah yang dilihat(diperhatikan)?
