The newsletter announces that two textbooks edited by physicians from the Al-Sabah Arrhythmia Institute were published by Lippincott Williams & Wilkins over the past year. The Next Generation FIRM-Assisted Ablation AF Treatment Approach
Dr. Sanjiv Narayan developed the FIRM-assisted ablation approach for treating atrial fibrillation (AF) which involves widespread mapping of the heart and computational analysis to identify rotational or focal electrical sources causing fibrillation. This approach precisely maps circular rotational activity sites called rotors that were previously not well understood. Patients can then be treated with a FIRM-assisted ablation procedure using catheter-based application of heat or freezing to address the identified electrical chaos points causing their AF. Several new technologies have now adopted this focal impulse and rotor modulation approach developed by Dr. Narayan to map and treat localized drivers of atrial fibrillation.
QRS FRAGMENTATION: ADULT TETROLOGY OF FALLOT
QRS fragmentation on electrocardiogram (ECG) is a proposed risk factor for mortality in adults with repaired tetralogy of Fallot (TOF). This study evaluated whether QRS fragmentation could predict mortality in a different population of 465 adults with repaired TOF. The presence and severity of QRS fragmentation on baseline ECG was assessed. Over a mean follow up of 13.6 years, 55 deaths occurred. Moderate to severe QRS fragmentation was associated with a 2-fold increase in risk of all-cause mortality and reduced 20-year survival compared to those without or with only mild fragmentation. ESC New Pericardial Guidelines

Cardio cerebral resuscitation
Cardio-cerebral resuscitation is a new approach for treating patients experiencing primary cardiac arrest that emphasizes minimizing interruptions to chest compressions, passive ventilation, and early epinephrine administration. Sheep were fed a high calorie diet to induce obesity over 8 months while controls maintained normal weight. The results showed that increasing weight was associated with larger left atrial size, more atrial fibrosis, inflammation and lipid deposits. Weight gain also reduced conduction velocity, increased conduction heterogeneity and AF inducibility. Molecular markers of fibrosis increased with greater adiposity. Therefore, progressive obesity leads to structural and electrical remodeling in the atria that promotes the development of AF.

Stroke is a leading cause of death and disability in the U.S., with 800,000 cases occurring each year. Each year in the United States, an estimated 300,000 cases of VTE occur. Mortality can be as high as 3.8 percent in patients with deep vein thrombosis (DVT) and 38.9 percent in those with pulmonary embolism (PE). VTE is associated with a high risk of death in the U.S. and Europe, with an estimated incidence rate of 1 in 1,000 patients. VTE is particularly common after a stroke. Approximately 20 percent of hospitalized immobile stroke patients will develop DVT, and 10 percent a PE.

This study examined the effects of progressive weight gain on the development of atrial fibrillation (AF) in sheep. Sheep were fed a high calorie diet to induce obesity over 8 months while controls maintained normal weight. Cardiac imaging, electrophysiological testing, histology, and molecular analysis were performed at baseline, 4 months, and 8 months. The results showed that increasing weight was associated with larger left atrial size, more atrial fibrosis, inflammation and lipid deposits. Weight gain also reduced conduction velocity, increased conduction heterogeneity and AF inducibility. Molecular markers of fibrosis increased with greater adiposity. Therefore, progressive obesity leads to structural and electrical remodeling in the atria that promotes the development of AF.

Stroke is a leading cause of death and disability in the U.S., with 800,000 cases occurring each year. Each year in the United States, an estimated 300,000 cases of VTE occur. Mortality can be as high as 3.8 percent in patients with deep vein thrombosis (DVT) and 38.9 percent in those with pulmonary embolism (PE). VTE is associated with a high risk of death in the U.S. and Europe, with an estimated incidence rate of 1 in 1,000 patients. VTE is particularly common after a stroke. Approximately 20 percent of hospitalized immobile stroke patients will develop DVT, and 10 percent a PE.

PGx Analysis in VarSeq: A User’s Perspective
Since our release of the PGx capabilities in VarSeq, we’ve had a few months to gather some insights from various use cases. Some users approach PGx workflows by means of array genotyping or what seems to be a growing trend of adding the star allele calling to the existing NGS pipeline for whole genome data. Luckily, both approaches are supported with the VarSeq software platform. The genotyping method being used will also dictate what the scope of the tertiary analysis will be. For example, are your PGx reports a standalone pipeline or would your lab’s goal be to handle a dual-purpose workflow and report on PGx + Diagnostic findings.

Following genotyping, explore alternative tertiary workflow concepts in VarSeq to handle PGx reporting.

The first, entitled A Practical Approach to the Catheter Ablation of Atrial Fibrillation, was edited by Dr. Steinberg, along with internationally recognized experts, Dr. Jais from France and Dr. Calkins from Johns Hopkins in Baltimore. Contributors were selected for their experience, academic credentials and practical know-how and were asked to produce a variety of chapters that would allow readers to establish ablation programs, specifically focusing on the most challenging of all arrhythmias, atrial fibrillation. Atrial fibrillation is the most common sustained arrhythmia and is responsible for considerable morbidity including stroke, and can produce a variety of debilitating symptoms. In addition to providing editorial guidance, Dr. Steinberg along with Arrhythmia Institute colleagues Drs. Mittal and Arshad, contributed chapters. The 2nd textbook, entitled Electrophysiology: The Basics, was intended to provide a framework for the education of cardiology trainees. Edited by Drs Steinberg and Mittal, it has already been translated into several languages. All members of the Arrhythmia Institute were co-authors on several chapters, and experts from around the world also contributed.
