This document provides instructions and a performance appraisal form for evaluating vice presidents, deans, and department chairs at Fort Valley State University. The appraisal assesses employees on factors such as human relations, managing resources, leadership, communication, work quality and quantity, and personal characteristics. Raters are asked to indicate the level of importance and provide a rating for each performance factor using a scale of 0 to 5. Section III allows for comments on the employee's overall performance and strengths/weaknesses. The form concludes with an overall numerical rating and signatures from the employee, vice president, and president.
