Clinical Synthesis Map: Cancer Care Pathways in Canadian Healthcare
Jones PH, Shakdher S and Singh P. Systemic visual knowledge translation for breast and colorectal cancer research. Current Oncology 2017 (in press).
The Clinical Map visually represents breast and colorectal cancer processes across Canadian provincial and territorial systems. A roadmap metaphor illustrates a system-wide view of patient flow across the stages of cancer care. Green “road signs” identify clinical cancer stages across the roadmap: Pre-Diagnosis, Peri-Diagnosis, Diagnostic Interval, Diagnosis, Treatment, Rehabilitation, After Care, and Survivorship (with Palliative Care expressed as an end point). The visual metaphor of seasonal trees visually connects these stages to the patient’s cancer journey from pre-diagnosis (summer) through treatment (winter), followed by new growth (spring) in survivorship.
The levels of primary, secondary and tertiary care guide the vertical dimension. Information and communications technology reaches across levels and stages, but is shown disconnected from primary care. The road-like pathways are colour-coded where experts differentiated care pathways between breast cancer (pink) and colorectal (blue). Where not distinguished (white), the pathways indicate current practices shared across the cancer journeys.
Yellow navigation signs indicate cancer events across primary care pathways. Starting with Prevention and ending with Long-term Care, these events show points for primary care continuity during cancer treatment. A parallel path below the stages indicates where some patients may also employ complementary or alternative therapies.
Significant areas of complexity generalized across cancer care are revealed in peri-diagnosis and the diagnostic interval pathways. A patient can be screen-detected (and then present to a family physician, shown in the breast cancer pathway) or may be initially diagnosed in primary care (white pathway). The circular pathways in the diagnostic cycle suggest multiple possible tests within primary care. With a primary care diagnosis, patients are referred and flow to secondary/tertiary cancer care. The stages of intake, biopsy, pathology, and confirmed diagnosis are shown, and the complex pathways of cancer treatment, shown on the map in a typical (not definitive) order of surgery, radiation/chemotherapy, and continuing treatment through assessment of outcome.
Though a recent study found repeat colonoscopy is good for certain patients, accurate documentation is still a crucial factor to determine whether it is appropriate.
To address family history collection, interpretation, and application in busy primary care practices, NCHPEG has collaborated collaborating with the March of Dimes, Genetic Alliance, Harvard Partners, and the Health Resources and Services Administration to develop and evaluate a novel family history tool that focuses on prenatal and neonatal health. The tool helps to improve health outcomes for the female patient, fetus, and family by providing clinical decision support and educational resources for risk assessment based on family history. New Directions in Targeted Therapeutic Approaches for Older Adults With Mantl...i3 Health
i3 Health is pleased to make the speaker slides from this activity available for use as a non-accredited self-study or teaching resource.
This slide deck presented by Dr. Kami Maddocks, Professor-Clinical in the Division of Hematology and
Associate Division Director for Ambulatory Operations
The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.
Target Audience
Hematology/oncology fellows, attending faculty, and other health care professionals involved in the treatment of patients with mantle cell lymphoma (MCL).
Learning Objectives
1.) Identify clinical and biological prognostic factors that can guide treatment decision making for older adults with MCL
2.) Evaluate emerging data on targeted therapeutic approaches for treatment-naive and relapsed/refractory MCL and their applicability to older adults
3.) Assess mechanisms of resistance to targeted therapies for MCL and their implications for treatment selection

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

Center for Women’s Imaging, which provides the experts and advanced diagnostic technologies needed to
detect breast disease in its earliest stage.
Oceanside, NY… For the third consecutive year, Becker's Hospital Review has named South Nassau
Communities Hospital to its of "100 Hospitals with Great Women's Health Programs" in the United States.
South Nassau was selected based on its clinical excellence, quality care and clinical quality and patient
safety awards for women's health services. Becker's Hospital Review is a bimonthly magazine published by
ASC Communications located in Chicago, Ill.
“This award underscores our commitment to providing the very best of care to women. From high-risk
pregnancies and infertility to breast cancer and reproductive malignancies, women across the South Shore know
they can depend upon South Nassau,” said Richard J. Murphy, South Nassau's president and CEO.
“We have been strengthening our specialty services for women in recent years at South Nassau and this
recognition validates those efforts,” added Alan Garely, MD, chair of obstetrics and gynecology.
The editorial team of Becker's Hospital Review based their selection of hospitals to the list on national
rankings and awards received from several institutions, including U.S. News & World Report national and
regional rankings for gynecology, CareChex rankings for women's health care, women's health Healthgrades
awards, Women's Choice Award's Best Breast Centers list and Baby-Friendly designation from Baby-Friendly
USA, the accrediting body for the Baby-Friendly Hospital Initiative in the U.S.
South Nassau offers a comprehensive scope of women's healthcare services, including breast health,
obstetrics and gynecology, women's imaging, and gynecologic oncology.
South Nassau’s Department of Obstetrics and Gynecology offers women access to specialized health
care for optimum treatment of reproductive health conditions, from endometrial biopsies and colposcopies to
News From:
2. gynecologic oncology. At South Nassau's Women's Center for Pelvic Floor Disorders, all-too-common
conditions such as incontinence and constipation are effectively diagnosed and treated using state-of-the-art
technology administered by highly skilled surgeons. This service is found in only a handful of locations in the
country, and South Nassau is one of them.
In the hospital’s Center for New Beginnings, a team of compassionate, experienced obstetricians labor
and delivery nurses, high-risk pregnancy specialists delivered 2,051 babies in 2015. In addition to advanced
maternal-fetal medical technologies, the Center features 26 private family/newborn rooms with first-class hotel
like accommodations, spacious labor and delivery rooms, and a 6-bed neonatal intensive care unit (NICU).
Staffed by board-certified neonatologists, the NICU provides the specialized care and is equipped with the
dedicated medical technologies that premature deliveries and newborns with developmental complications or
conditions require. The Center also has a dedicated state-of-the-art surgical suite for mothers needing a
Caesarean section delivery.
A recipient of the New York State Department of Health’s Perinatal Quality Collaborative Obstetrical
Improvement Quality Improvement Award, the center maintains a “five star” rating for patient safety and
quality care from an independent national hospital and healthcare ratings organization. The rating is based on
an in-depth evaluation of the quality of maternity, obstetrical and gynecological care over a three year period, in
the 19 states, which includes New York, where hospital patient outcomes data is publically available.
South Nassau’s Center for Women’s Imaging (CWI) provides the experts and advanced diagnostic
technologies needed to detect breast disease in its earliest stage. A designated Breast Imaging Center of
Excellence by the American College of Radiology, CWI offers an array of state-of-the-art diagnostic
technologies, including digital mammography, high resolution ultrasound and MRI of the breast.
If a tumor or abnormality is detected, the Center for Breast Health at South Nassau Communities
Hospital’s Gertrude & Louis Feil Cancer Center offers the most advanced treatments for breast cancer and uses
targeted surgical techniques that aim to preserve as much of the healthy breast and surrounding areas as
possible. This includes breast-conserving surgery (commonly known as lumpectomy), in which only the tumor
or abnormality is removed from the breast; or mastectomy, an operation in which the whole breast is removed.
For invasive breast cancer, these procedures may be accompanied by a sentinel lymph node biopsy, which is a
minimally invasive technique for staging of lymph nodes.
Oncoplastic surgical techniques are also offered. Oncoplasty combines surgical cancer removal with
plastic surgery techniques to produce a superior cosmetic end result. Recommendations for whole-body or
"systemic" treatments, such as hormonal therapy, radiation therapy, chemotherapy, or a combination of two of
the three therapies, may follow either surgical approach. If early-stage breast cancer is present, sentinel lymph
node biopsy is used as an alternative to traditional lymph node dissection.
The Center for Breast Health is accredited by the American College of Surgeon's National Accreditation
Program for Breast Centers (NAPBC). NAPBC accreditation is granted only to those centers that have
3. voluntarily committed to provide the best in breast cancer diagnosis and treatment and is able to comply with
established NAPBC standards.
The “100 Hospitals with Great Women’s Health Programs” listing is among a multitude of other awards
for safety and quality that South Nassau has earned during the past year, including:
• ‘A’ for patient safety from The Leapfrog Group;
• Being named as the South Shore’s only ‘Baby Friendly’ designated hospital, a program
sponsored by the World Health Organization and the United Nations Children’s Fund;
