The main concept of roles of leadership is based on impacting group activities and adjusting to change Sullivan, E. J. (2013. There is a challenge when considering leadership in the healthcare context because most theories are not formed within the healthcare framework for they were normally established for business contexts and later applied to the healthcare. Management can simply be defined as the process that managers utilize to realize organizational goals. It’s the process of achieving organizational set goals through the available resources.
How do management and leadership activities contribute to the success of change initiatives:
The activities involved in this process include planning, staffing, organizing, directing, controlling, and making decisions in the operations of the system to attain anticipated result and improve its entire performance. Management involves a combination of steps to follow to achieve the set requirements. Leaders look for suitable ways of doing things for instance they will establish the goals and purposes for the people (American Nurses Association, 2010; Sullivan, 2013). Anyone can be a leader without necessarily being in an authority position.
Transformational leadership is necessary along with good management of staff and resources. This entails committing individuals to action, converting those under you into leaders and converting the leaders themselves into change agents. It has less to do with use of power to pressure and suppress others in order to achieve result. Instead, it involves empowering them to understand and own the vision of the organization and trusting them to work towards the goals that profit not just themselves but the organization in general (Sullivan, 2013). In nursing empowerment would result to enhanced patient care, reduced sick days, and less attrition.
Through this, the staff will have higher job satisfaction and there will be higher retention of staff amongst there nursing functions.
What change projects are needed in your agency at this time?
Among the changes required is to ensure nurses practice to the full capacity of their education and training and programs to be formed to ensure nurses attain higher training to much the growing demands in the sector. This has led to the poor compliance with the standards of practice, and increase costs in the facility. Real World Case 6.1
A large urban children’s hospital in Dallas, Texas, is leading in the delivery of care provided to children from birth through age 18. After implementing an electronic health record, the hospital identified operations in need of improvement. It found that individual business units were working in their own silos with little interdepartmental communication occurring, and the individual business units had different policies, procedures, and processes for information governance and data management. The hospital quickly realized the need to standardize processes and create an effective information governance program to help streamline and manage the vast amount of data being collected across the organization.
Using tools that are available through AHIMA’s Information Governance Adoption Model (IGAM), the hospital evaluated the current state of information governance at the organization. This was done through the evaluation and review of information-related policies and procedures throughout the system. It also created the foundation necessary to implement a process to review, edit, and update all those information policies and procedures to create a consistent and standardized process across all business units of the organization. Most important, it showed the need to educate workforce members on the importance of having a consistent format for data collection across the entire organization.
The outcome of implementing an information governance program at the children’s hospital produced many benefits. The hospital was able to create a consistent process for training and educating all workforce members to support the transparency of data management to use the information to its competitive advantage. It created a platform to have open and transparent conversations throughout the healthcare organization, supporting the mission of the organization. By streamlining all the policies and procedures across the organization, the hospital was able to break down department silos that existed within the organization and implement an organization-wide culture supporting the information governance program. Doctor’s Hospital was facing a serious financial hardship as healthcare costs continued to spiral out of control and reimbursement plummeted. A new chief executive officer (CEO) was hired to turn things around in an effort to salvage the hospital. The CEO was determined to change the company culture, which he identified as apathetic and accustomed to mediocrity. He noted that the financial performance was suffering and attributed much of this to a variety of process issues as well as a lack of focus on the core business of patient care.
The CEO immediately took action to look at financial issues and cut costs. A drastic cost and labor reduction strategy was implemented with an aggressive timeline to turn the financial bottom line around. Within a few months, the hospital started to show less of a financial loss and things seemed to stabilize financially. However, the morale of the staff had taken a significant hit. Turnover increased as a sense of job security decreased, and the impact on the patients began to be seen in an increase in patient complaints and lowered patient satisfaction scores. A training program was introduced to remind employees about customer service with no impact on results. Finally, the CEO hired a consultant who performed an assessment. A multilevel program was implemented that incorporated all levels and all aspects of the business. This assisted leaders in understanding the link between finances, employee morale, and patient satisfaction. A culture of accountability began to emerge and began to shift the culture to one of service after two years. Finally, all business metrics began to move in the same direction.

As you all know, healthcare is always in high demand. With such a growing demand for healthcare services creates an even greater demand for health providers. Because the populations demand for healthcare is ever-growing and lack of health providers is on the rise, patient care deficits are prevalent. Patient care is not being met because there simply aren't enough providers to provide care for every patient in need. On the other hand, health care providers and organizations are suffering too. The workload is very strenuous on healthcare providers and detrimental to healthcare organizations. In our organization we are affected by this by having an overwhelming demand of patients in our emergency rooms and an underwhelming amount of staff to accommodate these patients.
In more detail this affects our organization by causing an increase in nurse burnout, 30-day readmits, and poor patient satisfaction scores to name a few examples. Heavy workload environments along with poor patient to nurse ratios contribute significantly to nurse burn out (Norful, 2018). Because of this, the chances of nurses making medical errors are more likely. As a result, our organization chances of liability for medical errors are increased. If patient care was not provided adequately because of lack of providers, many patients will come back to the hospital within 30 days. When this happens often insurance companies find the hospital responsible and will not pay (Park, 2018). As a result, the hospital is held accountable for lack of care and suffers financial losses. Satisfaction rating by patient often result in greater financial opportunities for our organization. A lot of patient dissatisfaction responses came from their needs not being meet. Things such as time spent with physicians and wait time to be seen are common issues.
Now that we know what the issue is what are we going to do about it? Though the lack of healthcare providers is a prominent issue for all healthcare entities it is not a new. For years studies have been conducted to help alleviate this issue. In a study, advanced scheduling was used to alleviate high wait times and overall continuity of patient care (Qu, 2007). The daily health care provider demand was examined along with the patient need to create a schedule that consist of adequate provider to patient ratios. As a result, healthcare providers were not overbooked, and patients had less of a wait time and more time spent with providers.
In another study, federal qualified health centers were initiated in order to decrease the shortage of health providers (Xue, 2018). These health centers provided underprivileged communities that did not have access to health care facilities, a place where they could be seen by primary providers. As the practice grew, it evolved from a “mom-n-pop” operation to a more complex model. The founding physician recruited four new neurologists to join and continue to help build the practice. This was very unusual, as physicians don’t typically write proprietary software for a practice. In addition, the entire platform including the billing program, which he also developed, used MS-DOS.
The practice employed a practice manager who had started with the founding physician. As the practice grew, the manager did not keep up with the basics of managing a practice. Her information relating to billing, reimbursement, and changes to current CPT and ICD-9 issues was out of date. She was also ignorant of the contracts the practice had, but more importantly the impact of those contracts on the practice and how to carry them out appropriately.
While the physicians were very productive, several significant management problems became apparent as the practice grew. In particular, the infrastructure suffered and there was no governance. Infrastructure for a private practice is different from that of a corporate model. For example, in a corporate model individual departments exist with defined responsibilities to support the needs of the corporation and other areas of the entity, such as an IT department. Conversely, in a private practice when IT systems need repair, the responsibility falls to the administrator or manager, and this individual must know how to address and fix the problem. In 1873 The University of Iowa began providing medical services when it reached an agreement with Sisters of Mercy to operate a small hospital in the area. It began with two wards, one for women and the other for men containing four private rooms and a surgical amphitheater. In 1865 this agreement was terminated when the Sisters of Mercy moved across town and opened up Mercy Hospital. Today, the University of Iowa Hospitals and Clinics is a public -teaching hospital affiliated with the University of Iowa and a Level 1 trauma center. It has 711 beds including a 190-bed UI Children’s Hospital (About Us, n.d.). On an average day, there are close to 9,000 individuals providing care to patients, including employees, students and volunteers (About Us, n.d.). Fiscal Year 2012 Facts
There were 32,000 patients admitted to the hospital for in-patient care with 59,000 emergency room visits. In the 200 outpatient clinics of the UIHC, 977,337 clinic visits were counted. In addition to the 1,300 volunteers of UIHC, it employed during FY2012:

This was done through the evaluation and review of information-related policies and procedures throughout the system. It also created the foundation necessary to implement a process to review, edit, and update all those information policies and procedures to create a consistent and standardized process across all business units of the organization. Most important, it showed the need to educate workforce members on the importance of having a consistent format for data collection across the entire organization.
The outcome of implementing an information governance program at the children’s hospital produced many benefits. The hospital was able to create a consistent process for training and educating all workforce members to support the transparency of data management to use the information to its competitive advantage. It created a platform to have open and transparent conversations throughout the healthcare organization, supporting the mission of the organization. By streamlining all the policies and procedures across the organization, the hospital was able to break down department silos that existed within the organization and implement an organization-wide culture supporting the information governance program. A culture of accountability began to emerge and began to shift the culture to one of service after two years. Finally, all business metrics began to move in the same direction.

Because the populations demand for healthcare is ever-growing and lack of health providers is on the rise, patient care deficits are prevalent. Patient care is not being met because there simply aren't enough providers to provide care for every patient in need. On the other hand, health care providers and organizations are suffering too. The workload is very strenuous on healthcare providers and detrimental to healthcare organizations. In our organization we are affected by this by having an overwhelming demand of patients in our emergency rooms and an underwhelming amount of staff to accommodate these patients.
In more detail this affects our organization by causing an increase in nurse burnout, 30-day readmits, and poor patient satisfaction scores to name a few examples. Heavy workload environments along with poor patient to nurse ratios contribute significantly to nurse burn out (Norful, 2018). Because of this, the chances of nurses making medical errors are more likely. As a result, our organization chances of liability for medical errors are increased. If patient care was not provided adequately because of lack of providers, many patients will come back to the hospital within 30 days. When this happens often insurance companies find the hospital responsible and will not pay (Park, 2018). As a result, the hospital is held accountable for lack of care and suffers financial losses. Satisfaction rating by patient often result in greater financial opportunities for our organization. A lot of patient dissatisfaction responses came from their needs not being meet. Things such as time spent with physicians and wait time to be seen are common issues.
Now that we know what the issue is what are we going to do about it? Though the lack of healthcare providers is a prominent issue for all healthcare entities it is not a new. For years studies have been conducted to help alleviate this issue. In a study, advanced scheduling was used to alleviate high wait times and overall continuity of patient care (Qu, 2007). The daily health care provider demand was examined along with the patient need to create a schedule that consist of adequate provider to patient ratios. As a result, healthcare providers were not overbooked, and patients had less of a wait time and more time spent with providers.
In another study, federal qualified health centers were initiated in order to decrease the shortage of health providers (Xue, 2018). These health centers provided underprivileged communities that did not have access to health care facilities, a place where they could be seen by primary providers. In 1873 The University of Iowa began providing medical services when it reached an agreement with Sisters of Mercy to operate a small hospital in the area. It began with two wards, one for women and the other for men containing four private rooms and a surgical amphitheater. In 1865 this agreement was terminated when the Sisters of Mercy moved across town and opened up Mercy Hospital. Today, the University of Iowa Hospitals and Clinics is a public -teaching hospital affiliated with the University of Iowa and a Level 1 trauma center. It has 711 beds including a 190-bed UI Children’s Hospital (About Us, n.d.). On an average day, there are close to 9,000 individuals providing care to patients, including employees, students and volunteers (About Us, n.d.). Fiscal Year 2012 Facts
There were 32,000 patients admitted to the hospital for in-patient care with 59,000 emergency room visits. In the 200 outpatient clinics of the UIHC, 977,337 clinic visits were counted. In addition to the 1,300 volunteers of UIHC, it employed during FY2012:
· 1,548 physicians, residents, and fellows
· 8,221 non-physician employees of whom 1,845 are professional nurses (About Us, n.d.)
Since U.S. News & World Report began to rank hospitals in 1990, UIHC has made the list as one of the best and has over 271 physicians ranked as “Best Doctors in America”.

This Dissertation explores the particular circumstances of Mirzapur, a region located in the
core of India. Mirzapur, with its varied terrains and abundant biodiversity, offers an optimal
environment for investigating the changes in vegetation cover dynamics. Our study utilizes
advanced technologies such as GIS (Geographic Information Systems) and Remote sensing to
analyze the transformations that have taken place over the course of a decade.
The complex relationship between human activities and the environment has been the focus
of extensive research and worry. As the global community grapples with swift urbanization,
population expansion, and economic progress, the effects on natural ecosystems are becoming
more evident. A crucial element of this impact is the alteration of vegetation cover, which plays a
significant role in maintaining the ecological equilibrium of our planet.Land serves as the foundation for all human activities and provides the necessary materials for
these activities. As the most crucial natural resource, its utilization by humans results in different
'Land uses,' which are determined by both human activities and the physical characteristics of the
land.
The utilization of land is impacted by human needs and environmental factors. In countries
like India, rapid population growth and the emphasis on extensive resource exploitation can lead
to significant land degradation, adversely affecting the region's land cover.
Therefore, human intervention has significantly influenced land use patterns over many
centuries, evolving its structure over time and space. In the present era, these changes have
accelerated due to factors such as agriculture and urbanization. Information regarding land use and
cover is essential for various planning and management tasks related to the Earth's surface,
providing crucial environmental data for scientific, resource management, policy purposes, and
diverse human activities.
Accurate understanding of land use and cover is imperative for the development planning
of any area. Consequently, a wide range of professionals, including earth system scientists, land
and water managers, and urban planners, are interested in obtaining data on land use and cover
changes, conversion trends, and other related patterns. The spatial dimensions of land use and
cover support policymakers and scientists in making well-informed decisions, as alterations in
these patterns indicate shifts in economic and social conditions. Monitoring such changes with the
help of Advanced technologies like Remote Sensing and Geographic Information Systems is

Real-World Case 19.1 Central Community Hospital hired Susan Davis as the new manager to work in the health information management (HIM) department. One of the first items Susan reviewed was the workflow process for how documents were handled between the intake department and HIM. The intake department is responsible for assuring all documentation needed for a new admission to the hospital is received from the clinic that is admitting the patient. She noticed that the two departments were managing a lot of the same work, which created duplicate documents in patient charts. She noticed that intake would send documents to HIM that they (intake) already scanned. HIM would then see the documents come through, and scan them as well, but also created a copy to send to the utilization nurse who is responsible for ensuring that reimbursement authorizations are in place and managed. When Susan asked intake about why they sent the documents to HIM, the answer was “because that’s how it has always been done.” When thinking about other ways the situation could be handled, Susan came up with the following ideas: Have intake scan the documents into the EHR, and then hand the copy to the utilization review nurse, leaving HIM out of the process for this particular situation all together; or Once intake is done with the document, do not scan into the chart, rather hand it directly over to HIM, and continue with the
- 2. rest of the process. What is the process Susan is examining? What is the step called when Susan introduces the formal process of implementing a new change? What type of analysis would Susan conduct before implementing the change? 4.Real-World Case 19.2 A small community hospital in a rural area of Alaska is seeing an increase in their patient population in both the inpatient and outpatient setting. In reviewing the data for the increase the chief executive officer (CEO) noticed that it is due to a large project—the construction of a dam on a nearby lake. In speaking with the city administrator it was determined that an additional 2,000 people are in the town for the two-year duration of this project. The hospital CEO determines that by midyear the operating units of the hospital need to make adjustments to their budgets. They have the need for an increase in employee overtime, supplies, equipment, and part-time hires. What budgets need to be looked at with this increase of population? What is the potential impact on staff time with this change to normal business operations?
- 3. How could the hospital have prepared for this situation? 5.The hospital that you work for has decided to upgrade the HIM systems. You have been asked to lead the team in the selection process. Explain the process that you will use in the selection and implementation of the system. 6.The HIM director in the following situations have been approached by administration with personnel budget concerns and given constraints to apply. Review the budgeting issues presented and make calculations as appropriate. 1. The HIM director, Lisa, has been given $15,000.00 for the overtime needed to catch up on the loose filing. If the average salary of the HIM clerical staff is $15.75, how many hours of overtime can be worked? 2. The HIM director, Carlotta, has been given $23,500.00 to hire temporary employees.
