When you are faced with medical concerns, it is important to realize that you always have the right to make your own decisions about the type of medical treatment and care you want at the end of your life. 
When you are faced with medical concerns, it is important to realize that you always have the right to make your own decisions about the type of medical treatment and care you want at the end of your life. If you ever become unconscious or unable to make those decisions for yourself, you can still make your wishes known, if you create a plan in advance. That is the purpose of Advance Medical Directives. 
WHAT IS AN ADVANCE DIRECTIVE? 
An Advance Medical Directive is a legal document that sets out your choices for medical treatment, and/or designates someone to make medical decisions for you, if you cannot. There are various types of advance directives, including living wills and durable powers of attorney for health care. Each legal document has a different purpose, but they are both equally customizable for each client. 
THE PURPOSE OF A LIVING WILL 
A Living Will is the legal document that tells all health care professionals as well as your family, the extent of the measures that should be taken to keep you alive in the event you have an incurable illness or are determined to be

permanently unconscious, such as with brain death or coma. The purpose is to allow you to make our end of life choices now, in the event you were unable to express your wishes at that time. 
A Living Will is a valuable legal tool because it gives you a voice in the important decisions regarding your medical care, while you are still able to communicate those decisions. It is important to understand that, as long as you are able to express your own wishes, your advance directive cannot be used. You will retain your ability to accept or refuse any medical treatment. However, if you become unable to make those decisions or to express them, the Living Will can speak for you and ensure your personal wishes are respected. A Living Will does not mean that you will be abandoned by your health care providers. Your health care providers will continue attending to your needs and comfort.

DURABLE POWER OF ATTORNEY FOR HEALTH CARE A Durable Power of Attorney for Health Care allows you to choose someone to be your representative regarding health care decisions, should you become temporarily or permanently incapable of doing so. The health care representative you choose can only make medical treatment decisions based on your state wishes.

WHAT TO DO WITH YOUR ADVANCE DIRECTIVE DOCUMENTS 
The best thing to do is to keep the originals in a safe and easily accessible place, while making an extra copy for yourself, just in case the original is lost or destroyed. It is also a good idea to discuss these documents with your physician and your family. You can also provide them with copies. If you are admitted to the hospital, it is a good idea to take a copy with you. 
CAN I CHANGE MY MIND? 
Yes. Your Living Will and/or Durable Power of Attorney for Health Care can be revoked at any time, as long as you retain the capacity to do so. You need only tell your doctor and family members that your wishes have changed. Then, simply tear up and throw away all copies of the document(s) you have revoked. You are now free to draft new advance directives, if you choose.
