In June 2008 Casey Family Services made the decision to replace their internally developed Case Information System (MS Access\SQL Server\Crystal Reports) with commercial off-the-shelf software. This ppt slide deck was created by me to document the current information system and business processes that a new replacement system would need to support. This set of slides focused on the demographics and workflows needed to manage the records of Foster Families.

Foster Family Service has only one level of care intensity at this time. There could be an argument that there are two levels if we consider “Respite Only” service vs. full “Foster Care” services provided by a family. Question does licencing and inspections requirements differ for “Respite Only” vs. “Foster Care” in any of the states CFS is active in?
Web Leads come from internet web page. Phone Leads come from Admin or SW entry in office or collected at a recruitment event. Web/Phone Leads can be accepted or rejected. Accepted leads become prospective families in the Foster Family Service. In future would like ability to search within lead form to see if family already exist in the system and if match found and if accepted for referral activity link to existing family rather than create new family in the system.
This form will only be visible if family is in CF status or had placements in the past in (found in closed service folders under the same case folder for the family)
