Issues Caused – The agency’s proposal had a
field mapped to the Audit Code on the ACORD
126. When this went away the mapping
stopped working.
Resolution – to use the Audit Code field on the
ACORD 125

selves which field to complete
Consequences – New field not always
updated which causes the merge to return
wording – “Not Subject to Audit” which may or
may not be true.

Your policy includes a sublimit for 3rd party Breach of Personal Data. We
recently received advice from outside counsel that upon receiving personal
information from customers, regardless of whether or not the agency
requested the information, the agency has the duty to protect it. As a result,
the underwriters now review agency websites for any free-form text fields
which allow current or potential customers to send a message to the agency
through the website. The free form text field under the “Contact Us” tab on the
your website allows customers to send a message that could include personal
data. To avoid a claim, please consider removing the text box, and replacing it
with radial options, such as contact me or inserting a hyperlink that allows the
customer to send an email directly to the agency.
