• Client name: Mohammad H Allergies: Free Of Allergies

• Dominant language: Arabic
• City of residence: Tulkarm refugee camp- Tulkarm
• Marital status: Single
• Diagnosis: drug abuse .
• Chief complaint:
The client become free from Israeli jails after he spent 17 years their
as he said in the prison they was gave him tablet to decrease
headache after this he discover that this tablet are drugs and he
became addict to this tablets.
But according to his cousin he said that Mohammad habituation to
take alcohol drinks and until now he still taking.

The length of time stay in jail
• Family dynamics: he live alone because his wife separate from him
after he leave jail and don’t accept to come back to him.

• 1. CULTURAL AND SOCIAL HISTORY:
a. Environmental factors: they live in a camp they have some of
neighbors they don’t contact with others, because he spent 17 yrs in jail.
b. relationship beliefs and practices: he want to become in relation and
to has friendship
c. Religious beliefs and practices: he don’t pray as he said he belief in
Islam .
d. Educational background: he complete to 9th class
e. Significant losses/changes: he loses his family, relation and his old
friends
f. Peer/friendship relationships: he start make new relationship and try
to contact with old friends.
g. Occupational history: now he don’t work and take monthly salary
from government.
h. Previous pattern of coping with stress: client used to solve his
problem by diplomatic behavior.

There’s no any abnormalities with client cardiovascular status
he is in a good health in this situation.

The client wake up in morning and leave to school, back and go to play

Dressing .

A 57 years old male (Z.M.I), lived in Tolkarm refugee camp .
His weight65 kg, his Dominant language Arabic,
His chief complains “drug addict".

But according to his cousin he said that Mohammad habituation to take alcohol drinks and until now he still taking. II. Predisposing Factors The length of time stay in jail • Family dynamics: he live alone because his wife separate from him after he leave jail and don’t accept to come back to him.

Environmental factors: they live in a camp they have some of neighbors they don’t contact with others, because he spent 17 yrs in jail. b. relationship beliefs and practices: he want to become in relation and to has friendship c. Religious beliefs and practices: he don’t pray as he said he belief in Islam . d. Educational background: he complete to 9th class e. Significant losses/changes: he loses his family, relation and his old friends f. Peer/friendship relationships: he start make new relationship and try to contact with old friends. g. Occupational history: now he don’t work and take monthly salary from government. h. Previous pattern of coping with stress: client used to solve his problem by diplomatic behavior.

General appearance: The client has a good care for his appearance or what he dress. Personal hygiene: The client has good hygiene for himself . Participation in group activities and interactions with others: He is good in communication with people . THOUGHT PROCESSES: The client thought was clear, and he talk about any things I ask . COMMUNICATION PATTERNS: He is a cooperative, communication level with family is very low. INTERACTION PATTERNS: The client friendly in dealing with other.

Physiological PSYCHOSOMATIC MANIFESTATIONS : Free physically health PERTINENT PHYSICAL ASSESSMENTs: Respirations: Normal breathing with slight wheezy chest regard to smoking. Skin: intact , warm no lesion or bruises , no hair loss . Degree of range of motion Good movement with slow motion skeletal muscle intact. Cardiovascular: B/P Pulse There’s no any abnormalities with client cardiovascular status he is in a good health in this situation. f. Gastrointestinal: the client diet is a full diet & there’s no allergies to any type of food

Activity/rest patterns: The client wake up in morning and leave to school, back and go to play on computer and study sometimes.
