2. Identifying data
Patient name : Ali Jassim Mohammed.
Age :10 years old.
Sex : male.
marital status: non.
Residence :Baghdad.
4. History of present illness
Fairly well with social skills.
Hardly spoken few words during the whole interview. Mother would constantly
answer all the points.
The mother is frustrated with her son's condition .
He is stubborn, if he wants something he insists on getting it, once he gets it ,
he cools down easily, but will never say sorry.
He fights with older sister, she is dominating.
Difficulty understanding and following social rules.
Very slow learner.
Selective memory he remembers songs and pictures.
5. He studied everything, works hard but forgets again.
Angary wants to throw things.
Mother beats him frequently so he expresses his anger by shouting.
He is scared of crackers.
He takes care of his things . Has a great sense of curiosity , tries to open his toy cars
and sees what is inside
When he is in good mood , he doesn’t make many mistakes, he very shy by nature
,he used to hide behind curtains.
Appetite is good.
Sleep only 5-6 hours.
6. PAST PSYCHIATRIC AND MEDICAL HISTORY:
Down syndrome.
milestones are delayed with the ability to crawl typically occurring around
8 months and the ability to walk independently typically occurring around
21 months. lose their ability to speak.
epileptic seizures.
congenital heart disease.
Problems of the thyroid gland.
He has many time hospitalization because his syndrome and complication
depression and anxiety.
7. Family history
He has a brother and sister, they have no health or psychological problems,
and the mother and father are educated.the don’t have any previous
condition like him.
8. PERSONAL AND SOCIAL HISTORY:
He entered a private school in Baghdad, but he complains about problems
with the children of the neighbors, and also that his relatives do not
approach him.
His hobbies are Repeating songs and cross words and drawing.
He hits small children.
He doesn’t want to give anything to other people. He his very attached to his
mother and prefers to be at home all the time.
13. Diagnosis
The diagnosis is mild mental retardation because the IQ level is 50 and the
child have dawn syndrome that is one of the causes of mental retardation.
