Culture drives quality—if an organization does not have a culture in which they hold themselves and others accountable, it is probable that it will not achieve and sustain high-level quality outcomes. Patient safety is defined as freedom from accidental injury; you have seen how medical errors are harmful, and that there are second victims that suffer as well. Remember quality can be defined as “the cumulative impact of all that happens to a patient while in an organization’s care” (Porter, 2012, p. 193). A Just Culture gives organizations a template to uniformly address the shortcomings or errors of those who fail. It addresses failures in four different areas, which include 1) human errors and mistakes, meaning unintentional harm; 2) carelessness or at-risk behavior—or not paying attention that results in an error; 3) recklessness or a flagrant disregard for norms where an error occurs unintentionally, but because of recklessness; and 4) those who just do not pay attention and have no regard for authority. In this Discussion, you will identify a model that a health care organization might use to improve their culture of quality and describe whether the ethical theory of Just Culture would improve the quality and why. From a blame culture to a just culture in health care Naresh Khatri Gordon D. Brown Lanis L. Hicks Background: A prevailing blame culture in health care has been suggested as a major source of an unacceptably high number of medical errors. A just culture has emerged as an imperative for improving the quality and safety of patient care. However, health care organizations are finding it hard to move from a culture of blame to a just culture. Purpose: We argue that moving from a blame culture to a just culture requires a comprehensive understanding of organizational attributes or antecedents that cause blame or just cultures. Health care organizations need to build organizational capacity in the form of human resource (HR) management capabilities to achieve a just culture. Methodology: This is a conceptual article. Health care management literature was reviewed with twin objectives: (a) to ascertain if a consistent pattern existed in organizational attributes that lead to either blame or just cultures and (2) to find out ways to reform a blame culture. Conclusions: On the basis of the review of related literature, we conclude that (a) a blame culture is more likely to occur in health care organizations that rely predominantly on hierarchical, compliance-based functional management systems; (b) a just or learning culture is more likely to occur in health organizations that elicit greater employee involvement in decision making; and (c) human resource management capabilities play an important role in moving from a blame culture to a just culture. Practice Implications: Organizational culture or human resource management practices play a critical role in the health care delivery process. Health care organizations need to develop a culture that harnesses the ideas and ingenuity of health care professional by employing a commitment-based management philosophy rather than strangling them by overregulating their behaviors using a control-based philosophy. They cannot simply wish away the deeply entrenched culture of blame nor can they outsource their way out of it. Health care organizations need to build internal human resource management capabilities to bring about the necessary changes in their culture and management systems and to become learning organizations. T he aim of this article is twofold. First, it identifies a set of organizational attributes that perpetuate a blame culture and those that foster a just culture in health care. Defining a Culturally Competent Organization Culturally competent health care, broadly defined as services that are respectful of and responsive to the cultural and linguistic needs of patients, is increasingly viewed as essential in reducing racial and ethnic disparities, improving health care quality, and controlling costs. The U.S. government considers cultural competence as a method of increasing access to quality care for all patients. The aim should be to develop systems more responsive to diverse populations. Managed care organizations view cultural competence as driving both quality and business. By embedding cultural competence strategies into quality improvement initiatives to make care more efficient and effective, clinical outcomes are improved while costs are controlled. Those in academic settings agree that cultural competency education is crucial for preparing future health care workers, although appropriate education on the topic is provided in only half of the medical schools in the United States (Betancourt, Green, Carrillo, & Park, 2005). According to the Office of Minority Health, cultural competence refers to the ability of health care providers and organizations to understand and respond effectively to the cultural and linguistic needs of patients (Office of Minority Health, 2001). Cultural competence encompasses a wide range of activities and considerations. It includes providing respectful care that is consistent with cultural health beliefs of the clients and family members. Competent interpreter services and programs to promote staff diversity are other ways in which health care organizations can increase cultural competence (Clancy & Stryer, 2001). Because communication is a cornerstone of patient safety and quality care, every patient has the right to receive information in a manner he or she understands. Effective communication allows patients to participate more fully in their care. Communicating effectively with patients is also critical to the informed consent process and helps practitioners and hospitals give the best possible care. For communication to be effective, the information provided must be complete, accurate, timely, unambiguous, and understood by the patient. Many patients of varying circumstances require alternative communication methods: patients who speak and/or read languages other than English; patients who have limited literacy in any language; patients who have visual or hearing impairments; patients on ventilators; patients with cognitive impairments; and children. The hospital has many options available to assist in communication with these individuals, such as interpreters, translated written materials, pen and paper, communication boards, and speech therapy. It is up to the hospital to determine which method is the best for each patient. Various laws, regulations, and guidelines are relevant to the use of interpreters. These include Title VI of the Civil Rights Act, 1964; Executive Order .
