1. • ID cards shall be used as proof of identity and DoD affiliation to
facilitate access to DoD facilities and systems. Additionally, ID cards
shall represent authorization for entitled benefits and privileges in
accordance with DoD policies.

2. Additional Information
• New Check-In: Before being issued a CAC, personnel should contact ISD at 5717 to ensure
they obtain or verify their email address to ensure synchronization of their account and
proper function of their CAC.
• Dependent ID: The dependent’s sponsor must be present or fill out and sign a DD Form
1172.
• Adding new family members: To add a newborn, the sponsor must provide an original Birth
Certificate and Social Security Card. For adoptions: The sponsor must provide the child's
Birth Certificate and a copy of the Final Adoption Decree.
• Full-time College students: College students must provide a letter from their school
registrar's office stating that they are enrolled full time in an accredited college in pursuit of
an Associate Degree or higher to continue receiving dependency benefits extending to the
age of 23.
• Dual-Military Couples: Dual-Military members must provide a copy of their Marriage
Certificate, Social Security Card, and a photo ID. Both members will need to be present or
have a Power of Attorney.
• Recently Married: The sponsor must be present or the new spouse must present a valid
Power of Attorney with the following: Gov't issued photo ID, Original/Certified copy of
marriage certificate, and social security card. If dual military, both members must be
enrolled and listed under each others account in DEERS.

• Do I need to be in Uniform of the Day in order to get a CAC? Yes.
• My CAC is blocked do I need to make an appointment? No, walk-ins are accepted.
• I lost my CAC/Dependent card, what do I do? You need to report to Security Desk
Sergeant (BLDG 1655 extension 4645) and complete a Lost Statement form.
• Where are you located? We are located on Admin Hill BLDG 760 (extension 4879)
same building as MWR, next to Fleet Family Support Center and Bulkeley Hall.
• New Check-In and need an ID card? You must have the proper documentations (as
listed on the first slide) and an appointment to save time.

current account and if you are eligible for a new CAC.
• What do I need to get my dependent an identification card? You will require an DD
