1. Critical Incident In Social Work
Introduction Social workers deal with diverse group of people while in their call of duty. The social work theory and practices guide the social
workers in their work by providing them with the skills they need. Social work incorporates other aspects as diversity, ethics, and values and thus
social workers require skills that will enable them to educate people. It is important that social workers embrace diversity, follow the ethical rules and
uphold high standards of value. This essay will highlight an incidence in a supported accommodation setting where issues of diversity, ethics, and
values were clearly observed. The incident will be critically analysed using a critical incident analysis pro â forma and incorporate it with the PCS

2. Reflection Upon A Critical Incident Essay
This paper will reflect upon and explore a critical incident which occurred whilst attending a clinical placement. Reflective practice has become
very popular over the last few decades throughout a variety of professions. In some professions it has become one of the defining features of
competence. The wide spread utilization of reflective practice is due to the fact that it 'rings true' (Loughran, 2000). Within different disciplines, what
is understood by reflective practice varies considerably (Fook et al, 2006). Despite this, some agreement has been achieved. In general, reflective
practice is understood as the process of learning through and from experience towards gaining new insights of self and/or practice (Boud et al 1985;

The model encourages the person to think systematically about and experience or activity and allows the opportunity to recognise areas that can
be improved upon or developed. The model takes the form of a cycle, which uses a six step approach covering a description, feelings, analysis,
evaluation, conclusion and finally action plan. My choice of model, over Johns (2000) model for example, was due to its clear structured cycle
which allows the opportunity to renter a stage in order to try and improve upon a situation. Where as Johns (2000) model, does not allow this
opportunity although it is a useful model as a guide for analysis of a critical incident. Although, I will have to develop the model to ensure that I

my clinical practise placement. The area of drug administration is an important role for nurses and therefore I will incorporate research evidence,
policies and guidelines surrounding drug administration, discuss how the area can be developed and applied in professional practise. The National
Patient Safety Agency (NPSA 2010), defines a drug error as 'any preventable event that may cause or lead to inappropriate use of patient harm.
Although not all drug errors have lead to patient harm it is important to recognise that if a mistakes has been

3. Essay on Reflection on a Critical Incident
The reason for this essay is to reflect on a critical incident experience during my six week placement as a student nurse on an orthopedic ward. To
explore an event as a critical incident is a value judgment, and the basis of that judgment is the significance attached to the meaning of the incident.
Critical incidents are created or produced by the way we look at a situation. Tripp (1993)
The incident chosen has made an impact on me due to the fact the side effects of surgery can be very critical to a patient's life, as would be
demonstrated in the critical incident chosen (D.V.T).Deep vein thrombosis.
There are various reflective models written by various theorists and they include: Atkins and Murphy (1994), Stephenson (1993),...show more content...
"One may also reflect on practice while one is on the midst of it. This process involves both reflections in action and reflection in practice (Johns 2000).
Schon states that reflection in action consists of on the spot surfacing, criticizing, restructuring and testing of intuitive understanding of experience
phenomenon (Schon 1983) P.241.
"Reflective learning involves assessment and reâassessment of assumptions and critical reflective occurs whenever underlying premises are being
questioned".(Williams 2001) P.29.
In choosing Gibbs reflective model it would be illustrated in the six headings which guide me through my reflective process. These headings include:
(1.) Description â what happened?
(2.) Feelings â what were you feeling?
(3.) Evaluation â what was good or bad about the experience?
(4.) Analysis â what sense can be made of the situation?
(5.) Conclusion â what else could you have done?
4. (6.) Action plan â if the situation arises again what would you do?
Description:
This critical incident took place during my first sixâweek placement on the ward (Eleanor east). My rationale for this critical incident is because of the
impact it had on me. I did not know that the side effects of hip replacement surgery could result in DVT (deep vein thrombosis), which could be very
critical physically and mentally.

Reflection has its importance in clinical practice; we always seek to be successful and that can be achieved by learning every day of our life through
experiences we encounter. In that way we can reconsider and rethink our previous knowledge and add new learning to our knowledge base so as to
inform our practice. Learning new skills does not stop upon qualifying; this should become second nature to thinking professionals as they continue
their professional development throughout their careers (Jasper, 2006).

One research summary listed on AHRQ website under patient safety is a device that has potential in reducing the incidence of ventilatorâassociated
pneumonia (VAP) (Collard & Saint, 2010). Continuous aspiration of subglottic secretions (CASS) is an apparatus that has been shown to decrease the
incidence of VAP in certain patients. Included in this piece is a description of VAP, how CASS can help improve patients at risk for VAP and a patient
care situation regarding clients receiving mechanical ventilation.
VentilatorâAssociated Pneumonia
Pneumonia is the second most common hospitalâacquired infection and is the leading cause of death due to nosocomial infection in the United States
(Augustyn, 2007). A patient who is intubated with an endotracheal tube (ETT) is at increased risk of developing pneumonia. The bacteria colonizing
the oropharynx can move into the lower respiratory tract because the ETT provides a direct route into the lower airway (Craven & Hjalmarson, 2010).
This type of pneumonia is called ventilatorâassociated pneumonia (VAP), it occurs in patients receiving mechanical ventilation for an extended period
of time. Ventilator associated pneumonia can be categorized as either earlyâonset or lateâonset. Earlyâonset VAP occurs between 48 to 96 hours and is
usually caused by Haemophilus influenza, an antibiotic sensitive communityâacquired organisms (Collard & Saint, 2010). Lateâonset VAP is caused by

I agree the first responder has to be offered the ability to overcome their experience with a traumatic event. Providing early intervention is ideal in
helping responder deal with the traumatic event, these intervention includes critical incident stress debriefing (CISD) and critical incident stress
management (CISM), offering first responders alternative to dealing with their stress will led to a reduction in mental breakdowns. The programs that
are now available to first responders has to be made mandatory across the board, doing this takes the option out of the hand of the responder and puts it
in the hands of the department to enforce mandatory debriefing once exposed to any traumatic event.

Richard and Parker (1995) argue that reflecting and analysing the incident the practitioner is able to consider how the incident may have been
managed differently by applying other knowledge and consequently enable the practitioner to move forward and consider a future situation
differently. For the purposes of this piece of work I will be analysing an observation of a meeting between a male and a GP.
When I learnt that I was going to be shadowing at a clinic for service users with drug and alcohol problems I was quite worried and intrigued as to
what would take place. Then I realised I had to be holistic and also realistic, and although I am not close to anyone that has a heroin, crack cocaine or
even an alcohol addiction I have been surrounded by people that have used certain drugs such as cannabis or cocaine. I also feel that I will not
always know that this is the case as not everyone demonstrates through their behaviour that this is an issue for them, also some individuals feel
ashamed as drug and alcohol use is frowned upon and individuals will deny using drugs. Some individuals explain that they use drugs as a way of
numbing their problems or to gain confidence and self esteem. Beliefs about oneself and about the role of drugs or alcohol in one's life are sometimes
called existential models (Greaves, 1980). Khantzian (1985) has proposed that addicts use drugs to offset

I was pleased to see that child x was optimistic to participate in my lesson as he showed great enthusiasm. Nonetheless, my incident also reveals the
pessimistic point of my incident. Although child x was engaged in the lesson, his behaviour was disruptive and he interrupted my teaching which did
not benefit the pupils learning. Due to the fact that I am a reflective thinker, I could have thought about possible alternatives and considered the
incident from the child's point of view. For instance, rather than moving child x next to the teaching assistant, I could have considered how he felt and
the reasons why he did not put his hand up during class discussions. If I chose to make an alternative decision, what would the outcome be? I continue
to ask myself a series of questions.
After my maths lessons, I proposed a series of questions to myself: why does he call out during my lessons? Why do I have the expectation that he
should put his hand up? Should I have imposed a different solution rather than have child x sit next to the teaching assistant? Could I have handled
the situation more effectively? I continued to ask myself a series of questions which enabled me to reflect on what I did and why I did it. Although I
may not be able to find a solution to my critical incident, I aim to explore and analyse it by reflecting on my teaching practice. Dewey (1933) in

Furthermore, upon assessing Christina I noticed that she was experiencing breathlessness and showing signs of distress. According to Kisiel and
Perkins (2006) recording vital observation is an important aspect of nursing care as this provides the physiological state of the patient. At this point my
main priority was to assess the patient's baseline. I also felt it was unnecessary to disturb Christina with questions regarding her history and identity. As
this could have leaded the patient become more breathlessness.
However, I was distracted by the patient's breathlessness and distress. Therefore the consequence of my actions was that I wrote the vital signs on the
wrong chart. The lack of proper documentation could have lead a negative impact on the patient health and can ultimately cause fatal consequences.
The NMC (2008) requires practitioners to make correct documentation as this could cause dangerous consequences for patients and result in further
disciplinary action.
Looking back, Nurse James would have been held accountable for my action. The NMC (2008) clearly states that a registered nurse is obligated to

13. Analysis Of Leicestershire 's Police Force Essay
The way police manage critical incidents is of upmost importance. An incident that is manged well, will raise public confidence in the police, however
if the incident is managed poorly it can have devastating effects on the victim and the general public will subsequently lose confidence in the police.
ACPO's definition of a critical incident is as follows "Any incident where the effectiveness of the police response is likely to have a significant impact
on the confidence of the victim, their family and/or the community" (Constabulary, December 2007) Any police officer can notify an incident as
possibly critical, therefore, police officers must always be aware of the potential for any incident to escalate and become critical. A critical incident
can include; Suspicious, unexplained deaths, Homicides, Incidents requiring a police firearms response, Vulnerable persons missing from home,
Domestic Violence, Crimes involving particularly vulnerable or young victims and hate crimes, which will be our focus. For our essay, we will be
critically analysing Leicestershire's police force's 'critical incident' management of the case of Fiona Pilkington which is a case of antiâsocial behaviour
that deeply affected public confidence. Whenever a critical incident is identified, it must be addressed sharp and efficiently. Reassuring, rebuilding and

should be taken when moving patients to ensure the stay in place as it can be very uncomfortable and difficult to reinsert a cannula on a patient as
most of them are oeadematous. As I was involved in the care of the patient I had to explain to the wife why her husband was connected to a ventilator

cannot breathe on your own, you will need to be attached to a ventilator (See appendix 3). Being helped to breathe by a ventilator means that you

is likely you will have an endotracheal tube from the ventilator to your mouth or nose. The tube will usually be held in place behind your neck as
was the case with my patient. However, if you need help with breathing for more than a few days, you may have a short operation called a
tracheostomy. This replaces the tube in your mouth with a shorter tube that is placed directly into your trachea. As well as being more comfortable,

20. Reflection On A Critical Incident In PGCE
Introduction:
The way in which we reflect upon a situation, shaped by the interpretation of a significant event is defined as a critical incident (Tripp, 2011).
Reflecting has been crucial within my first two phases of PGCE as I have been able to value experiences and make judgements to improve further. My
home school practice was graded an 'outstanding' mainstream primary school (Ofstead, 2015), working 19 percentage points above their local
authority in reading, writing and maths (School performance tables, 2017). The school's numbers are below average for free school meals in the area
and they have an average percentage of special educational needs children. Pupil premium percentages are currently at 15 percent out of the 218
children on roll.
This assignment will further develop critical incidents that occurred throughout phases one and two in my home school. School, subject and
pedagogical knowledge incidents will be critically evaluated against theories and practice. According to Banks, Leach and Moon (2005) the
professional teaching knowledge vision consists of all three of these areas. A series of conceptual frameworks have been used to critically analyse
each incident further including: Brookfield's lenses, Banks Leach and Moons' model of reflection and Schon and Eraut (1995) reflection on, in and of
action.
I will primarily analyse my school knowledge incident. I have chosen this incident as it required me to reflect afterwards about how the situation could
