British Columbia launched a massive review of 8,000
diagnostic tests performed by four unqualified radiologists
in 2011. Three patients died and another nine
were harmed after being misdiagnosed.

in February by Dr. Doug Cochrane, chair of the BC Patient Safety and
Quality Council, after concerns were raised around the quality of
