proper prescription.
• Test reports and payment receipts
including X-ray, CT films
– Settlement within a 20-30 days after
complete submission

Claim Process
 Intimate the claim duly on toll free
 Get treated as prescribed till you get fit
 Get the accidental claim form signed from hospital mentioning weekly rest
period, All original prescriptions, bills, reports. Leave certificate from
employer
 Apply for Reimbursement, it takes 20-30 days for settlement
