A 21-year-old female presented to the emergency department after a motor vehicle collision with left arm pain from striking the steering wheel. Her left upper extremity was swollen and deformed without an open fracture. Reduction was achieved by placing the forearm in supination and flexing the elbow over 90 degrees to relax the biceps, then manually repositioning the radial head anteriorly and reducing the angulated ulnar shaft with pressure. Orthopedics was contacted to discuss urgent operative repair for a Monteggia fracture of the proximal ulna and anterior dislocation of the radial head.
