Reason for Referral:
TS has been referred to the Committee on Preschool Special
Education by his parents due to concerns with sensory and
social emotional development. Mrs. P and Mr. S served as
the informants and consented to the evaluation process.
Interview took place at the school.
2. Birth History:
According to Mrs. P., the pregnancy was planned and she was
compliant with prenatal treatment. The gestation was full
term with no complications. TS was born at NYU via vaginal
delivery. Neo natal stay was routine.
Developmental Milestones:
According to Mrs. P. , TS walked at 15 months, toilet
trained at age 2.5, rolled over between the ages of 2-3
months, sat up without support at 5 months, crawled at 11
months, stood without support at 13 months and pulled self
to stand at 10 months.
Sensory Motor:
Both parents have concerns about TS’s sensory development.
During the interview, Mrs. P reported that TS is sensitive
to loud noises and that he flaps his hands when he becomes
excited or overstimulated. According to Mrs. P this happens
frequently.
Personal Skills:
Mrs.P indicated that TS us able to eat using a fork and
spoon. He is also toiled trained. According to Mrs.P, TS is
currently unable to dress himself.
Medical History:
Mrs. P indicated that TS is a healthy child with no history
of ear infections or high fevers. He was admitted to the
hospital in 2005 as a result of falling and hitting his
head. According to Mrs. P, no serious injuries were found.
Immunizations are up to date and TS is currently in good
health.
3. Learning Skills:
TS presently attends preschool. Mrs. P stated this is TS’s
first time attending school. Mrs. P and
Mr. S are not concerned about any learning difficulties at
this time. According to Mrs. P, TS is able to sit still and
listen to a story for ten minutes. Mrs. P stated TS is able
to count from one to twenty and is able to recognize
numbers and letters. He also knows colors and shapes.
Social Emotional:
Mrs. P described TS as a happy, gentle, and inquisitive
child. She reported that TS prefers to be with other people
rather than alone and that he enjoys visiting with his
cousins. The school TS attends has reported difficulties
with socialization and transitions to Mrs. P and Mr. S.
However, both parents have a different perception than that
of the school and Mrs. P said she and her husband have seen
great improvement this year. Mrs. P indicated that TS is
slightly more excitable compared to children his age and
that he does not engage in pretend play.
Family History:
TS resides with his mother and father. Mrs. P (age 40) has
a college education and is employed as a corporate planner.
Mr. S (age 47) also has a college education and is employed
as a construction executive. Due process and parental
rights were explained to Mrs. P and Mr. S.
