Abstract
In this memoir told in four episodes, the author, an Italian child psychiatrist and family psychotherapist who lives in Canada, revisits his episodic encounters with his father from their first meeting to his father’s death. The first episode recalls how he met his Italian father for the first time in Brazil as an adult. He wrote a memoir about that emotional encounter, entitled “Strangers No More” (1), published in Italian and several other languages, then integrated as the concluding chapter of his book on cultural family therapy, A Stranger in the Family (2). After some 20 years of sporadic father-son and family encounters, the author revisits his relationship with his father and his Brazilian family, following his father’s death in the second episode. Deferred and episodic on one hand, provocative and profound on the other hand, the third episode describes the enigma of his father’s life with the metaphor of “the third bank of the river” (3), after a classic Brazilian short story. Towards the end of his life, with a final, startling revelation about the mysteries of his absence, his father allows the author to recognize retrospectively that they have been familiar but unknown to each other. In the fourth episode, this series of episodic yet profound encounters with the man who became an intimate stranger is understood as nothing less than an event in the author’s life, opening possibilities and transforming everything. The memoir closes with a coda of reflections for therapists on slow thought and evental therapy that has no other object than itself (4).

Telepsihiatria
Dacă m-aș plimba printr-un magazin de cuvinte
(bineînțeles, purtând mască pe față), aș descoperi
produse de import din sfera anglofonă: Telemedicine,
Telehealth, Telemental health, Teleconsultation, e-Pre-
scribing, Distant Site, Originating Site, Teleconferencing,
Videoconferencing, Electronic Health Record, Synchronous
etc. Câteva dintre aceste cuvinte au ajuns și în spațiul
nostru mioritic și se folosesc în traducere românească
(aproximativă), pe când altele sunt preluate ca atare,
din limbajul de lemn al unui Shakespeare transatlantic.
De exemplu: telemedicina. Să nu considerăm teleme-
dicina o specialitate medicală separată. Ea folosește
telecomunicațiile pentru a transfera informații de
la pacient la medic, în cadrul activității de acordare
a asistenței medicale în condiții improprii (dezastre
naturale, pandemii, zone izolate etc.). Vine de aseme-
nea și în ajutorul persoanelor cu mobilitate redusă, al
vârstnicilor, al celor cu handicap permanent. Este intens
folosită și într-o serie de specialități care își bazează
diagnosticele pe imagistică (teleradiologia, teleder-
matologia, tomografia în coerență optică, imaginile
histopatologice etc.). Telemedicina încorporează pe zi
ce trece tot mai multe servicii și programe la distanță
în tratarea pacienților, inclusiv în psihiatrie.
Telepsihiatria este deja recunoscută în multe țări,
ca o practică uzuală care mărește accesul la asistența
de specialitate, încurajează autonomia pacientului, îi
protejează confidențialitatea, reduce timpii de așteptare
și crește aderența la tratament.
A fost implementată prima dată la mijlocul anilor
’90, în Carolina de Sud, de psihiatra Jill Afrin, în cadrul
unui program de sănătate mintală pentru persoanele
cu deficiențe de auz. De atunci a început să fie folosită
în asistența unor tulburări psihice precum depresia,
tulburarea de stres posttraumatic (PTSD), tulburarea
hiperactivă cu deficit de atenție (ADHD), tulburările
anxioase, adicțiile, în prezent cuprinzând arii tot mai
extinse din patologia psihiatrică.
Am observat deseori că dezastrele exacerbează
afecțiunile psihice preexistente, iar în momentul de
față ne confruntăm din nou cu o provocare de acest fel,
în care pandemia de COVID-19 ne supune unei încercări
nemaiîntâlnite până acum, prin extinderea mondială
a morbidității și mortalității. Dezastrele anterioare
(tsunamiul din Oceanul Indian în 2004, uraganul Ka-
trina din 2005, epidemiile de SARS, MERS, Ebola etc.)
au fost mult mai circumscrise și localizate geografic.
Mai mult, constatăm că răspunsul la actualul dezastru
este minimal, în termeni de tratament sau prevenție.
Acțiunile (uneori eroice) ale corpului medical nu furni-
zează încă un dosar convingător de evidențe medicale,
care să fie încorporate apoi în ghidurile clinice ale
diverselor specialități.
Și totuși…
…criza pandemică ce a provocat consecințe în domeniul
sănătății mintale a generat, pe lângă disfuncționalități
sau blocaje, și noi oportunități pentru psihiatrie.
Astfel, declararea stării de urgență la 16 martie 2020
a deschis posibilitatea limitării deplasării doar către
obiective sau activități punctuale, în ideea limitării
transmiterii intracomunitare a noului tip de corona-
virus. Mai mult, după câteva zile, provocând în special
nemulțumirea pacienților, ordinul Departamentului
pentru Situații de Urgență a impus ca activitatea în
serviciile ambulatorii să se suspende pentru toate
consultațiile și tratamentele care pot fi reprogramate.
Acest ordin a generat o adevărată criză în sistemul
medical, inclusiv în cel psihiatric.
La solicitarea expresă a unor organizații medicale pro-
fesionale, inclusiv cea a psihiatrilor (Asociația Română
de Psihiatrie și Psihoterapie), guvernul a răspuns nevoilor
populației, destul de prompt (vezi HG 252/30.03.2020),
permițând introducerea consultațiilor la distanță,
online, prin metode de telepsihiatrie, în rețeaua ambu-
latorie. De asemenea, serviciile medicale (consultații,
prescripții medicale, bilete de trimitere) s-au putut

acorda fără a mai fi nevoie de utilizarea cardului
național de asigurări de sănătate. A rămas interzisă
prescrierea online a substanțelor psihoactive și a
derivaților de benzodiazepine (substanțe menționate
în TAB II și în TAB III ale Legii nr. 339/2005).
În prezent, criza pandemiei, prin distanțare fizică
(de multe ori confundată cu distanțarea socială) și
incertitudine financiară, la care se adaugă neliniștea
permanentă legată de sănătatea proprie, a familiei,
a prietenilor, generează stres, tulburări de adaptare
sau chiar stări emoționale asimilabile anxietății
sau depresiei. Acest lucru a dus la creșterea cererii
de asistență psihiatrică în camerele de gardă a mul-
tor persoane care nu prezentau tulburări din grupa
urgențelor psihiatrice.
Din păcate, în aceste vremuri, pacienții cu psihoze
prezintă un risc crescut de recădere, din cauza refu-
zului de a fi internați, a lipsei dispensarizării active
și a controlului lunar al complianței la tratament, a
sistării programelor de recuperare/resocializare și,
poate cel mai important pentru ei, a deprivării de
relația și suportul emoțional oferit de echipa tera-
peutică. La fel de dramatică poate deveni și situația
dependenților de droguri sau alcool, reflectată și în
dinamica ascendentă a internărilor pentru cazurile
de sevraj.
Să sperăm că aceste constrângeri în acordarea
asistenței în specialitatea noastră vor produce în viitor
și roade bune. În primul rând mă refer la perpetuarea
achizițiilor de telepsihiatrie (telemental health) și în
perioada postpandemie. Este democratic să existe
un acces egal la tămăduirea minții și sufletului și a
persoanelor din mediul rural, și a persoanelor cu han-
dicap cognitiv, și a celor în vârstă, și a celor „cazați”
în spații juridice…
Scepticismul față de telepsihiatrie parcă începe să
își facă simțită prezența, odată cu încetarea stării de
urgență. Ar fi păcat ca integrarea tehnologiilor infor-
matice în practica curentă, acoperind o arie vastă de
integrare electronică a activităților medicale (colec-
tarea și stocarea datelor medicale, foaia de observație
electronică, posibilitatea consultului integrat prin
comunicare interdisciplinară, prescrierea electronică
etc.), să fie părăsită după calmarea pandemiei actuale.
Incontestabil, practicarea telepsihiatriei pe scară extinsă
ridică probleme etice, logistice și educaționale. Aș zice
că prioritatea constă în deprinderea profesioniștilor
din sănătatea mintală a unor cunoștințe avansate din
cadrul tehnologiilor informatice.
Oferind majorității clinicienilor un program de
pregătire și antrenament al competențelor tehno-
logice, în foarte scurt timp se pot depăși decalajele
digitale între diferitele generații de psihiatri. Chiar
dacă, în pregătirea cadrelor, din punctul de vedere al
tehnologiilor aplicate în telepsihiatrie se întrevede o
diminuare a lipsei de competențe digitale, totuși în
România rămâne deschisă problema beneficiarilor
aparținând categoriilor defavorizate prin sărăcie,
analfabetism, domicilii izolate fără electricitate
și telecomunicații etc., pentru care asistența psihi-
atrică la distanță este încă utopică.
Sondaje extinse la grupe populaționale diverse ar fi
necesare pentru a testa satisfacția/insatisfacția față de
acest tip de asistență psihiatrică atât a pacienților și
a familiilor acestora, cât și a psihiatrilor, asistenților
sociali, a persoanelor de îngrijire etc.
Trebuie să ne asigurăm că, pentru viitorul care ne
așteaptă, procedurile bazate pe tehnologii informatice
pot să treacă testul timpului și să ne fie un sprijin nu
doar pe durata actualei pandemii, ci și în practica
clinică de rutină. Pentru un sistem psihiatric modern
și reformat ne interesează în egală măsură ajutorul
pe care telepsihiatria îl poate furniza în evidențierea
deficitelor din managementul de caz, precum și în
evidențierea disfuncționalităților aparținând lanțului
terapeutic.

The new context that appeared both in Romania and
worldwide – the COVID-19 pandemic – tested the
healthcare system in all countries. Considering that the
epidemics and pandemics can influence the emergence
of psychiatric disorders, the population’s needs regarding
metal healthcare are growing. The emergency state decreed
on a national level along with the military enactments
that followed-up restricted travel and therefore made
the healthcare access difficult. One of the legislative
measures that diminished the difficulties of accessing the
psychiatric hospitals was the introduction of legalized
telemedicine, implicitly telepsychiatry. From a structural
point of view, a key role in facilitating the healthcare
access is represented by the resources distribution at
the national level. The present study analyzed both the
human resource distribution as well as the psychiatry
hospital beds, by types of care, on a socioeconomic
macroregional and geographical zone level in Romania, as
defined by Eurostat (macroregions – MR). The admission
to hospitals trend in Romania was also studied.

Contextul nou apărut atât în România, cât și pe plan mondial
– pandemia de COVID-19 – a pus la încercare sistemul medical
la nivelul tuturor țărilor. Având în vedere faptul că epidemiile și
pandemiile pot sta la baza apariției unor patologii psihiatrice,
nevoile populației cu privire la sănătatea mintală sunt în
creștere. Starea de urgență decretată la nivel național împreună
cu ordonanțele militare ulterioare au restricționat deplasările
și astfel au îngreunat accesul la serviciile de sănătate. Una
dintre măsurile legislative care au diminuat dificultățile create
de accesul în spitalele de psihiatrie a fost introducerea cadrului
legal pentru efectuarea telemedicinei, implicit a telepsihiatriei.
Din punct de vedere structural, un rol-cheie în facilitarea
accesului la serviciile de sănătate mintală este reprezentat
de distribuția resurselor la nivel național. Studiul de față a
analizat atât repartiția resurselor umane (medici psihiatri),
cât și a paturilor de psihiatrie, pe categorii de îngrijiri, la nivel
de macroregiuni (MR) socioeconomice și arii geografice din
România, raportate de Eurostat. A mai fost studiată și dinamica
internărilor în unitățile spitalicești de psihiatrie la debutul
pandemiei în România.

Introducere
Problemele de sănătate publică din România au
un impact major asupra prevenției și tratamentului
tulburărilor psihice ale locuitorilor țării. Orice strate-
gie de intervenție care vizează ameliorarea sănătății
populației implică și măsuri de protejare a sănătății
mintale, idee susținută de mulți ani de OMS(1)
, prin
sloganul: „Nu există sănătate fără sănătate mintală”.
Este cunoscut faptul că epidemiile pot sta la baza
apariției unor patologii psihiatrice sau a unor recăderi
în cazul pacienților cu diagnostic psihiatric(2)
.
În contextul nou apărut în România în 2020, repre-
zentat de pandemia de COVID-19, este adecvat să ne
preocupăm de nevoile și de resursele populației care
are deja sau va prezenta tulburări psihice.
Astfel, pe durata pandemiei, dar și după termina-
rea acesteia, nevoile cu privire la sănătatea mintală
sunt în creștere, ceea ce poate suprasolicita resursele
existente de acordare a îngrijirilor de sănătate min-
tală disponibile, cele mai problematice fiind zonele
în care accesul este limitat din cauza infrastructurii
medicale sărace sau a personalului medical de spe-
cialitate insuficient.
Primul caz declarat de COVID-19 pe teritoriul Ro-
mâniei a fost pe 26.02.2020, urmat de o creștere lentă,
dar susținută a numărului de cazuri, care a generat o
serie de măsuri legislative menite să țină sub control
pandemia la nivel național.
De la începutul lunii martie (11.03.2020, data de
declarare a prezenței pandemiei de COVID-19, inclusiv

pe teritoriul UE și, implicit, pe cel al României), s-a re-
marcat o creștere a solicitărilor de asistență medicală
din partea pacienților în ambulatoriile spitalelor și în
unitățile de primiri urgențe, cauzată și de creșterea
prezentărilor cu simptomatologie anxioasă și de-
presivă. Această simptomatologie era în majoritatea
cazurilor de intensitate ușoară sau moderată și era
prezentă atât la pacienții cunoscuți cu antecedente
psihiatrice, cât și la cei care au luat contact prima
dată cu serviciile de psihiatrie.
Declararea stării de urgență prin Decretul
Președintelui României nr. 195 din 16 martie
2020 privind instituirea stării de urgență pe te-
ritoriul României(3)
a deschis posibilitatea emiterii
ordonanțelor militare. Astfel, Ordonanța Militară nr. 2
din 21.03.2020 prevedea restricționarea deplasării
persoanelor între orele 22 și 6, dar în afara acestor ore
reglementarea era doar sub forma unei recomandări
de limitare a deplasărilor.
La 23.03.2020, prin Ordinul nr. 74527 al Departa-
mentului pentru Situații de Urgență, statul român a
hotărât ca activitatea în serviciile ambulatorii să se
suspende pentru toate consultațiile și tratamentele
care pot fi reprogramate. Din cauza numărului foar-
te mare de pacienți care solicitau servicii medicale
în ambulatoriu, camerele de gardă ale spitalelor au
trebuit să suplinească acest tip de consultații. Ast-
fel, s-a prezentat un număr mai mare de pacienți la
camerele de gardă, care nu necesitau consultații de
psihiatrie de urgență.
Prin emiterea Ordonanței Militare nr.
3/24.03.2020 privind prevenirea răspândirii
SARS‑CoV‑2(4)
au fost luate măsuri de distanțare socială
a populației (ex.: interzicerea circulației persoanelor
în afara locuinței în afara celor 10 motive prezentate
în declarația pe proprie răspundere, permisiunea
circulației persoanelor de peste 65 de ani doar în
intervalul orar 11-13).
Ordonanța Militară nr. 4 din 29.03.2020 privind mă-
suri de prevenire a răspândirii SARS-CoV-2(5)
a revenit
asupra unor măsuri, permițându‑le persoanelor de
peste 65 de ani să-și părăsească locuința, din motive
medicale, în afara intervalului orar menționat anterior.
La solicitarea expresă a unor organizații medicale
profesionale, inclusiv cea a psihiatrilor, reprezentată
de Asociația Română de Psihiatrie și Psihoterapie,
Guvernul României a răspuns destul de prompt, prin
emiterea HG 252/30.03.2020, prin care a decis să permită
consultațiile de telemedicină (implicit și cele de telepsi-
hiatrie) de către specialiștii din rețeaua ambulatorie(6)
.
De asemenea, serviciile medicale se pot acorda fără a
mai fi nevoie de utilizarea cardului național de asigu-
rări de sănătate. În momentul de față se pot prescrie
online psihotrope, cu excepția substanțelor psihoactive
și a derivaților de benzodiazepine (cuprinse în TAB II
și TAB III ale Legii nr. 339/2005)(7)
, adică stupefiantele
și unele benzodiazepine care pot genera dependențe.
Ordinul ministrului sănătății nr. 555, în vigoare de
la 7.04.2020, privind aprobarea Planului de măsuri
pentru pregătirea spitalelor în contextul pandemiei
de COVID-19, a Listei spitalelor care asigură asistența
medicală a pacienților testați pozitiv cu SARS‑CoV-2 în
faza I și în faza a II-a și a Listei cu spitalele de suport
pentru pacienții testați pozitiv sau suspecți de infecție
cu SARS-CoV-2, a impus triajul epidemiologic la in-
ternare sau consultație la nivel de unitate de primiri
urgențe (sau cameră de primiri urgențe) din fiecare
spital, indiferent de specialitate(8)
.
Prin Ordinul Departamentului pentru Situații de
Urgență nr. 74553 din 7.04.2020, pentru sistemul me-
dical s-a impus suspendarea internărilor care nu re-
prezintă urgențe medico-chirurgicale(9)
. De asemenea,
s-au suspendat consultațiile care pot fi reprogramate,
din structurile ambulatoriilor de specialitate ale
spitalelor publice și private.
Decretul Președintelui României nr. 240 din 14
aprilie 2020 privind prelungirea stării de urgență pe
teritoriul României(10)
a menținut toate prevederile
privitoare la acordarea asistenței medicale instituite
prin Hotărârea de Guvern nr. 252/2020.
Intrarea în țară a unui număr mare de oameni din
state cu număr mare de cazuri de COVID-19, care
în marea ei majoritate reprezintă o populație fără
asigurări de sănătate în sistemul de stat (CNAS), a
reprezentat sursa de import a SARS-CoV-2, favorizând
creșterea rapidă a îmbolnăvirilor.
De exemplu, în perioada 26 februarie ‑ 18 martie 2020,
din totalul de 261 de cazuri confirmate, 127 au fost
înregistrate la persoane care au revenit în România
din străinătate, cea mai frecventă țară de proveniență
fiind Italia (cu o pondere de 66% din cazurile de import),
urmată de Franța, Germania și Spania (aproximativ
5% din cazuri) și de Marea Britanie și Austria (cu 4%
din cazuri), conform datelor furnizate de Institutul
Național de Sănătate Publică – CNSCBT(11)
.
Până la momentul emiterii HG nr. 252/30.03.2020,
pentru persoanele revenite în țară singura posibilitate
de îngrijire a unei afecțiuni psihiatrice a fost apelul
la consultațiile în sistemul privat (costisitoare) sau la
camerele de primiri urgențe ale spitalelor de psihiatrie.
În aceste circumstanțe a devenit acută problema
accesului la îngrijirile de sănătate mintală (SM), care
în țara noastră mai este încă limitat de unele bariere
(structurale sau atitudinale), precum:

Amplasarea geografică optimă și numărul suficient
de specialiști constituie criterii importante de eva-
luare a gradului de acces al populației la îngrijirile
pentru tulburările psihice. Pentru evaluarea accesului
sunt utile delimitările pe teritoriul UE ale regiunilor
socioeconomice, denumite macroregiuni (MR). În
România sunt descrise geografic patru macroregiuni:
macroregiunea 1 (MR1 – 4,879 milioane locuitori),
macroregiunea 2 (MR2 – 5,620 milioane locutori),
macroregiunea 3 (MR3 – 5,256 milioane locuitori),
macroregiunea 4 (MR4 – 3,720 milioane locuitori),
conform Institutului Național de Statistică (INS)(13)
.
Scopul cercetării de față a fost în primul rând ana-
liza sintetică a resurselor de care dispune sistemul
de sănătate mintală din România, dorind în acest fel
să evaluăm nivelul de acces la serviciile de sănătate
mintală în macroregiunile din România, în vederea
semnalării inegalităților între diferitele zone geogra-
fice ale țării noastre. Ca scop secundar, am urmărit
să comparăm situația accesului la îngrijirile de SM
din țara noastră cu cea a altor țări din EU.
Menționăm că nu a intrat în obiectivul acestui
studiu evaluarea barierelor atitudinale care se ridică
în calea îngrijirilor de SM din România.
Metodologia cercetării
Tipologia cercetării
Acest studiu este retrospectiv, observațional și
descriptiv.
Sursele de informație
Au fost folosite surse de informații naționale și
internaționale de date publice, neclasificate. Au fost
folosite informații din patru categorii de interes, astfel:
a. date demografice;
b. informații epidemiologice despre evoluția pan-
demiei de COVID-19 în România;
c. informații despre paturile din instituțiile de
asistență psihiatrică;

d. informații despre resursele umane, reprezentate
de medicii de specialitate de psihiatrie adulți și de
psihiatrie pediatrică. Nu s-au luat în evidență și ce-
lelalte cadre care lucrează în instituțiile de asistență
a persoanelor cu probleme de SM, din cauza lipsei de
date publice pentru acest sector.
a. Datele demografice au fost preluate din două surse:
n INS, pentru populația totală a României și populația
județelor țării(13)
;
n pentru delimitarea geografică a celor patru unități
teritoriale – macroregiuni (MR) ale României aflate
în nomenclatorul CE, am folosit datele publicate
în EUROSTAT(12)
.
b. Informațiile despre evoluția pandemiei de CO-
VID-19 în România – am folosit datele publicate
zilnic de Institutul Național de Sănătate Publică
– CNSBT(11)
.
c. Numărul absolut al paturilor din instituțiile psihi-
atrice din sistemul public (psihiatrie acuți, psihiatrie
cronici, psihiatrie pediatrică, secții de toxicomani) din
fiecare județ a fost furnizat de Centrul Național de
Sănătate Mintală și de Luptă Antidrog – CNSMLA(14)
.
d. Datele despre resursele umane disponibile pentru
acordarea tratamentului psihiatric, reprezentate de
numărul medicilor psihiatri din sistemul spitalelor
publice și al medicilor din ambulatoriile de specia-
litate (publice și private) au fost preluate de la Casa
Națională de Asigurări de Sănătate – CNAS și
CNSMLA(14)
.
Perioada de studiu
Cronologia pandemiei de COVID-19 din România,
din primăvara lui 2020, a folosit următorii indicatori:
T0
(26.02.2020 – prima infecție înregistrată pe terito-
riul României), T1
(13.03.2020 – declararea stării de
urgență), T2
(14.04.2020 – prelungirea stării de urgență)
și T3
(15.05.2020 – încetarea stării de urgență).
Pentru numărul de medici psihiatri și de paturi
din instituțiile psihiatrice (publice și private) am
luat în considerare datele raportate de organismele
guvernamentale pentru 2018(14)
.
Analiza statistică
Calculele statistice au fost realizate folosind Microsoft
OfficeExcel,versiunea2019.Astfel,amcalculatdistribuția
populației în macroregiuni, indicatorii epidemiologici ai
infecției cu SARS-CoV-2 raportați la 100 000 de locuitori,
numărul de medici care activează în spitale și cei care
lucrează în ambulatorii (în ambele situații raportarea
s-a realizat la 100 000 de locuitori) și numărul de paturi
din instituțiile psihiatrice, atât cele de stat, cât și private
raportate la 100 000 de locuitori.
Rezultate
I. Dinamica evoluției pandemiei în România
(26.02.2020 – 15.05.2020 )
Prima infecție pe teritoriul României a fost înregis-
trată pe 26.02.2020. Numărul de cazuri a urmat o curbă
ascendentă, corelată și cu creșterea capacității de tes-
tare (tabelul 1a). Primul caz vindecat a fost raportat la
5 martie, iar primul deces a apărut după aproximativ
trei săptămâni de la primul caz înregistrat.
Am luat în considerare și distribuția geografică
a cazurilor de îmbolnăvire cu COVID-19, prin cal-
culul numărului de pacienții la 100 000 de locuitori
(tabelul 1b).
II. Distribuția în macroregiuni a resurselor
umane și a capacităților instituționale de
tratament pentru afecțiunile psihiatrice
Distribuția în macroregiuni a medicilor psihiatri
din România
Asigurarea asistenței de SM prin numărul medici-
lor psihiatri din spitale și ambulatorii este diferită
în cele patru macroregiuni ale României. Numărul
psihiatrilor în fiecare macroregiune și repartiția lor
la 100 000 de locuitori, în 2018, sunt prezentate în
tabelul 2 și în figura 2.

Distribuția în macroregiuni a numărului de
paturi de psihiatrie din sistemul public, pe
categorii de îngrijire
Număruldepaturiînsecțiiledeacuțiraportatla100 000
de locuitori sunt aproximativ egale în MR2 (45,14) și MR3
(46,35), dar cu o scădere importantă a acestora în MR4
(36,95). Pe de altă parte, cel mai mare număr de paturi
în secțiile de cronici raportat la 100 000 de locuitori se
găsește în MR4 (75,57), față de MR3 (29,3), cu cel mai mic
număr pentru această categorie. O situație dezavanta-
joasă o reprezintă numărul insuficient de paturi pentru
tratamentul patologiei adicțiilor, cu un număr total pe
toată țara de numai 193 de paturi. De asemenea, paturile
pentru psihiatria pediatrică sunt puțin reprezentate în
toată țara (457), având în vedere ca în România în această
categorie de spitale sunt internați și adolescenți (până
la vârsta de 18 ani) cu afecțiuni psihice (vezi tabelul 3
și figura 3)(14)
.
Asistența psihiatrică în timpul perioadelor de
restricții impuse de starea de urgență decretată
ca măsură împotriva pandemiei de COVID-19
De la debutul pandemiei în România, s-a putut
constata o modificare semnificativă a tuturor in-
dicatorilor DRG, în ceea ce privește internările din
spitalele publice de psihiatrie, comparativ cu aceleași
date din 2019 (figura 4). Indicatorii luați în calcul au
fost: numărul internărilor, durata medie de spitalizare.
Impactul pandemiei asupra asistenței psihiatrice
din România se poate analiza ținând cont de dimi-
nuarea evidentă (procentuală) în 2020 a numărului
de persoane internate pe secțiile de psihiatrie acuți,
de toxicomanii (fără a include dependența de alcool)
și de psihiatrie copii etc., față de aceeași perioadă a
anului 2019.
Din contră, se constată o prelungire a duratei de spi-
talizare a pacienților internați în aceste tipuri de secții.

Discuție
Distribuția în macroregiuni a resurselor umane și a
capacitățilorinstituționaledetratamentpentruafecțiunile
psihiatrice este inegală, atât în ceea ce privește resursele
umane (medici psihiatri pentru adulți și copii), cât și în
ceeacepriveșteresurselelogisticedeacoperirecupaturi
depsihiatrie,diferențiateînspitalepediferitecategoriide
patologie(afecțiunipsihiatriceacutesaucronice,adicții)
sau vârstă (psihiatrie pediatrică, psihogeriatrie).
A. Distribuția în macroregiuni a medicilor psihiatri din
România arată că MR1 este cea care oferă cea mai bună
asigurare cu medici psihiatri atât la nivelul spitalelor
(12,21 medici psihiatri/100 000 loc.), cât și al ambulato-
riilor (8,4 medici psihiatri/100 000 loc.), urmată de MR3
(11,76medicipsihiatriînspitale/100 000loc.și8,07medici
psihiatri în ambulatorii) și MR4 (11,40 medici psihiatri în
spitale/100 000loc.și7,61medicipsihiatriînambulatorii).
Macroregiunea 2 este cea care oferă asigurarea cea mai
scăzutăcumedicipsihiatriatâtlanivelulspitalelor(10,69
medici psihiatri/100 000 loc.), cât și în ambulatorii (5,73
medici psihiatri/100 000 loc.).
B. Distribuția numărului de paturi din instituțiile psihi-
atrice variază în cadrul macroregiunilor României, MR4
având cel mai mare număr de paturi la 100 000 de locu-
itori (95,96/100 000 loc.), urmată de MR1 (91,5/100 000
loc.) și de MR2 (74,19/100 000 loc.). MR3 are cel mai mic
număr total de paturi (68,18/100 000 loc.).
Același clasament se păstrează și în ceea ce privește
numărul total de paturi de psihiatrie în secțiile de
cronici/100 000 loc. De remarcat însă inegalitățile
foarte mari între numărul de paturi la 100 000 loc.
între MR4 (75,67/100 000 loc.) și MR1 (60,94/100 000
loc.), pe de o parte, și MR2 (43,97/100 000 loc.) și MR3
(29,3/100 000 loc.), pe de altă parte.
MR1 are cel mai mare număr total de paturi de psi-
hiatrie în secții de acuți (48,64/100 000 loc.), urmată
de MR3 (46,35/100 000 loc.), MR2 (45,14/100 000 loc.)
și MR4 (36,95/100 000 loc.).
Toate macroregiunile au un număr total de paturi
de psihiatrie pediatrică aproape similar la 100 000 loc.
În ceea ce privește numărul total de paturi în secții
de toxicomanie la 100 000 loc., în MR4 nu există niciun
pat, iar în celelalte macroregiuni numărul este foarte
mic (MR1 – 0,73/100 000 loc., MR2 – 1,56/100 000 loc.,
MR3 – 2,17/100 000 loc.).
În ceea ce privește raportul dintre numărul de
paturi de psihiatrie la 100 000 loc. în secțiile de acuți
și cel al paturilor în secțiile de cronici, se observă că
în MR4 numărul de paturi în secțiile de cronici este
de peste două ori mai mare decât cel al paturilor din
secțiile de acuți, pe când în MR3 numărul paturilor
din secțiile de acuți (46,35/100 000 loc.) este cu mult
mai mare decât cel al paturilor din secțiile de cronici
(29,3/100 000 loc.).
Restricțiile impuse de starea de urgență decretată în
legătură cu pandemia de COVID-19 (16.03-14.05.2020)
au dus la diminuarea numărului de internări în spita-
lele și secțiile de psihiatrie de toate categoriile. Aceste
restricții au fost contrabalansate prin prezentarea
pentru consultații de urgență la unitățile de primiri
urgențe a unui număr crescut de pacienți, chiar în
absența unei patologii psihiatrice încadrabile în
categoria urgențelor. Excepție au făcut cazurile de
adicții (alcoolism cronic și dependențele de substanțe
psihoactive), care au înregistrat o creștere a cazurilor
cu simptomatologie de sevraj și care au fost internate
într-un număr mai mare în secțiile de acuți decât în
anii anteriori.

Comparație între resursele de personal și
paturile de spital din sectorul de psihiatrie din
România și valorile medii din UE
Numărul de medici psihiatri la 100 000 de locuitori se
situează sub media din UE, care este de 18,5/100 000 de
locuitori.Menționămcănumărulmaximdemedicipsihiatri
dinEuropasegăseșteînElveția(51,4/100 000delocuitori).
Numărul de paturi la 100 000 de locuitori este, în
medie, de 68,8 în UE, cu un maxim reprezentat de
Belgia, care are 136,1 paturi de psihiatrie la 100 000
de locuitori (pentru toate categoriile de asistență).
România, cu o medie de 80,5 paturi la 100 000 de
locuitori, se situează peste valoarea mediei din UE.
Limitări
Întrucât am utilizat datele disponibile din surse ofi-
ciale despre resursele de personal (din spitale, cabinete
ambulatorii, centre de sănătate mintală) din statisti-
cile anului 2018 și date demografice corespunzătoare
(numărul total al populației României și din cele patru
macroregiuni ale țării) din același an, ar putea să existe
unele diferențe față de datele statistice din anul 2020.
Concluzii
S-a constatat că distribuția în macroregiuni a re-
surselor umane și a capacităților instituționale de
tratament pentru afecțiunile psihiatrice este inegală,
fapt care a dus la o ofertă mult scăzută a serviciilor de
asistență pentru patologia psihiatrică în perioada de
debut a pandemiei în România. Inegalitatea la nivel de
resurse de asistență s-a manifestat cel mai pregnant
în MR2, în condițiile dramatice ale prezenței celui mai
ridicat număr de infectări cu SARS-CoV-2.
Reglementarea prin HG 252/20.03.2020 a
consultațiilor de telemedicină și, implicit, a celor de
telepsihiatrie a contribuit la micșorarea discrepanței
între cererea crescută de asistență psihiatrică și
oferta mult diminuată pentru accesul la internări
sau consultații.
Chiar și în aceste condiții de insuficiență a mij-
loacelor de asistență psihiatrică, remarcăm capa-
citatea de flexibilitate și reziliență a sistemului de
sănătate mintală din țara noastră, în condițiile de
criză multidimensională generată de pandemia de
COVID-19. n

Mental health
in the pandemic context
The spread of the severe acute respiratory syndrome
coronavirus 2 (SARS-CoV-2) has generated a pandemic
that caused governments around the world to take
unprecedented measures to limit it. In this context, we
conducted a review of the literature to establish the
mental health consequences of this situation. Our findings
show that this situation is a social crisis that has led to an
important increase in the prevalence of clinically significant
depressive, anxiety and stress-related symptoms, which
can thus generate and increase the prevalence of related
mental and behavioral disorders. Furthermore, various
social groups – i.e., young adults, patients, medical
staff, domestic violence victims, the elderly – are at an
even higher risk for developing such conditions or for
the relapse or worsening of the preexisting conditions.
Therefore, authorities should also be concerned with
limiting the psychological impact of this situation.

Răspândirea severe acute respiratory syndrome coronavirus 2
(SARS-CoV-2) a generat o pandemie care a făcut ca guvernele
din întreaga lume să ia măsuri dure pentru a o limita. În acest
context, am realizat o recenzie a literaturii de specialitate cu
scopul de a pune în evidență eventualele consecințe asupra
sănătății mintale. Datele publicate până în prezent arată că
această situație reprezintă o veritabilă criză socială care a dus
la creșterea prevalenței sindroamelor depresive, anxioase și
legate de stres, ceea ce va duce, cel mai probabil, la o creștere
a prevalenței tulburărilor mintale și de comportament
înrudite. În plus, datele arată că o serie de grupuri sociale
(adulții tineri, pacienții, personalul medical, victimele
violenței domestice și vârstnicii) au un risc chiar mai mare
de a dezvolta asemenea tulburări sau de a suferi o recădere
în cadrul unor afecțiuni preexistente. În acest context,
autoritățile ar trebui să acorde atenție și limitării impactului
psihologic al acestei situații.

Introduction
The spread of the severe acute respiratory syndrome
coronavirus 2 (SARS-CoV-2), the virus that causes
coronavirus disease 2019 (COVID-19), has generated a
pandemic that forced governments around the world
to take unprecedented measures to limit it. On April
8th
, 2020, the United Nations Educational, Scientific
and Cultural Organization (UNESCO) stated that 188
countries had their schools suspended nationwide, an
unprecedented measure that left at least 1.5 million
young people out of formal education(1)
. Furthermore,
many businesses suspended or drastically reduced
their activities, causing an economic contraction
that is estimated to reach around 12% in the Euro-
zone alone, and characterized as “unprecedented in
peacetime” by representatives of the European Central
Bank(2)
. Apart from probable economic (i.e., unem-
ployment) and financial implications, the measures
taken generated drastic changes in lifestyle for many
individuals that are now under pressure to fulfill
their job roles from home while also caring for their
children. In addition, instrumental social support
has become very difficult to provide because of the
imposed travel restrictions. Thus, we can state that
this situation generated major changes in the lifestyle
of most individuals and that many of them will find
it impossible to adapt by using their familiar coping
strategies. It is therefore reasonable to consider this
situation a social crisis, as it will cause numerous
individuals to go through “a period of psychological
disequilibrium, experienced as a result of a hazard-
ous event or situation that constitutes a significant
problem that cannot be remedied by using familiar
coping strategies” – the very definition of a crisis, as
Roberts formulated it in 2000(3)
.
A social crisis can lead to increased symptoms
and prevalence of major depressive disorder, anxi-
ety disorders, stress-related disorders (i.e., including
posttraumatic stress disorder), and mental and be-
havioral disorders due to psychoactive substance use.
Furthermore, stress is a factor that increases the risk
of relapse in other mental and behavioral disorders
as well. The resolution of any crisis and the efficient
management of stressful situations are dependent
upon factors related to the event (e.g., severity, locali-
zation) and factors related to the individual (e.g., the
perception of the traumatic event, social support and
coping and defense mechanisms)(4)
. However, promot-
ing adaptation is dependent upon knowledge about
the stressor, its direct consequences and putative

mitigating factors (e.g., demographic, socioeconomi-
cal, cultural etc.). Thus, this study aimed at reviewing
the currently available empirical results on mental
health in the context of COVID-19 pandemic. For
this, a PubMed search was conducted using the terms
“COVID-19” and “mental health”. All abstracts were
reviewed and all papers that contained empirical
evidence were further processed. Opinion papers
were excluded. Full-text versions were obtained for
all included studies.
Epidemiological data on general population
All papers identified started from the assumption
that an increase in the frequency and severity of
symptoms related to stress, anxiety and depression
is expected(5,6)
, but the magnitude of this increase is
unknown and nearly impossible to predict. Further-
more, the extent to which these symptoms will add
up to cause an increase in the prevalence of stress-
related disorders, anxiety disorders, major depressive
disorder, posttraumatic stress disorder (PTSD) and
other mental and behavioral disorders and problems
(e.g., psychoactive substance use disorders, self-harm
or suicide attempts) is yet unknown and needs to be
researched(5)
. Studies that include diagnosis are how-
ever more elaborate and difficult to implement during
the crisis. Thus, all published papers used screening
tools and reported prevalence rates based on these.
Both Asian and European studies report higher
rates for clinically significant depressive symptoms
in the general population, varying between 16.5% and
48.3%(7-10)
, values that are significantly higher than
the 12-month prevalence of 7% reported in the United
States for major depressive disorder(11)
.
The prevalence of clinically significant anxiety
symptoms in the general population varied between
21.6% and 35.1%(7-10)
, again higher than the 12-month
prevalence of 17.7% reported in general(12)
.
The prevalence of PTSD symptoms was found to
be 15.8%(8)
.
Risk and protective factors for the general
population
The risk for developing a mental and behavioral
disorder or worsening of a preexisting one relies in
the intensity and nature of the stress reaction, which
is influenced by the objective nature of the stressor,
the perception of the event, the available social sup-
port and the coping strategies(4)
.
The COVID-19 pandemic is objectively a particular
type of stressor in the sense that it is poorly localized
and yet generalized, giving individuals little chance
for avoidance or control.
The perception of a stressful situation is based
on the appraisals individuals perform when faced
with it. In general, persons will perform two types of
appraisals when faced with a stressor, and these ap-
praisals then determine how they will cope with that
stressor. The primary appraisal evaluates the nature
of the stressor per se in terms of severity, certainty,
predictability etc. The secondary appraisal is focused
on their own ability to deal with the situation (e.g.,
controllability, agency)(13)
.
In the case of COVID-19 pandemic, several factors
concurred to create an image of severity. First, the
nature of the stressor is still somewhat unclear in
respect to the severity of the illness, speed of spread,
prognosis etc. Conflicting messages from the authori-
ties made the situation even worse, in many cases, by
creating even more uncertainty. Secondly, looming
shortages of resources, both for basic needs and for
prevention and treatment, high financial losses and
impaired ability to get social support due to self-
isolation and travel restrictions created an image of
poor controllability of the situation for many people(14)
.
In the general population, the most important risk
factors associated with psychopathology were: inad-
equate information, loneliness, duration of quaran-
tine, female gender, younger age, previous diagnosis
of mental and behavioral disorders, fear of infection,
having symptoms associated with COVID-19, hav-
ing a close relative with COVID-19, frustration and
boredom, inadequate supplies, financial problems
and stigma(8,10,15)
.
On the other hand, the most important protective
factors were: the belief that information has been
provided, perceived survival likelihood and low risk
of contracting SARS‑CoV‑2, older age, economic sta-
bility, personal precautionary measures, spirituality
and high level of confidence in doctors(7,8,10)
.
Data on special population groups
The COVID-19 pandemic per se and the measures
taken to limit it affect vulnerable populations even
more(16)
.
Youth has been impacted more severely by the pan-
demic, causing significant disruption in routine, peer
contact, skills acquisition and putting significant
strain on their relationship with adults/caretakers.
Students were further impacted by the suspension of
classes, examinations, with little remedy put in place
by the authorities in many cases. Graduating students
faced even more uncertainty regarding graduation
and entering the work market(17)
. In a cross-sectional
study from China, nearly 40.4% of young adults were
found at risk for psychological problems, with lower
educational attainment and using negative coping
styles among the most important risk factors(18)
.
The measures taken by authorities worldwide have
had a major impact on the elderly due to their bio
psychosocial vulnerabilities(19,20)
. Older persons are
especially vulnerable to loneliness, an independent risk
factor for depression, anxiety disorders and suicide(20)
,
especially through worsening their perception of risk
of death or disease(19)
. Furthermore, because most of
them are not familiar with modern communication
technologies, they have a poorer access to quality
information and even limits their ability to secure
quality in mental health care within the COVID-19 pandemic

proper healthcare(20)
. Also, cognitive impairment
and other mental disorders due to brain damage and
dysfunction can make it difficult for these patients
to follow proper preventive measures(20)
.
The patients with mental and behavioral disorders
are another high-risk population since stress can
precipitate a relapse of their preexisting condition
and because isolation measures limit the availability
of resources for them(21)
.
Medical staff is also considered a high-risk popu-
lation, and available data show very high rates of
clinically significant depressive symptoms (50-51%)
and of anxiety symptoms (45%)(22,23)
. Furthermore, the
medical staff seems to be the professional category
most affected by poor sleep quality(10)
, with insomnia
rates affecting more than one in three individuals in
this group(22)
.
Moreover, the isolation and quarantine measures
imposed by authorities have led to a grim situation
for the victims of domestic violence – a phenomenon
that is thought to have increased during the pandemic
– because they can no longer escape their abusers(24)
.
An increase in complicated grief prevalence is also
expected because of the limitations imposed on nor-
mal familial care for terminally ill patients and on
burial customs(25)
.
Conclusions
The situation created by the COVID-19 pandemic
is a social crisis that has led to a notable increase in
the prevalence of clinically significant depressive,
anxiety and stress-related symptoms, which can thus
generate and increase the prevalence of related mental
and behavioral disorders. Furthermore, various social
groups – i.e., young adults, patients, medical staff,
domestic violence victims, the elderly – are at an even
higher risk for developing such conditions or for the
relapse or worsening of their preexisting conditions.
Therefore, authorities should also be concerned with
limiting the psychological impact of this situation by
providing adequate, noncontradictory information,
facilitating social support and spirituality, and limit-
ing the economic impact on individuals, as these are
currently considered the most efficient protective
factors against developing a mental or behavioral
disorder in the pandemic context. n

Neurotoxic behavior of
antidepressant medication
and its clinical consequences:
serotonergic syndrome and
discontinuity syndrome
Nowadays, antipsychotic and antidepressant medications
are recommended and prescribed for a large variety of
psychoorganic syndromes and for other neuropsychiatric
clinical conditions. However, not much has been discussed
about their neurotoxicity and their consequences on
specific brain structures. Certain effects of their neurotoxic
impact can mask other neuropsychiatric disorders, and
they can be a real challenge and a diagnostic pitfall.

În zilele noastre, medicația antipsihotică și antidepresivă este
recomandată și prescrisă pentru un spectru larg de sindroame
psihoorganice și în alte afecțiuni neuropsihiatrice. În ciuda
acestui fapt, nu s-a discutat foarte mult despre neurotoxicitatea
lor și despre consecințele asupra anumitor structuri ale
creierului. Unele efecte determinate de impactul neurotoxic pot
masca alte afecțiuni neuropsihiatrice și pot constitui adevărate
provocări sau capcane de diagnostic.

The behavioral toxicity of antidepressants
Antidepressants are a category of drugs clinically
indicated in relieving symptoms of depression, social
anxiety disorder, seasonal affective disorder, mild
chronic depression, or dysthymia. They are designed
to influence, change and modulate the neurochem-
istry of the brain, in mood and emotional pathology.
Selective serotonin reuptake inhibitors (SSRIs) are
some of the most frequently prescribed antidepressants
available, used to treat the major depression, mood dis-
orders, sometimes the attention deficit hyperactivity
disorder (ADHD), the obsessive-compulsive disorder
(OCD), menopause symptomatology, fibromyalgia and
neuropathic pain. Those inhibitors are largely used
in the treatment of different stages of depression and
have less neurotoxic effects. Most antidepressants
may have undesirable effects caused by maladaptive
neurogenesis, deficient synaptic functionality, poor
regulation of glycogenesis, and on neurotransmitter
postsynaptic transduction pathways, with a final
impact on the neural network in the central nervous
system. When deciding and choosing antidepressant
medication and its initiation, we must consider two
aspects, namely the critical flicker fusion threshold
(CFF) and the choice reaction time (CRT). These two
pharmacodynamic characteristics are a kind of indi-
cator of the clinical efficacy of both antidepressants
and psychotropics(1)
.
Due primarily to their direct action on the central
nervous system, antidepressants may somehow disrupt
cognitive function, thus increasing anhedonia, with
impaired sexual function by decreasing sexual libido.
Sometimes there is a decrease in the ability to think,
synthesize and process, and we can even talk about
a cognitive toxicity. Also, antidepressant drugs and
psychotropic drugs cause changes in the behavior of
patients(2)
. The metabolism of antidepressants is a
very important aspect, as well.
If the medication is taken orally, it will respect the
classical pathway: small and large intestine, liver,
bloodstream. Once it has entered the bloodstream,
the active compound molecule will interact with
various cellular effectors (proteins, receptors, fatty
acids), an interaction which will affect the distribu-

tion. From the hepatocellular point of view, through
a demethylation cycle, the half-life of antidepressants
is longer than normal, justifying their slow inactiva-
tion and elimination(3)
.
The neurochemistry of antidepressants’ metabolism
in certain areas of the brain is an extremely complex
process that will influence both the success or the
failure of treatment, and the worsening or improve-
ment of the symptoms. The involvement of cellular and
molecular mechanisms at the level of neural metabolic
signaling cascades in case of antidepressants often
involves brain structures such as: frontal, temporal,
occipital, parietal cortex, cerebellum, thalamus with
neurochemical effectors such as G proteins, adenylyl
cyclase, protein kinases, protein cyclase, kinase activa-
tion receptors, and phosphoinositide. Besides these,
the metabolism of antidepressants at the neuronal
level involves structures such as: cortex, limbic cortex,
amygdala, hippocampus, with molecular components
such as ATP, glucose and inositol, sensitive to the
action of pharmacological agents such as fluoxetine,
imipramine, moclobemide, paroxetine, sertraline and
tranylcypromine. Decreased cognitive function and
neuromotor disorders have been observed in depressed
elderly with chronic antidepressant medication. For
example, tricyclic antidepressants cause a certain
impairment of cognitive and motor function, and
that is not only due to sedation. Antidepressants
that do not affect these functions are those with a
relatively unsaturated profile and those that inhibit
serotonin reuptake(4)
. Comparative studies have been
conducted on the psychopharmacological profiles of
several selective serotonin reuptake inhibitors, and
their neurotoxicity contrasted with both the placebo
effect and the tricyclic antidepressants dothiepin and
amitriptyline(5)
. New-generation antidepressants, such
as escitalopram and mirtazapine, are preferred by
most clinical guidelines(6,7,8)
, but very few clinical trials
have been performed to demonstrate their effects on
cognition and neuromotor function. Also, from the
experience of some psychiatric clinics, it has been
observed that escitalopram and mirtazapine improve
the psychomotor function of patients with endogenous
depression, while amitriptyline deteriorates it, there-
fore in clinical practice escitalopram and mirtazapine
are preferred to amitriptyline.
The neurotoxic effects with neuropsychiatric conse-
quences in the case of antidepressants have led to the
description of two distinct clinical entities. The first one
is serotonergic syndrome, which occurs if only one
type of serotonin reuptake inhibitor is chosen, may/may
not be installed until the optimal dose is established,
and may occur from overdose and/or administration
of two serotonergic drugs simultaneously.

The symptoms of serotonin syndrome can be mild
to severe, revealing anything from a slight serotonin
excess to a life-threatening syndrome. The most com-
mon symptoms include(9)
:
Restlessness Sudden jerky or

High blood pressure Muscle rigidity
Shivering Confusion
The serotonergic syndrome, if well individualized
and controlled, does not cause any problems. Once the
neuronal serotonin levels have returned to normal,
there is no risk of worsening. If the dose is played
with, if the patient is not strictly supervised when
administering the medication, then serious unwanted
problems – such as hyperpyrexia, arrhythmias un-
consciousness and even death – may occur.
The discontinuation syndrome (antidepressant
discontinuation syndrome) – or antidepressant with-
drawal syndrome – is a psychiatric condition that can
appear following the uncontrolled stoppage, reduction
or interruption of antidepressant medication. The
symptoms are equivocal and fluctuating, includ-
ing flu-like symptoms, gastrointestinal symptoms
(nausea), sensory disturbances, affective symptoms,
mood balance problems and sleep disturbance(10)
.
Discontinuation symptoms can be confused with the
recurrence of the underlying psychiatric illness and
might also be misdiagnosed. This syndrome needs to
be distinguished from relapse. Due to a lack of specific
clinical protocols, the management and prevention
of this syndrome need to be personalized for each
patient, according to severity and clinical condition.
Onset and duration: generally, on the first week, but
evidence studies suggest a mean time of onset of two
days after stopping. A spontaneous resolution between
two days and three weeks after onset was reported.
Prevention and management of
antidepressant discontinuation syndrome(11,12)
Tapering may not be necessary for patients taking
an antidepressant for less than four weeks or for those
who are substituted with fluoxetine.
n Tapering at a slow rate.
n Antidepressants with short half-life need to be re-
duced progressively; however, progressive tapering
may not preclude the syndrome onset in all patients.
n Providing patients assurance.
Conclusions
The clinical and pharmacological profile of the
antidepressants therapy neurotoxicity and its clinical
consequences need to be emphasized, taking into ac-
count that depression is a common mental disorder, a
recurrent pathology with a self-induced vulnerability
and that, globally, more than 264 million people of
all ages suffer from depression. The neurotoxic syn-
dromes were briefly explained, suggesting, among
other things, that the choice, initiation, continua-
tion, interruption and stopping of the antidepressant
therapy contribute fundamentally both to the quality
and effectiveness of treatment, and to the patient’s
quality of life. Neurotoxicity hypothesis of depressive
disorder, apart from allowing a different strategy of
communicating with our patients and to facilitate
their compliance, plays an important role in treat-
ment adhesion. n

Associating structured
psychotherapies with
psychoeducation for the
prevention of relapses in
bipolar disorder
Mental disorders are generally considered to be among
the most important public health problems, due to
the frequency in the population and the very high
costs regarding therapy, along with all the negative
consequences, such as limiting the patient’s work capacity.
Statistics show that 20-25% of patients from the medical
records of general practitioners have mental disorders.
Mood disorders are common treatable conditions in
which patients experience an abnormally low or high
mood. Abnormal moods are accompanied by multiple
signs and symptoms that affect all areas of functioning.
These disorders are in the center of the most extensive
nosographic changes, through relevant findings in the field
of etiology, from a biochemical and genetic perspective.
Bipolar affective disorder is characterized by repeated
episodes (at least two) in which the patient’s mood and
activity levels are significantly disrupted and, between
episodes, the recovery is usually complete. Early diagnosis
and treatment of bipolar disorder have important clinical
consequences. An effective treatment involves an early,
correct and complete diagnosis, as well as a therapeutic
strategy aimed to obtain the remission of the current
affective episode, but especially the prevention of relapses
and recurrences. The association between psychotherapies
and psychotropic medication is more effective than these
two therapeutic methods used separately and ensures
improved therapeutic compliance, reducing the rate of
hospitalization and increasing the asymptomatic period
between episodes. Thus, in this context, psychoeducational
measures have a very important role in terms of involving
patients and family in understanding and managing
affective symptoms and increasing the quality of life.

Tulburările psihice sunt considerate printre cele mai
importante probleme de sănătate publică, prin prisma
frecvenței în populație și a costului foarte ridicat al
tratamentului, precum și din cauza consecințelor negative,
cum ar fi limitarea capacității de muncă a pacienților.
Statisticile arată că 20-25% dintre pacienții aflați pe listele
medicilor de familie au tulburări psihice. Tulburările
dispoziției sunt afecțiuni frecvente și tratabile, caracterizate
prin dispoziție anormal de scăzută sau expansivă, însoțite
de semne și simptome multiple ce duc la afectarea tuturor
ariilor de funcționare a pacientului. Aceste tulburări se află
în centrul celor mai importante modificări nosografice,
generate de descoperiri relevante în domenul etiologiei
acestora, la nivel biochimic și genetic. Tulburarea afectivă
bipolară se caracterizează prin episoade repetate (minimum
două) de afectare semnificativă a dispoziției și activității,
iar între episoade recuperarea este în general completă.
Diagnosticul și tratamentul precoce al tulburării afective
bipolare au consecințe clinice importante. Tratamentul
eficient presupune diagnostic precoce, corect și complet
și o strategie terapeutică menită să obțină remisiunea
episodului afectiv actual și, mai ales, o prevenire a recăderilor
și recurențelor. Asocierea dintre psihoterapie și medicația
psihotropă este mai eficientă decât utilizarea separată a
celor două modalități terapeutice și asigură o creștere a
complianței terapeutice, o reducere a ratei spitalizării și o
prelungire a perioadei asimptomatice dintre episoade. În
acest context, măsurile de psihoeducație au un rol foarte
important în ceea ce privește implicarea pacienților și a
familiilor acestora în înțelegerea și gestionarea simptomelor
afective și în creșterea calității vieții.

Bipolar affective disorder is a long-term, episodic
psychiatric condition. The prognosis of this disorder is
often reserved, due to inadequate therapeutic modali-
ties. This will lead to an evolution with predominantly
mixed episodes, shortening the inter-episodic periods
with age, with exacerbation of cognitive deficits, somatic
comorbidities and the risk of suicide. The unfavorable
evolution of bipolar affective disorder is associated with
severe alteration of interpersonal and family relation-
ships, social dysfunction, and altered quality of life of
the patient, family and relatives(1)
.
Achieving adequate therapeutic management involves
ensuringabalancebetweenefficacy,toleranceandsafety.
Improving therapeutic compliance requires ongoing
monitoring of patients, as well as an adequate patient’s
education on the characteristics of the disorder and the
roleofthetreatment.Long-termtherapeuticmanagement
depends on comprehensive medical care, which ensures
the collaboration of all those involved in the mental
health services, through an adequate model of care(2)
.
Thus, an appropriate therapeutic plan must take into
account the following elements(3)
:
n the treatment of patients with bipolar affective dis-
order includes pharmacotherapy associated with
psychotherapeutic methods;
n monitoring of physical parameters;
n support services for bipolar patients, including mental
healthservicesandothersocialorprofessionalservices;
n modalitiesbywhichthefamiliesofthesepatientslearn
to manage this disorder and ensure mutual support.
In bipolar affective disorder, ensuring compliance with
treatment is essential as these patients are among the
most noncompliant of all psychiatric patients, because
even during the inter-episodic periods, the extraversion
and the confidence in their health determine them to
exclude the possibility of relapses, and it is difficult to
accept the therapy, abandoning it after variable periods.
Also, in the depressive episode, noncompliance is based
on the feeling of inutility and uselessness, the lack of
confidence and hope on the therapeutic measures. To
this, it is added the influence of the side effects of the
medication and the delay regarding the presence of
positive results(4)
.
Psychoeducational measures are effective both in the
acute phase and in preventing recurrences from bipolar
affective disorder. Relapses and recurrences may be due
to patients’ noncompliance with treatment, adverse
effects of medication, or lack of family support. Thus,
an important factor in preventing relapse could be the
identification of early symptoms, the prodromes char-
acteristic of manic or depressive episodes(5)
.
Since the 1990s, it has been found that most bipolar
patients experience, throughout their lives, numerous af-
fectiveepisodes,whichgeneratepsychosocialdisabilities.
Kraepelin’s statement that affective disorder has a good
prognosis is no longer valid in the postmodern era. In
general, patients with numerous symptomatic relapses,
as well as those with chronic forms, resistant to lithium
therapy or not compliant with treatment, are considered
not to benefit from adequate therapeutic management.
Combining pharmacotherapy with psychotherapy is the
best therapeutic way(6,7)
.
Studies to date have shown that cognitive therapy
favors reducing the number of relapses and increasing
the period of time in which they occur, with increas-
ing compliance with treatment(8)
. Cognitive-behavioral
therapy has proven effectiveness both in the acute phase
and in preventing relapses and recurrences of the depres-
sive episode of unipolar disorder, therefore good results
are expected to reduce relapses in bipolar disorder.
Thus, patients will learn new coping skills, recognition
of prodromes symptoms and early signs that precede
an acute episode. Recent studies have shown that there
are prodromes for both manic and depressive episodes,
and patients with bipolar disorder can identify them. It
seems much easier to see the prodromes of the manic
episode, probably due to the fact that they have a more
sudden, dramatic appearance, while the depressive ones
have an insidious appearance(9)
.
The association of cognitive-behavioral therapy with
pharmacotherapy has led to a reduction in relapses,
compared to the use of pharmacotherapy only, according
to the studies of Lam from 2003 and Scott in 2001(10,11)
.
However, the studies performed were pilot studies, with
a small number of patients. In a study conducted by
Fava, on 15 patients with bipolar affective disorder, in
whom relapses had occurred while they were on lithium
prophylaxis, the association of cognitive-behavioral
therapy led to a reduction of residual symptoms and an
improvement of lithium prophylaxis results, but this
study had no therapeutic comparator group (Fava et
al., 2001)(12)
. In another randomized study conducted by
Perry et al. in 1999, patients were taught to detect the
early symptoms of the disease and, thus, see a therapist
sooner than other patients.
This has been achieved by improving symptom self-
monitoring and learning coping strategies, which have
led to reduced relapses(13)
. Cognitive-behavioral therapy
has also proven useful in the depressive episode of bi-
polar disorder. Goldstein et al. investigated the effects
of cognitive-behavioral therapy in bipolar depression.
He concluded that bipolar depressive symptoms can be
reduced with the help of techniques specific to cognitive-
behavioral therapy(14)
.
Psychoeducational measures can be divided into three
categories, according to Colom and Vieta(15)
:
a) awareness of the disorder, early detection of pro-
dromal symptoms, increased treatment compliance;
b) stress control, avoidance of substance use and
abuse, regular and controlled lifestyle, prevention of
suicidal behavior;
c) awareness of past and future emotional episodes
and social consequences, improvement of specific
social, interpersonal activities in the periods be-
tween the affective episodes, confrontation with
the subsyndromic residual symptomatology and its
consequences, improving well-being and increasing
the quality of life.

All of the measures from the first category can be
considered partial objectives and their non-fulfillment
can mean the failure of psychoeducation. Each of
these categories of objectives, taken separately, can
be considered themselves specific objectives to be
achieved by the patients with bipolar disorder, but
all three together make this program, proposed by
these authors, solid, more effective, including the costs
involved in the long term. The objectives in the third
category are part of the so-called excellent scenario,
the completion of the program, after successfully
completing the other two categories(15)
.
Psychoeducational measures can be based on the
relapse exercise method (“relapse drill”), which takes
the model from the one realized in 1985 by Alan Marlatt
and Judith Gordon for the prevention of relapses in
case of alcohol addiction(16)
. In case of patients with
bipolar disorder, this exercise involves the dynamic
follow-up of measures to be followed to prevent a
catastrophic event and consists in identifying prodro-
mal symptoms, making a list of preventive measures,
performing procedures that they have in mind, and
detailed measures of prevention. These measures will
involve family members, but also the psychiatrist or
family doctor, because together with them there will
be established all the details regarding the moment
when the prodrome appears, and it is necessary to
ask the doctor for help and even to discuss about
hospitalization. Usually, patient’s support tends to be
provided by close relatives, family members, whether
we are talking about the care of patients with mental
disorders or chronic disabling medical conditions.
But, on the other hand, it will have as a consequence
an important impact in terms of their functionality
and psychological stress(16,17)
.
Usually, the first psychotherapy sessions aim to edu-
cate the patient about bipolar disorder, the character-
istics of the treatment to be followed, the importance
of compliance with treatment, the particularities of
emotional symptoms and their close monitoring. Pa-
tients can be taught to perform activities according
to a well-established schedule: for the prevention of
the manic episode, it is recommended to engage in
calm activities, increase the rest period, reduce the
exiting factors, stimulants, and reduce the number
of activities; for the depressive episode – occupying
time with an increasing number of activities, so that
later they can make a plan for carrying out activities
at work(18)
.
The treatment phases and the objectives of psy-
chotherapy sessions (according to Basco and Rush,
1996) are(19)
:
n Patient education
✔ general information about cognitive-behavioral
therapy, information about bipolar disorder,
mood-stabilizing medication, antidepressant
medication, individual symptoms of bipolar dis-
order, symptom monitoring
n Compliance with treatment
n Cognitive-behavioral interventions
✔ cognitive distortions, cognitive changes in de-
pression, logical analysis of negative automatic
thoughts, cognitive changes in mania, aspects
of behavior in depression, changes in behavior
in mania
n Psychosocial problems
✔ analysis of psychosocial functioning, development
of problem‑solving techniques, solutions.
Cognitive therapy has been studied in particular
regarding the increase of bipolar patients’ compliance
with mood stabilizer therapy, and behavioral therapy
can be effective during the in-hospital treatment of
manic patients. Supportive psychotherapy is indicated
during the acute phases and in the early period of
remission. It is more commonly indicated in chronic
bipolar patients, in whom there are significant residual
inter-episodic symptoms and social impairment(20)
.
Group therapy is useful in the social reintegra-
tion of patients, in influencing denial and defensive
grandeur, and in dealing with various problems of
manic patients (loneliness, fear of mental illness,
shame, loss of control). Family therapy consists of
psychotherapeutic methods regarding the mental
disorders and interfamily relationships, like family
and marital counseling, changing communication
systems, therapy of family emotional life, multifamily
therapies, family crisis therapy(21)
.
The main indications for psychotherapy are the
following(22)
:
n the need to understand and accept long-term
therapy
n the need for association with pharmacotherapy
n refusal of medication
n avoidance of adverse effects related to the admin-
istration of antidepressants, such as agitation or
rapid cycling
n long-term maintenance therapy phase.
Psychotherapy can be recommended for all patients,
especiallypsychoeducationalmeasures,cognitivetherapy
or other structural therapies. Studies related to psycho-
social therapies and therapeutic guidelines support the
idea ofusing psychotherapy as an adjunctive therapeutic
methodtopharmacologicaltherapy.Patientswithbipolar
disorder have relapses in up to 40% of cases in the first
year, 60% in the second year, and 73% in the next five
years(23)
. The combination of psychotherapy with mood
stabilizer medication and especially psychoeducational
measures will reduce the number of hospitalizations
and the number of relapses, as well as increase compli-
ance with treatment and improve overall symptoms.
Psychosocial interventions will help the patient with
a number of problems related to understanding the
mental disorder itself, self-confidence and compliance
with treatment. It will also help the patient to avoid
stigmatization, ensure a proper sleep schedule and limit
access to various substances that can have a negative
influence on mood by informing one about the aspects
of this mental disorder(24)
