1. Tell me a little about you, your disability history, your age, and what you’re doing now in
terms of work or school or both.

2.   When and where were you a client of vocational rehabilitation?

3.   How did you find out about VR?

4.   What was the application process like? What did you have to do?

Did you have help or assistance in applying? No

5.   Were you ever denied service? When, why and what did you do?

6.   How was/is your relationship with your counselor?

7.   What kind of services did you/have you applied for?

8.   What services have you actually received? For how many years?

9. Have you ever had to appeal a denial of service? Tell me about that in as much detail as you
can, including the end result.

10. Have you been a VR client more than once? For what reason? Was there a different
counselor? Different experience?

11. Did you ever change your goal? Was your counselor supportive?

12. Did your counselor try to steer you toward a goal, career or education you didn’t want?

13. Has your counselor or VR agency helped you with job placement?



14. Has VR ever helped you obtain a wheelchair or other mobility equipment?



15. Have they helped you obtain a vehicle, or adapt one you purchased?

16. Have they helped with home modifications?

17. What about with job accommodations?
18. What would you tell someone applying to VR today? How would you advise them?

19. Anything else you want to add?
