day and documenting findings in appropriate forms.

(b) To collect information on level of awareness of Immunization Polio
Days(IPD)
by conducting cross-sectional surveys in sample households and children of
and on the streets.
(c) To ensure missed child(ren) found in the process is vaccinated by
referring Vaccination teams to transits sites, households and streets where
unvaccinated children are found for vaccination.
(d) To create enabling environment for motivation via Interpersonal
Communication (IPC) and providing solutions to challenges and problems
encountered by/ with vaccination teams on field work.
(e)Reporting to the State/LGA Immunization Focal Person when problems
demand higher-level contribution.
