Athletic Club for an appropriate guest

times in one calendar year.

their guests are properly registered and the
appropriate guest fees are paid.

allowed only when accompanied by an adult.

-Guest of member: $ 15
-Under 13 years of age: $ 10
- Daily guest fee when not accompanied by a member: $ 30
9. Included in your member benefits are discounted guest fees when
utilizing our associated clubs. Please present your membership card to
the service desk upon arrival.
8. CHECKING IN
1. Members and guests must check in at the
Athletic Club service desk by use of their
membership card or an identification scan.
2. Membership cards are unique to each member
and are not transferable

admittance to the club.

order for any member to use the Athletic
Club.
10. MEMBERSHIP CARDS
is a physical embodiment of your
company, staff and services. This
promotes a degree of connection
and loyalty between you and your
clients, meaning that they are more
likely to renew or upgrade their
membership when the time comes.
11. MEMBERSHIP CARDS
Membership cards may be
provided to members and
required for entry into facility. A
fee of $ 10 will be charged to
replace a lost card.
12. Enrollment Fee
means the non-refundable fees
to be paid by the Members to
the Club to enable the Members
to acquire Memberships from
the Club.
13. There is a one-time enrollment fee to the Athletic
Club as long as the member is active.
If membership is terminated and the former member
wishes to re-join, applicable enrollment fees will be
charged.
The Athletic Club reserves the right to change the
amount of the enrollment fee at any time.

used to fund activities carried out by the
organization. Membership dues are distinct
from membership fees which tend to be one
off payments.

17. 4. The Athletic Club reserves the right to change the
amount of monthly dues at any time. Members will
receive notification through mail.
5. The credit card on file is charged the first of the
month for all activity on the account from the
previews month. Members may pay their account
balance before the first of the month so that the
credit card on file is not charged.
19. Freezing membership typically refers
to temporarily halting membership to a
particular organization or membership service.
This could mean stopping a membership to a
gym or other physical activity, pausing
magazine or subscription services, or halting
payments made to a certain organization or
business.
20. 1. Memberships may be moved to a
“freeze” status in the event that illness or
injury incapacitates a member and he/she
cannot use the club. Proof from a doctor
is required.
21. 2. Only Golden Membership may
be moved to a “freeze” status if a
member leaves the state for an
extended period of time. Proof of
dual residency required.
3. Membership will be frozen
for a minimum of three
months and a maximum of six
months, once per calendar
year.
22. 4. Requests to place a membership
account on “freeze” status must be
received in writing by certified mail or
by completing the status change form
at the member services office.
23. 5. During the “freeze” status,
50% of monthly dues will be
charged until the member
reactivates Athletic Club
privileges. At the time of
reactivation, regular monthly
dues are applicable.
6. During the “freeze” status,
member may be use the Club
by paying applicable guest
fees.
25. Resignation or Withdrawal from Membership
Generally, membership in an association is
voluntary. An individual is free to join or resign
from an association. Membership in an
association can be terminated by resignation or
withdrawal by the member.
26. 1. Memberships are month – to-
month and renew automatically.
2. A member may resign their
membership with minimum 30-
day written notification to the
member services office through
the approved cancellation form
of authorized certified/return
receipt requested mail.
27. 3. Resignation is effective
on the first day of the
month following the
expiration of the 30 day
period.
4. Resignation from Athletic
Club does not relieve the
member of the
responsibility to pay all
applicable charges prior to
expiration of the 30 – day
period.
28. 5. The Club reserves the right to terminate any membership or usage at any time at its
sole discretion, for any reason, but not limited to:
Non-payment of member account.
Failure to comply with club policies and procedures.
Aggressive, disruptive or abusive behavior and comments.
Any action that interferes with the operation of the Club members’ enjoyment
of the facility.
Incomplete membership application, including but not limited to all necessary
signatures, proof of residency and valid payment information.
29. Payment for
Services
Payment is the transfer of
money, goods, or services in
exchange for goods and services
in acceptable proportions that
have been previously agreed
upon by all parties involved. A
payment can be made in the
form of services exchanged,
30. 1. Monthly membership dues are
automatically billed to your credit card on the
first day of each month for that month’s dues.
Any house charging from the previous month
will also be char4ged to your credit card.
31. 2. The Athletic cards accept
MasterCard, Visa, Discover, and
American Express. A credit card is
required for month to monthly billing
to mai8ntain membership. Only one
credit card may be used in an
account. Members may also pre-pay
for membership annually.
32. Unpaid accounts will be
assessed a $ 15 per month fee
until balance id paid in full.
Returned check and credit cards
are subject to a service fee.
Members and designated
associate members may bill
incidental charges to their
account for additional facility
services and/or merchandize.
Primary members are
responsible for payment for all
incidental charges.
33. 5. Members may establish house- charging
privileges through the use of their personal
credit card.
34. Locker Room and Lockers
is a room containing lockers at a gymnasium, stadium,
or school where people can change their clothes, often
to get ready to do sport.
35. 1. Towels are available in the locker rooms at no
extra cost.
2. No food is permitted to locker room areas.
3. Children under 13 must be supervised by an
adult in the locker room area.
36. 4. Lockers are for daily use only. Articles are not to
be left in lockers overnight. The club removes all
contents daily. Articles removed from lockers are
donated to charity after 30 days.
5. Permanent rental lockers are available with
laundry service for a monthly fee. A fee of $5 will
be charged to replace a lost key.
37. 6. Children Ages four and older are not allowed in the
opposite sex lockers. Please use the family locker room.
7. The athletic Club is not responsible for lost or stolen
articles. Please do not bring valuables into the club.
