5. SDH and
Chronic
Disease
The effect of social determinants extends to chronic diseases such as:
Cardiovascular Disease Type 2 Diabetes Cancers
Stroke Pulmonary Diseases Kidney Disease
6. CanSocial
Factors
Institute the
onset?
The simple answer is, yes!
Social factors can initiate the onset of the pathology and in this
way serve as a direct cause for a number of chronic diseases.
7. Smoking and
Chronic
Disease
Smoking is associated with more diseases than other health-
related behavior
Smoking is a learned experience that typically begins socially.
Through this social interaction, paths are created to lung cancer
and various other tobacco-related diseases.
8. Literature
Conclusion
Social determinants of health such as the conditions in which
people are born, grow, work, life, and age all influence health.
Assigning causation solely to biological anomalies has not been
wholly successful in accounting for all relevant factors in some
chronic diseases
9. Discussion
Overview
Share current practices in identifying SDoH
Share what your facility is doing to manage patients with negative
SDoH
Analyze trends
Brainstorm future practices and barriers
10. Discussion
What are some Social Determinants of Health that are prevalent
to your area?
Has your facility noticed a direct correlation between certain
(these) SDoH and certain chronic diseases?
Does your facility ask about SDoH?
How does your facility track SDoH?
What are some barriers that you face to help address SDoH in your
area?
Is continued tobacco use a problem with your cardiovascular
disease patients?
11. Discussion
A new trend is the rise of vaping. Have you noticed a trend in your
area? Are you doing anything to identify patients that vape? What
education are you providing about vaping?
What do you see as the long term health effects of vaping and
how should we be addressing these now?
Do you think this is a new chronic disease of the future?
How has the legalization of marijuana affected your patient
population?
What other new trends do you think will affect the future health of
the population and how should we start thinking about addressing
these now?
How do you target isolation ?
