1. Faradism under pressure
It’s a procedure of treatment where we apply SURGED-FARADIC current along with a
compression bandage to reduce the swelling/ oedema of the extremities.
Plan of mgt.:
1. To remove the fluid from the site of oedema ASAP
2. This can be achieved by the combination of limb elevation,compression of the swelled up
area by compression bandages,and isometric muscle contraction voluntarily by the patient.
3. use of compression bandaging or compression stockings along with iso. Muscle contraction
leads to push the fluid from the tissue interstitial space into the blood or lymphatic vessels.
4. if u are applying compression bandages then you should ponder that wrapping of the
bandage should be from the distal to proximal. The pressure should be applied more at the
distal part and coming toward the proximal part pressure should be gradually decreased.
Application of the pressure should not be felt uncomfortable to the patient
4. Patient should be in his most comfortable position,most preferable position is Supine
lying(lying on back).His treatable limb should be above the heart level so that the removal of
the fluid will be easier.
5. check the skin texture whether there is any rashes,remove the skin hairs and skin should be
properly cleaned , there should not be any oil or lotion on the treated part to get the max
benefit by the treatment
Applicationof the current:1.SF current with surge duration of 30 sec. and interval of 90
sec.
2. Intensity should be enough to get the visible muscle contraction of muscles of the creating
cleanching of toes/fingers.
3. Patient is besingaasked to do voluntary muscle contraction simulteniously with the flow of
the current andrelax during the interval.
2. Faradism under tension
Application of thr SF current over a muscle which is keptin a tension.
Indications:shortening of contractile soft tissue like muscle and some type of connected
tissue in and around joints, post burn contracture, contracture after muscle grafting, etc.
Plan:
1.Tetanic contraction induced by SF ,generates intrinsic tension which is made to contact
against external traction force trying to stretch them apart.
2. Interplay of opposing forces pulls apart the shortened myofibrils ,gradually increasing
length,and much less pain felt by the patient during this than the forced passive movement.
3. The patient is asked to contract the tightened muscle as hard as possible in time with the
surge of stimulation.this leads to quicker release of contractureof tightness as well as power
gain
Example.Suppose a patient having a elbow flexion movt. Restriction/contracture.so he/she
is positioned in a supine lying hand kept aside by the patient over a pillow or cloth roll .The arm
is stabilized with a sand bag placed at the lower end of the arm keeping the fee movt of elbow.
Tension is applied by a sand bag of weighted 1.5-2 kg. placing over the ventral aspect of the
wrist and its strapped.
Placement of electrode:passive electrode placed over prox.1/3rd and active over distal 1/3rd
at the front of the arm.
Applicationof currentis same as the FUP.
