Provincial Health Officer II
Dear Sir,
Greetings!
The UB Psych Clinic would like to ask for your permission and support in conducting an
equipping seminar for select community health team members among Benguet municipalities
from May to June 2015. This project is part of our clinic’s vision and mission of delivering
psychological services to the community. We acknowledge that ensuring the health of everyone
is a noble but herculean task. It would be our honor to do what we can in helping you achieve
this goal. We hope to personally present the program for feedback and guidance from your
municipal health officers on May 7, 2015 or any date you deem appropriate.
Attached is the program for the proposed whole day seminar. It includes free training kit,
meals, and certificate courtesy of Syngenta as part of their corporate social responsibility. We
hope that this endeavour will inspire and further make our community health team members
more effective in performing their mandate.
Thank you so much for your most gracious consideration. Here’s looking forward to
serving your constituents.
Sincerely,
Dr. Liberty L. Catbagan
Registered Psychologist
