Parent-School Partnership Survey
Parents/Guardians of students currently enrolled in Lakewood City Schools are eligible to
participate in this survey. Each parent/guardian may complete a survey (no household limit). For
families with children attending more than one school, you may complete a survey for each of the
schools your children attend, or one survey if you feel that your experience is consistent among
those schools.
If you would like to share your personal story with the Lakewood Council of PTAs email
lakewoodcouncil@gmail.com. All information will be kept confidential.
