Precedence Health Care is inviting qualified health professionals to become Implementation Partners to help practices adopt its new integrated care platform. As a partner, professionals would help practices implement chronic disease management programs using the platform and have opportunities to grow their business through referrals, training opportunities, and access to PHC's marketplace. Partners go through an application process including a questionnaire and agreement. PHC aims to support partners, help them build their referral networks and businesses, and partner for mutual success and growth.

If you are interested in becoming an Implementation Partner we ask that you follow the steps below:

Complete and
submit the pre-
partnership
questionnaire
10. We will review the questionnaire
to see if both parties are suitably
aligned to be mutually beneficial
to both our organisations. We will
then send further documentation
to you for review.
Once both parties agree to
proceed, the partnership
agreement will be signed and
you will be invited to an
induction day at PHC.
