Clinical interpretation of Arterial
Blood Gas for surgeon
Five step sequential steps
1. PH
2. PaCo2
3. Base Excess
4. Urine PH
5. Anion gap
1. pH
• <7.20: Severe acidosis
• <7.35: acidosis
• >7.45: alkalosis
• >7.55: Severe alkalosis
2. PaCO2
• Ventilation 이 환자의 acidosis/alkalosis에
contribute(=Primary) 하고 있는지
compensation(=Secondary) 하고 있는지 여
부를 평가
• >40mmHg: Hypoventilation
• <40mmHg: Hyperventilation
• 인공호흡 중인 환자라면 minute ventilation
의 양을 조절함으로써 PH 교정이 가능하다.
3. Base Excess
• Base excess= corrected bicarbonate –
expected bicarbonate(24mEq/L)
- Corrected bicarbonate= 측정된 bicarbonate를
PaCo2가 40mmHg 일 때로 교정해서 correction
한 것이다.
- Expected bicarbonate= PaCo2가 40일 때의
bicarbonate 정상치로 24mEq/L 이다.
3. Base Excess
• Base excess 값에 따라 Sodium
bicarbonate 를 줄지 Hcl을 줄지 결정한다.
(이 때, PH는 관계없다.)
<-10mEq/L: sodium bicarbonate 투여
>10mEq/L: Hcl 투여
4. Urine PH
• Renal function 이 acidosis/alkalosis에
compensate 하는지 contribute 하는지 여
부를 알 수 있다.
5. Anion gap
• Anion gap= Na-Cl-HCo3
• Normal range: 10~12mmol/L
• Acidemia 의 원인을 알 수 있다.
• >12mmol/L: lactic acidosis, ketoacidosis
• =10~12mmol/L: hyperchloremic acidosis
• <10mmol/L: low albumin, multiple myeloma
오로지 pulmonary function 확인 가능
ABG
1. Estimate
- pH, pO2, pCO2, SO2%, tHb, Hct
2. Calculate
- BE, SBC, HCO3-, tCO2, A, AaDO2, a/A,
PaO2/FiO2
tCO2= 측정된 HCO3- X 1.04
AaDO2 normal range: <27mmHg
PAO2- PaO2 normal range: <15mmHg
