Can maintaining good oral health habits lead to better
vision and hearing? It can with Ameritas Rewards.
Members who visit the dentist at least once a year
can begin earning monetary rewards they can use
for dental and other services covered by Ameritas,
including vision, hearing, LASIK and orthodontia.
If your plan includes Dental Rewards, you know
that visiting the dentist each year and using only a
specified portion of the dental plan annual maximum
benefit means additional benefit dollars to use in
the future.
With Ameritas Rewards, your plan offers even
more rewards:
1. unlimited dental maximum carryover
2. additional types of coverage to enhance your
benefits and promote overall wellness
3. additional benefit dollars to use for services
and materials other than dental
How to Earn Rewards
Members qualify for rewards by submitting at least
one dental claim each year, and keeping total benefits
received for that year at or below the Ameritas
Rewards annual threshold amount. See the example
for details.
Members who visit the dentist but exceed the annual
threshold amount keep their accumulated rewards,
but do not earn new rewards. Members who do not
submit a dental claim receive no rewards, and any
accumulated rewards are lost. But they can begin
building rewards again the very next year.

Dental Rewards®
Employers choose from many different plan designs with corresponding
thresholds and carryover amounts.
PPO Bonus
Members who visit Ameritas dental network providers earn an additional
PPO bonus amount to add to their rewards bank. Network dentists
provide quality care at reduced rates. Network dentists and the PPO
bonus are available in most states.
Ameritas Rewards
Employers specify a portion of the rewards that members can use
for each additional type of coverage – $100 or $200. These reward
amounts are subject to applicable deductible, coinsurance and
plan provisions.
Sample Rewards
The following example is for a member who has dental and vision plans
with Ameritas Rewards.

dentist for an annual cleaning and

• Rewards for hearing and vision apply to annual
maximum benefits.
• Rewards for LASIK and orthodontia apply to
lifetime maximum benefits.
• Rewards for vision are used toward the cost of
frames and contact lenses.
• Orthodontia must be a new treatment program.
• LASIK, vision and hearing coverage must be tied
to dental coverage.
• Rewards for coverages other than dental may
be in the form of reimbursement after the
member has paid the provider.
