Section 1 - Overall Performance Rating
Follow the procedure below when making your Performance Ratings.
1. Refer to the Rating Description and carefully read and understand each of the Performance Evaluation
Factors of the position. Specifically read the Performance Evaluation Factors' definition and Behavioral
Evaluation Standards associated with each performance rating.
2. Using the Behavioral Evaluation Standards, decide which rating best describes the employee's level of
performance for the particular Performance Evaluation Factor based on facts and data. Enter the rating of
each performance factor. It is acceptable to place greater emphasis on a critical bullet point within a
performance as long as the same criteria is applied to all employees who are evaluated via this document.
3. Refer to Overall Performance Rating to determine the employee's overall performance. Click the
Calculator Icon to calculate the Overall Performance Rating. The Overall Performance Rating will appear
based on the weighted rating.
4. In the Comments Section, document specific behavioral examples of the employee's performance,
focusing on specific job-related behaviors, that support your ratings.

procedures. This includes following Federal, state, and local requirements and regulations; all Compliance
policies, the Code of Business Ethics and safety policies/procedures; upholding ethical standards; and

Liz makes sure specimen integrity is kept by following the strict guidelines that Quest has set
in place. Liz helps her co-workers with their job assignments. Uses her knowledge from past
Supervisory position to help resolve problems, able to point out potential problems along with
solutions. She is an example of excellence to her piers...earning the respect of her team
members.

Description Interacts effectively with others and actively participates as a committed member of a team. This
includes interacting with others in a respectful and professional manner; remaining calm in difficult interactions;

managers. Brings problems and solutions to the group meetings. Remains calm in stressful
situations, such as computer outages. Can always be relied upon to stay or come in early in
emergency situations. She is able to prioritize work that is given to her in order to meet the
goals for the day. Liz needs little supervision. she is a trusted and valued employee.
2. Work Orientation
Description Ensures work activities are completed thoroughly, accurately, efficiently, and in a timely manner
ensuring quality standards are met. This includes taking initiative to complete duties; working without close

employee. Excellant attendance, always on time, wilinging to adjust hours as needed for the

her team.
Deliver Quality Services
Description Delivers services that consistently meet or exceed standards with little or no rework required. This
includes verifying the accuracy and completeness of all work, identifying missing or inconsistent information or
data, asking for assistance when appropriate, documenting and sharing work-related problems and solutions with
co-workers, and striving for Six Sigma quality.

major errors during the 2013 year. Needs little to no supervision, is able to find the necessary
resources in order to resolve issues in a timely manner.

Description Identifies work related problems including attempting to resolve the problem, referring to the proper
individual(s) when appropriate, and following appropriate Standard Operating Procedures (SOP). This includes
identifying, diagnosing, and resolving problems by: analyzing information; generating alternatives; making sound,
timely, and objective decisions; and committing to action, even when uncertain.

Liz is able to identify problems, resolve them, figure out alternatives. Works well alone or
within a group. Liz has been of great help to her team with her past experiences in problem
solving. She is able to handle most issues without intervention from a LA or Supervisor. She

Process Specimens
Description Quickly and accurately enters requisitions into computer. Measures specimens to proper amount
required for testing, screening, and/or examination. Aliquots/pours master tubes into new tubes/containers.
Correctly labels/numbers specimens. Prepares and sorts specimens for delivery, examination, screens and/or
laboratory testing. Delivers, stores, and/or retrieves specimens in a timely manner.

Liz is currently at a productivity of 135 per hour..well above the minimum requirement of 90 per
hour. She keeps a neat and orderly work area. No major errors have occurred during the past
6 months. She is a leading example to her piers.
