Responsibilities include but are not limited to;
 Round with medical teams to provide drug information and monitor patients' responses to
their medications.
 Monitor medication orders and review patient profile to ensure appropriate drug selection
and dosage.
 Provide accurate, adequate and timely drug information to the professional staff and to
patients and their families.
 Support the formulary development process by consulting with medical staff and
promoting rational drug therapy selection.
 Actively involved in monitoring antibiotic therapy, total parenteral nutrition (TPN)
therapy, IV therapy, and any medications that require additional laboratory monitoring
such as warfarin, phenytoin, and certain antibiotics.
 Provide patient education in-house and at discharge upon request.
 Arrange discharge prescriptions and provide counseling for patients receiving
anticoagulant therapy i.e. warfarin education and counseling
 Conducts both formal and informal training for nursing, medical and pharmacy staff on
drug utilization, dosage, incompatibles, preferred forms and toxicity.
 Pharmacy representative for Antimicrobial sub-committee of PTC
 Monitor medication orders and review patient profile to ensure appropriate drug selection
and dosage in Internal medicine area
 Preceptor responsibilities include but are not limited to;
o Precept Pharmacy students and interns
o Participate in didactic and experiential teaching, evidence based training, and
service to the department.
o Conduct both informal and formal education programs for MMU.
o Adjusts patients' drug therapy regimen utilizing appropriate pharmacokinetic and
pharmacodynamics principles.
o Detects and documents adverse drug reactions.
o Interviews and counsels patients and/or families and answers drug information
questions and increase medication adherence.
o Provides counseling to patients and/or families on medications in conjunction
with physicians.

Responsibilities include but are not limited to;
 Provide specialized pharmaceutical services to improve drug usage and therapeutic
outcomes including advising physicians on issues concerning drug therapy, the toxicity of
drugs and side effects, as well as assisting in the prescription of appropriate doses.
 Maintain liaison relationships with medical and nursing staff; provide timely information
pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal
regulations regarding drug controls, and Joint Commission standards.
 Conduct and evaluate medication histories, assess compliance and suggest modifications
so as to achieve desired outcomes; instruct patients in the proper use of prescribed drugs.
 Individualize medication regimens using sound principles, accounting for
pharmacodynamics and pharmacokinetic variations in drug absorption, distribution,
metabolism and elimination with responsibility for establishing and continually
improving the delivery of pharmaceutical care to patients within areas of direct
responsibility and assisting others in the department with the same.
 Reviews and interprets orders and prescriptions written by authorized prescribers to
assure accuracy and appropriateness prior to entry onto the patient profile.
Assesses relevant patient data, in conjunction with prescribed therapies and makes
appropriate pharmaceutical care interventions.
 Pharmacokinetic consults on antibiotics, antiepileptic, anti-arrhythmic and anticoagulants
 Anticoagulation monitoring of patients on thrombolytic and anticoagulants such as
Warfarin, Argartroban, Arixtra and Prasugrel.
 Participate in interdisciplinary committees
 Maintain interface with medical and nursing staff to provide regular information
 Participate in Hospital Based Clinic policy and procedure development and ensure
compliance with all regulatory practice standards.
 Suggest performance improvement plans and assist with implementation
