Implementation Plan and Timeline
Purpose: All students will learn what we want them to know and to the depth of understanding we want
them to know it, and will be able to transfer the skills and knowledge learned to familiar contexts in their
present world and unfamiliar contexts in their future world.
Commitment: Our commitment is to ensure that every student engages in classroom activities that require
the kind of deep reading, thoughtful writing, and informed discussion that prepares him to apply these skills
as habitual behavior patterns to new and different circumstances in his future.

How/when will you monitor this Activity? Evidence for documentation?
Formative Assessment- DIBELS
 DIBELS Benchmarks: August, January and May
 DIBELS Progress Monitoring- All students will be progressed monitored at least once per month.
How will you measure Impact of this Activity on Student Learning? Pre-Post Evidence for
documentation?
Students will create project-based activities to promote literacy skills. Students will upload projects into
their Edmodo folders, which will serve as portfolios. In addition, the DIBELS _____ Test will reflect
understanding of word meaning and decoding skills. Finally, 2014 CRCT Science test scores will be
compared to 2013 test scores to determine if literacy test scores have been impacted in the 4th
and 5th
grades.
How will you ensure Quality of the process/product/work done? Evidence for documentation?
Students will complete personalized learning assignments by pursuing information of interest. They will
use computer-generated rubrics to judge their performance. In addition, third parties will administer
DIBELS Benchmark tests.
