1. Bio:
Danette Core PMHCNS-BC is Health Home Manager of Physical Integration at Zepf Center; she
provides clinical and administrative leadership to the Health Home team, oversees integration
projects between Zepf Center and the medical community, manages aspects of physical
education for staff and consumers and develops prevention/ wellness programs. In addition to her
work at Zepf Center Ms. Core works at a local community hospital providing direct patient care,
supervision and mentoring on acute care.
Her educational history includes a Bachelors of nursing from Lourdes College in 1997, Masters
of Liberal Studies; focus on disability studies and educational psychology from the University of
Toledo 2006, Masters in Nursing/ Psychiatric CNS; University of Toledo, 2012. She currently is
enrolled at the University of Michigan for a certification in Integration Primary Care and
Behavioral Health. She is credential through the American Nurses Credentialing Center and is
board certified through the State of Ohio. Academic honors include: Lourdes college:
Excellence in Public Health Nursing; Outstanding Nursing Leadership Award, Toledo District,
Ohio.
Ms. Core is an active member with the American Psychiatric Nurses Association and the Ohio
Association of Advance Practice Nurses. Career highlights are as follows: Psychiatric
Community Liaison Nurse, University of Toledo Medical Center; responsibilities included
designing community outreach program , Alzheimer’s Association Northwest Ohio Chapter,
duties performing physical assessments and health history documentation for Passport
certification , Clinical Nurse Affiliate for Gambro Dialysis which included hospital credentialing
at three Toledo Hospitals, responsible for ensuring continuum of care for inpatient clients. This
included but was not limited to, making rounds, performing complete assessments evaluating
ordered diagnostics and writing doctors’ orders after discussing client with physician. Full-time
Lecturer in the College of Nursing at the University of Toledo Medical Center, responsible for
the development of the curriculum and course content for Nursing students in advanced medical-
surgical nursing. Ms. Core has knowledge in addiction medicine from her experience working at
St. Vincent’s Medical Center Tennyson Center.
Papers and research include: 2004 member of a multi-disciplinary team with department of
criminal justice and social work department researching the stigma associated with mental health
and drug abuse; refereed publications; Baldwin, C., Hibbeln, J., Herr, S., Lohner, L., Core, D.,
(2002). Self-Care: As Defined By Members of the Amish Community Utilizing The Theory of
Modeling and Role-Modeling. The Journal of Multicultural Nursing & Health, vol. 8, (3), pages
60-64. Ms. Core is currently a member of three researching and implementing nutrition programs
for community group homes.
