service. Furthermore, I checked patient’s prescription before dispensing using a BNF and labelled
the medications. I learned the importance of keeping good record of POM medicines.

During this placement, I had a case study on a child with a presenting complaint of
Hirschsprung’s Disease end Ileostomy. The information of the patient were obtained from
patient’s notes, the pharmacist in charged and patient’s care taker. I did a presentation on this
case. Through this placement, I had a few conversations with both pharmacist and patients,
analysed patient’s past medical history and medications.

2. During this attachment, I was trained to do labeling for different type of drugs including tablets,
capsules, solution and the others. I also begin to develop the knowledge of high standards of
ethical, legal, and professional conduct. Besides that, I observed the interactions between
pharmacist and patients. Apart from that, I learnt to find examples of medicines that have
different licenses under different circumstances, example are P and POM doses. I had become
more familiar with different prescription types and able to explain the differences between them.
Additionally, I gain awareness on medicines information resources that are available to a
pharmacist.
Guardian Pharmacy Attachment year 1 (2014)
Guardian Pharmacy attachment in KLCC gives me an insight into various aspects especially in
observing the sales of an OTC medicine. I was given the opportunity to check the prescribed

pharmacy attachment provided me in how to investigate the range of dosage forms for a single
medicine such as tablet, capsules, solution and so on. I was also exposed to perform stock check
