1. Mission Statement
SALVATION ACADEMY is dedicated to
providing exceptional, cost effective, family fo-
cused care for the medically complex patient. Our
goal is to meet the patients’ medical and personal
care needs in the nurturing environment of their
homes, minimize the impact of the patient’s
condition on the family while respecting the role
of the family in the care of the patient. Agency
Objective
We maintain quality Home Health Care Programs
consistent with the agency’s Mission Statement
and the Patient’s Bills of Rights.
Goals: To ensure that all patients are provided
with quality, comprehensive health care services
through a coordinated Plan of Care to provide
continuity of health care to facilitate unin-
terrupted service within the health care system.
To promote early recovery or maximum rehabil-
itation in the patient’s home environment.
To provide effective channels of communication
with the patient, physician and other organiza-
tions and providers within the health care com-
munity. To foster professional and personal sat-
isfaction of all employees by maintaining a posi-
tive environment that stimulates pride and pro-
fessional growth.
SALVATION ACADEMY HOME HEALTH
functions as a Licensed Home Health and
Community Support agency providing multi-
disciplinary companion care, short and long
term care on an intermittent part-time and
full-time basis to patients of all ages, to include
children, young adult and geriatric patients.
Services provided directly include:
Skilled Nursing, Physical Therapy,
Speech Therapy, and Occupational
Therapy, Intravenous Therapy, and
Medical Social Worker services,
Psychiatric Services, Nutritional
Services, Home Health Aide Services
and Companion
Services are provided to patients with medical,
diagnosis and personal care needs. Care is
delivered in the patient’s home at the
physicians and or family member’s request,
frequency; on-call as needed basis, after hours,
on holidays and weekends.
Services are available on
a 24-hour basis seven
(7) days a week as
deemed necessary by the
patient’s condition.
When all administrative personnel will be
out of the office during business hours, a
note will be posted on the door and the
answering service will provide infor-
mation on how to contact the person in
charge

2. ADMISSION QUALIFICATIONS
The patient will be accepted on the basis of
the following criteria:
1. The patient is under the care of a
licensed physician or legal guardian
who will provide orders for services
2. The physician or legal guardian or
representative will review the need for
and risks of all services and therapies
provided before initiation of the
services.
3. The patient’s diagnosis and therapy and
personal care needs shall be appropriate
and can be adequately met in the home
environment.
4. The patient and/or caregiver shall agree
to accept the home care services.
5. Ability and availability of home care
staff and resources to provide necessary
services to the patient.
6. Safe environment exists and adequate
facilities in the patient’s residence to fa-
cilitate proper delivery of care and
ensure the safety of the patient and
home care personnel. The home shall
have a functioning telephone. If the
telephone is not in the home,
arrangements shall be made with ease
of communication and access.
7. Patient must live within the services
area as stated in our policy.
8. Acceptance will not be influenced by
race, religion, age, sex, national origin,
marital status, or physical/mental
handicap.
9. There is a written plan of treatment
reviewed and approved by the physician
or legal guardian or family member and
reviewed at least every (60) days.
10. The Medicare patient will be
homebound. Each patient will have an
approved diagnosis, financial coverage
verified and initial assessment done.
OUR PERSONAL CARE SERVICES
Our personal care services does not need a
physician's order and may include but are not
limited to:
Assistance with nutrition and diet activities
such as shopping and meal preparation;
Performance of household services such as
changing bed linens, making beds, washing
dishes, cleaning the kitchen, dusting and
vacuuming rooms, shopping for essential
supplies;
Assistance with basic personal care such as
bathing, grooming, bathroom and/or bedpan
routines, walking, transferring from bed to
chair or wheelchair; and
Assistance with self-administration of medi-
cations.

We provide Home Health Care Services for

such as Bodyguard, Parking Security
and security Guard Services.
We also provide advanced special
operations services if the client
demands it. Our clients are our
KINGS.
Our services include the prevention of
unauthorized activity or entry, traffic
regulation, access control, and fire
and theft prevention and detection.
We provide other services when
requested. .

Academy Inspiration Ministries
(HOFSAIM) teaches many basic
Christian doctrines, such as the Bible
being the inspired Word of God, the
creation and fall of man, repentance,
divine healing, communion, foot
washing, the second coming of Jesus,
the millennium, and final judgment.
All of these are covered in the Articles
of Faith. We believe in one God. We
believes that God manifested, or made
himself known, in different ways: as
the Father in creation, the Son in
redemption, and the Holy Ghost in
emanation.
