A 34 year old short statured man presented with complaints of painful swellings in both groin. These swellings were present since childhood but recently increased in size along with pain and stiffness of both the hips. Examination revealed multiple bony hard swellings, of about 15 cm in diameter, in both groin and painful restriction of movements of both hips. There were similar multiple bony hard swellings present in right scapula, left proximal tibia and bilateral distal femora.

A 34 year old short statured man presented with complaints
of painful swellings in both groin (Fig. 1Panel A). These
swellings were present since childhood but recently
increased in size along with pain and stiffness of both the
hips. Examination revealed multiple bony hard swellings, of
about 15 cm in diameter, in both groin and painful restriction
of movements of both hips. There were similar multiple bony
hard swellings present in right scapula, left proximal tibia
and bilateral distal femora. His sibling had similar asymp-
tomatic swellings all over the body. Radiographs revealed
large sessile osteochondromas arising from bilateral femoral
neck and from the right pubis (Fig. 1Panel B, C). Hereditary
multiple exostoses is an autosomal dominant condition
associated with multiple osteochondromas which lead to
skeletal deformities, short stature, limb length discrepancy
etc. Acetabular and proximal femoral involvement is re-
ported in approximately 90% such cases and may result in
secondary osteoarthritis.
