  2. 2. <ul><li>Poisons of this groups exhibit well-marked deliriant effect.
  3. 3. Eg: 1. Datura</li></ul> 2. Atropa Belladonna<br /> 3. Hyoscyamus Niger<br /> 4. Cannabis Indica<br /> 5. Cocaine.<br />
  4. 4. DaturaFastuosa ( Dhatura)<br />Types: 1. Datura Alba<br /> 2. Datura Niger<br />
  5. 5. Flowers: Bell shaped<br />Fruits: Spherical with spines<br /> contains 400-500 seeds<br />Seeds: Yellowish-brown resembling capsicum seeds.<br />
  6. 6. <ul><li>All parts or the plants are poisonous.
  7. 7. Seeds and fruits are most toxic.
  8. 8. They contains 0.2-0.4% of hyoscine, hyoscyamine and traces of atropine.</li></li></ul><li>Mode Of Action<br /><ul><li>Exerts central and peripheral action
  9. 9. Mainly anticholinergic effects</li></ul>CNS, CVS, Eyes, Smooth Muscles, Glands, Body Temp, Local Anasthetics, decreases bronchial secretion, etc.<br />
  10. 10. CNS Effects<br /><ul><li>In small doses stimulates CNS
  11. 11. Large doses depresses CNS, causes delirium and coma.</li></li></ul><li>Peripheral Effects<br /><ul><li>Depression of cholinergic fibers with parasymphatic paralysis.</li></li></ul><li>Signs/Symptoms<br /><ul><li>Hot as hare.

  14. 14. Blind as bat.
  15. 15. Mad as hen</li></li></ul><li>These features appear in two stages:<br />Stage of delirium<br />Stage of coma<br />
  16. 16. Stage of delirium<br /><ul><li>Onset: few min. to ½ hour.
  17. 17. Dry as bone: Mouth & throat will be dry
  18. 18. Intense thirst
  19. 19. Difficulties in talking and swallowing
  20. 20. Burning pain in abdomen</li></li></ul><li><ul><li>Dizziness
  21. 21. Confusion
  22. 22. Drunken gait
  23. 23. Inability to stand
  24. 24. Hoarseness of voice/aphonia
  25. 25. Hot as hare: Skin becomes dry, hot & exfoliatiated</li></li></ul><li><ul><li>Body temp. may rise as high as 105-107 F
  26. 26. Red as beet: Face becomes flusied, conjuntiva red.
  27. 27. Blind as bat: Pupils dilated, photophobil, diplopia& loss of accomodation.</li></li></ul><li><ul><li>Pulse: increased
  28. 28. Respiration: increased
  29. 29. Muscle tone: decreaseedeep reflx. muscular spasm.
  30. 30. Convulsion.</li></li></ul><li>Delirium sets in as patient may become restless, noisy, violent and may have to be kept controlled in bed by several persons, as he is inclined to run away from bed and wander about.<br />
  31. 31. <ul><li>Mad as hen: Person may be silent or may mutter indistinct, incoherent & inaudible words.
  32. 32. Visual, auditory hallucination</li></li></ul><li><ul><li>Show certain ludicrous movements
  33. 33. Grasping imaginary objects
  34. 34. Picking up bed clothes; imaginary threads from the tips of his fingers(car phologia)</li></li></ul><li><ul><li>May be in fits of laughter and at other times in tears.
  35. 35. a/c delirium may pass off in an hour or so, when patient becomes drowsy & silent.</li></li></ul><li><ul><li>Person begins to improve & awakes with severe headache, vertigo, nausea and vomiting.
  36. 36. Vision remains disordered for a few days until dilatation of pupil subsides.</li></li></ul><li><ul><li>Finally the patients recovers completely and remembers nothing of his illness or events that had happened.</li></li></ul><li> Stage of Coma<br /><ul><li>Occurs when drowsiness passes into stupor followed by convulsions and coma.
  37. 37. Gen. sings of deterioration
  38. 38. Death usually results from respiratory depression</li></li></ul><li><ul><li>Datura is excreted unchanged in urine for 10-20 Hrs. after ingestion.
  39. 39. Urine should always be preserved for chemical analysis</li></li></ul><li><ul><li>Fatal Dose: 100-125 seeds confirms death
  40. 40. For stupefacation:50-60seeds
  41. 41. Fatal Period: within 24 Hrs.</li></li></ul><li>Treatment<br />Stomach wash: Pot. Perm.<br /> 4-5% tannic acid.<br /> strong tea, coffee.<br /> purgatives by mouth.<br />2. Emetics<br />
  42. 42. 3. Physostigmine 1mg i.v./i.m. per hour for 2-3 hours.<br />4. Pilocarpine 5mg s.c.<br />5. Short acting barbiturates for delirium.<br />6. Light diet <br />
  43. 43. Post-mortem Appearance<br /><ul><li>Not characteristic.
  44. 44. Cyanosis
  45. 45. Dilated pupils
  46. 46. Gen. signs of poisoning
  47. 47. Seeds resists putrefication hence can be recovered from decomposed body.</li></li></ul><li>Medico-legal Aspects<br /><ul><li>Rarely used for homicide or sucide.
  48. 48. Used as stupefying agent for theft, robbery, rape or kidnapping.
