23. Statine nelle SCA
• L’evidenza indica che nei pazienti con SCA la terapia con
statine deve essere iniziata subito e con alte dosi anche
allo scopo di attivare i potenziali effetti pleiotropici ad essa
correlati
• L’atorvastatina (alla dose di 80 mg/die) è la statina
dimostratasi più efficace e sicura
• E’ ragionevole che la terapia con alte dosi debba essere
continuata fino alla stabilizzazione del quadro clinico
indipendentemente dai livelli di colesterolo (4-6 mesi
dopo l’evento acuto)
• La sospensione o lo “switching” tra statine o verso dosaggi
più bassi è rischioso potendo aumentare in maniera
significativa l’incidenza di MACE (tanto più quanto più
precoce)
• Nella fase di stabilizzazione i livelli di LDL colesterolo
(<100 mg/dl o < 70 mg/dl) costituiscono l’indicatore più
adeguato per la titolazione del dosaggio delle statine

La maggiore efficacia di rosuvastatina nel ridurre la mortalità cardiovascolare è conseguenza non solo di un maggiore effetto sulle lipoproteine ed apolipoproteine plasmatiche e sul metabolismo lipidico in genere, ma anche a una particolare efficacia sull’infiammazione vascolare. Questo effetto di rallentamento dello sviluppo e progressione dell’aterosclerosi è legato a una maggiore all’inibizione HMG-CoA reduttasi rispetto alle altre statine.
Nelle placche stabili, il cappuccio fibroso rappresenta fino al 70% del volume. Le placche instabili hanno un cappuccio fibroso sottile e il core lipidico rappresenta la maggiore componente. Falk ha dimostrato che circa l’86% degli infarti miocardici sono causate da stenosi inferiori al 70%, non aggredibili angiograficamente. E’ quindi importante la stabilizzazione delle placche.
