– Forces of Change Assessment (3 hour meeting): Focuses on
the identification of forces such as legislation, technology and
other impending changes that affect the health of our
community. January 2013
– Community Themes and Strengths Assessment (2 hour
meeting): Provides insight into the thoughts, opinions and
concerns of the community. February 2013
– Local Public Health System Performance Assessment (all day
event): Identifies capacity and capability of the local public
health system. March 2013

• What we have learned:
– A community health assessment must be done in
partnership with outside agencies
• And that it is continuous
– Internal DOH CHA committee must represent all
Divisions within a Health Department
• Environmental data was absent from our CHA process
– Community requesting analysis of data at a smaller
geographic area than County level
– Difficult to complete a CHA when data isn’t easily
accessible
