Home Ventilator Transitions Overview
 This training deck will assist you with identifying key changes in
the Kaiser/Apria home ventilator transition process.
 All ventilator transitions into the home will be over sighted by the
Kaiser SCAL Regional Clinical Manager
 All ventilator orders will be processed as a home assessment

Ventilator Discharge Process
 Appointment made to assess home environment
 Teleconference scheduled with key stakeholders
 Identified topics discussed regarding patient’s care
 Ventilator delivered to hospital to initiate caregiver and
clinical training
 Equipment and supplies delivered to patient’s house
 Closing conference to discuss planned discharge
 Apria RT to meet patient at home upon hospital
discharge
 Follow up email on discharge status sent to key
stakeholders
