the Kaiser/Apria home ventilator transition process.
All ventilator transitions into the home will be over sighted by the
Kaiser SCAL Regional Clinical Manager
All ventilator orders will be processed as a home assessment

Ventilator Discharge Process
Appointment made to assess home environment
Teleconference scheduled with key stakeholders
Identified topics discussed regarding patient’s care
Ventilator delivered to hospital to initiate caregiver and
clinical training
Equipment and supplies delivered to patient’s house
Closing conference to discuss planned discharge
Apria RT to meet patient at home upon hospital
discharge
Follow up email on discharge status sent to key
stakeholders
