PRIMARY CARE CLINICAL PROGRAM
INTERMOUNTAIN DIABETES PREVENTION PROGRAM
IN EARLY 2013, THE PRIMARY CARE CLINICAL PROGRAM BEGAN WORK ON THE CREATION OF A DEFINED
DIABETES PREVENTION PROGRAM (IDPP) TO PILOT WITHIN PRIMARY CARE CLINICS AT INTERMOUNTAIN.
Previously, there was no systematic way to identify patients who met the criteria of preDM and there was no
standardized way for providers to refer patients to existing wellness programs. This program identifies prediabetic
patients who meet prediabetes criteria for clinicans, provides an introductory class focused on awareness,first steps,
and goal setting for people with preDM, and leverages existing initiatives (ie. Weigh to Health and Medical
Nutrition Therapy) for intensive lifestyle and behavior change programs that have demonstrated effectiveness in the
peer-reviewed literature. The program is designed to fit within exisiting Medical Group clinics and leverages
existing Registered Dieticians from within the medical group and hospital.
Key components of this program includes:
 100% of patients can begin participation through a 2 hour introductory class: Prediabetes 101
 Primary goal of program: To achieve a 5% weight loss at 6 months
 Evaluation will include: Change in weight, HbA1c,FPG, BP, BMI, lipid profile, physical activity, chronic
disease incidence, and utilization of MNT and Weigh to Health
 8 regions have begun implementation of Prediabetes 101 class
 Education curriculum, EMR documentation, and clinical flow process are consistent system-wide
System Level Enrollment Data (as of August 31st, 2015)
- 18,124 patients within IMG have been identified in last three years by HbA1C or FBS
o 4272 additional IMG patients have Prediabetes,Glucose Intolerance, or Impaired Glucose Tolerance on
the problem list
- 335 patients have been invited to participate in the DPP through the referralprocess in CPOE*
- 2179 distinct patients have participated in DPP
 72 Cache Valley Group
 457 Central Salt Lake Group
 8 Central Rural Group
 258 North Salt Lake/South Davis Group (includes North SL Region)
 140 South Salt Lake Group
 373 Southwest Group
 242 Timpanogos Group
 361 Weber/North Davis Group

o 1470 taught through Prediabetes 101 class (Prediabetes Introduction class)
o 776 patients have participated in MNT
o 257 patients have participated in W2H
