star in Pandemic Pictures Horror film production as they have
completed the audition process successfully.
As your child is under the age of 16 years we need to have permission
for them to star in this AS level production. Please sign the consent form
below.
We hope that your child will be able to star in the film and is excited
about it as we are at Pandemic Pictures.
I do/don’t give my son/daughter permission to be involved in Pandemic
Pictures production.
