1. Social Work Services
Upon admission, nursing staff assesses the patient and
family for existing needs and concerns. When entered during
the initial interview the following will trigger an auto-
response referral for Social Services. These auto-response
referrals include, but are not limited to:
• Suspected abuse
• Significant self-care deficit at home
• Restraint use at home
• Psychiatric admissions
• Readmissions within 30 days
Once the auto-referral is made, Social Services will see these
patients after they have completed any current in-progress
referrals
2. Social Work Services (cont)
When patients report financial
concerns regarding
hospitalization, these are
referred to the Business office
not Social Services
Social Workers notes will be
documented electronically and a
copy will also be placed on the
chart under Progress Notes
3. Social Work Services (psychiatric care)
When patients in psychiatric crisis are admitted, often the
physician requests evaluation by HRA (Health Resources of
Arkansas). With these evaluations, note the following:
• HRA is available for referral 24 hours/day, 7 days/week
• Patients must be medically stable for possible transfer to
inpatient psychiatric treatment the day the referral is made
• HRA is a resource of uninsured patients
• Patients with private insurance will receive planning via WCMC
social services
4. Social Work Services (psychiatric care)
Physicians may also request a referral for Compass or Clearview.
Social Services is available to assist with these referrals but if the
request is made during the Social Services’ off hours nursing
CAN make the referral.
When making the referrals please remember the following:
When calling Clearview/Compass have the demographic
data, recent vital signs, any abnormal lab, whether or not the
patient wears oxygen, has aggressive behavior, or any history of
substance abuse available to report and if the patient is
ambulatory or not.
• The patient must be medically stable
• The patient must agree to admission
