Desktop/Laptop Leasing Refresh Program
EXECUTIVE OVERVIEW
The H. Lee Moffitt Cancer Center & Research Institute (MCC), located in Tampa, Florida, began
operations in 1986. As an academic and research medical center, MCC is the only National Cancer
Institute-designated oncology research institute in Florida and one of the Southeast's leading cancer
centers.
Comprised of an inpatient facility, ambulatory outpatient surgery center, ambulatory clinics, a cancer
screening facility and research laboratories, MCC offers a sophisticated network of services and
technologies that assure the citizens of its region convenient, cost-effective, high quality health care.
Moffitt’s workforce is currently comprised of approximately 5300 employees, 700 medical residents, 600
volunteers, and 1000 students and interns.
OBJECTIVE
Moffitt Cancer Center is seeking a company to review its current desktop/laptop leasing refresh program
and looking for a leasing partner to provide leasing solutions through a master lease program. The intent
of this RFP is to identify a single experienced leasing partner that will work collaboratively with our
Integrated Solutions partner and our individual local banking partners to provide a comprehensive leasing
program at competitive rates and with fair, straight-forward terms.

This Request for Proposal shall be conducted under the following time line, which is subject to change
only upon prior approval by Purchasing and granted to all vendors.

VENDOR PRE-SUBMISSION CONFERENCE
Moffitt Cancer Center will conduct a vendor Pre-Submission conference call to further clarify and discuss
the requirements of this RFP on October 17th

On the date indicated in the timeline section of this RFP Overview, your bid must be received via e-mail to
contact listed above no later than 2:00pm EST.
RFP CONTENTS
The RFP package includes the following documents, which require response as part of the vendor’s
proposal as indicated:
1. Cover Letter (e-mail)
2. Overview Document
3. Vendor Acknowledgement Form – requires response
4. Supplier Diversity Utilization and Subcontracting Plan – requires response
5. Statement Of Work/Need Requirements – requires response
Your response should be provided in electronic format. All responses will be confidential.
AWARD CRITERIA
The award of this Request for Proposal is subject to terms and conditions contained herein and any that
will be developed by Moffitt Cancer Center during the Request for Proposal process to augment purchase
order conditions of purchase.
Quality of service, pricing, products, Supplier Diversity and other terms of purchase will be an integral part
of the decision selection process.
If you are awarded this bid, a guideline will be developed that will quantify, monitor, and provide a plan for
cure of deficiencies which shall include, but not be limited to, reimbursement of personnel and
administrative costs, monetary assessment for continual deficiencies, and possible cancellation of
agreement.
We reserve the right to award this agreement in whole or in part to the vendor that can best meet Moffitt’s
business needs.
H. Lee Moffitt Cancer Center assumes no responsibility and bears no liability for costs incurred by a
Company in the preparation and submittal of a quote proposal in response to this RFP.
STATEMENT OF WORK
The selected vendor must prepare an overview document that concisely condenses and highlights the
contents of the overall leasing services proposed in such a way as to provide the Moffitt Cancer Center
with a broad understanding of the entire proposal. They must also include their detailed response to the
Moffitt Cancer Center’s needs and objectives as they relate to detailed requirements specified below.
Detailed requirements:.
Lessor Information
1. Describe your company structure including parent and/or subsidiary companies.
2. Provide a brief company history and a financial overview.
3. Is your company a certified Minority, Women, Veteran or Service Disabled Veteran Business
Enterprise (MBE/WBE/VBE/SDVBE)?
4. Provide Supplier Diversity Utilization and Subcontracting Plan in accordance with the
requirements outlined in the RFP.
5. Describe how long you have been in the IT leasing business and your experience with a quarterly
lease program tailored to handle large quantities of equipment?
6. List the key people who will be servicing the Moffitt Cancer Center account.
Master Lease
1. Provide a copy of your company’s Master Lease agreement.
2. Describe your company’s flexibility related to specific language contained within the Master Lease
agreement.
Lease Rate and Other Costs
1. Provide your lease rates for the equipment for 24, 30, 36 and 48 month terms.
2. Are lease rates locked in for the life of the asset’s lease term?
3. How often are your lease rates changed and how is this communicated.
4. Do you provide early termination options?
5. List all administrative fees or other up-front or end-of-lease fees.
6. Are there restocking fees for equipment returned at the end of lease?
7. Describe any limitations to the Cancer Center’s choice of banking partners used to fund the
program.
Equipment Acquisition
1. Describe the equipment ordering and acceptance process.
2. Describe any limitations to our choice of vendors and/or manufacturers.
3. Can equipment from multiple vendors be combined on lease schedules and invoices?
4. Do you provide electronic signature options for lease schedules?
Invoicing
1. Provide what information is included on your invoices and to what extent invoices are
customizable.
2. Provide any relevant supporting documents to assist us in understanding your processes and
capabilities in regard to invoicing.
3. Describe your back-office support from the time of acquisition through the invoicing process.
4. Describe your payment terms and options.
Asset Information and Reporting
1. Describe all asset information and reporting you will provide regarding leases and leased
equipment.
2. Do you have an on-line system to track and provide this information?
3. Can you provide monthly Subcontract Expenditure Reports to Moffitt Cancer Center identifying
certified MBE/WBE/VBE/SDVBE participation that lists total payments made to subcontractor(s)
in accordance with the requirement outlined in this RFP?
Lease Term
1. Describe how equipment upgrades are addressed.
2. Describe how lease extensions are addressed.
End of Lease Services
1. Describe your end of lease administrative processes.
2. Describe the options available at the end of the lease.
3. Do you accept like for like returns?
4. Describe how buyout is determined and any resolution processes.
5. Provide your packing and shipping requirements and whether peripheral items are required to be
returned.
6. List all returns locations and whether they are company-owned facilities.
7. Describe any end of lease services that will be subcontracted or outsourced.
8. Describe your procedures and cost (if any) of protection for data remaining on hard drives and
whether this service is subcontracted or outsourced.
9. Provide an example of the certificate provided proving sanitization for each asset.
10. Describe how non-working equipment is disposed of and whether this service subcontracted or
outsourced.
11. Describe how you sell or dispose of off-lease equipment and whether this service is
subcontracted or outsourced.
12. Describe all end of lease reporting and cost (if any).
13. Describe when and how this information is made available.

SUPPLIER DIVERSITY UTILIZATION AND SUBCONTRACTING PLAN
REQUIREMENT
Moffitt Cancer Center recognizes the importance of supplier diversity in all aspects of our business and
procurement practices and actively encourages the development, utilization and economic growth of
certified Minority, Women, Veteran and Service Disabled Veteran-owned Business Enterprise
(MBE/WBE/VBE/SDVBE). Central to this initiative is the inclusion and participation of a diverse group of
vendors doing business with Moffitt Cancer Center and as such, Moffitt encourages the participation of
MBE/WBE/VBE/SDVBEs in its procurement process both at the prime vendor level as well as at the
subcontractor level of its prime contracts. Moffitt Cancer Center is committed to a comprehensive

  • 1. Request For Proposal (RFP) 19-07-SSP Desktop/Laptop Leasing Refresh Program EXECUTIVE OVERVIEW The H. Lee Moffitt Cancer Center & Research Institute (MCC), located in Tampa, Florida, began operations in 1986. As an academic and research medical center, MCC is the only National Cancer Institute-designated oncology research institute in Florida and one of the Southeast's leading cancer centers. Comprised of an inpatient facility, ambulatory outpatient surgery center, ambulatory clinics, a cancer screening facility and research laboratories, MCC offers a sophisticated network of services and technologies that assure the citizens of its region convenient, cost-effective, high quality health care. Moffitt’s workforce is currently comprised of approximately 5300 employees, 700 medical residents, 600 volunteers, and 1000 students and interns. OBJECTIVE Moffitt Cancer Center is seeking a company to review its current desktop/laptop leasing refresh program and looking for a leasing partner to provide leasing solutions through a master lease program. The intent of this RFP is to identify a single experienced leasing partner that will work collaboratively with our Integrated Solutions partner and our individual local banking partners to provide a comprehensive leasing program at competitive rates and with fair, straight-forward terms. TIMELINE This Request for Proposal shall be conducted under the following time line, which is subject to change only upon prior approval by Purchasing and granted to all vendors. Issuance of Bid 10/05/2018 Return of Intent to Bid 10/12/2018 Vendor Conference Call 10/17/2018 Bid Packages Due from Vendors 11/01/2018 Award of Bid TBD VENDOR PRE-SUBMISSION CONFERENCE Moffitt Cancer Center will conduct a vendor Pre-Submission conference call to further clarify and discuss the requirements of this RFP on October 17th , 2018 11:00am-12:00pm EST: 800-206-6032 Conference ID: 7457113. On the date indicated in the timeline section of this RFP Overview, your bid must be received via e-mail to contact listed above no later than 2:00pm EST.
  • 2. RFP CONTENTS The RFP package includes the following documents, which require response as part of the vendor’s proposal as indicated: 1. Cover Letter (e-mail) 2. Overview Document 3. Vendor Acknowledgement Form – requires response 4. Supplier Diversity Utilization and Subcontracting Plan – requires response 5. Statement Of Work/Need Requirements – requires response Your response should be provided in electronic format. All responses will be confidential. AWARD CRITERIA The award of this Request for Proposal is subject to terms and conditions contained herein and any that will be developed by Moffitt Cancer Center during the Request for Proposal process to augment purchase order conditions of purchase. Quality of service, pricing, products, Supplier Diversity and other terms of purchase will be an integral part of the decision selection process. If you are awarded this bid, a guideline will be developed that will quantify, monitor, and provide a plan for cure of deficiencies which shall include, but not be limited to, reimbursement of personnel and administrative costs, monetary assessment for continual deficiencies, and possible cancellation of agreement. We reserve the right to award this agreement in whole or in part to the vendor that can best meet Moffitt’s business needs. H. Lee Moffitt Cancer Center assumes no responsibility and bears no liability for costs incurred by a Company in the preparation and submittal of a quote proposal in response to this RFP. STATEMENT OF WORK The selected vendor must prepare an overview document that concisely condenses and highlights the contents of the overall leasing services proposed in such a way as to provide the Moffitt Cancer Center with a broad understanding of the entire proposal. They must also include their detailed response to the Moffitt Cancer Center’s needs and objectives as they relate to detailed requirements specified below. Detailed requirements:.
