The inspector forget to write the PP information on the internal COC, why?
When she finished writing the S/N on COC, she thought she finish this batch and took
some rest, why?
She was careless and didn't finish her work.

Ⅰ: The packer didn't follow the internal COC to print the tag & Bag label, why?
He found the internal COC misses the PP information, and this PP was not
cancelled, so he added the PP information on the Tag & Bag label, but he didn't talk
with inspector about this, why?
He forgot to talk to inspector when he finished his work.
Ⅱ: The PP information is only on the Tag & Bag label, nor on the outer packaging,
why?
The packer thought the PP information just take it on the Tag & Bag, the
concession information need to take it on the Tag & Bag and outer packaging, as
same as the CEO, why?
They didn't fully understand the standard about packaging.
For Process ④:
COC 150114-7 consists of 2 batches BRR23319 (207708 & 207651). Batch 207708
has 9 off parts. Batch 207651 has 11 off parts, the concession 210763672 CAT2 is
just on the 207651, why?
The batch 207651 has 20 off parts, these parts were put into 2 boxes, the last box
has 11 off parts, the packer take some new labels replace the old Tag & Bag label,
missed one box, why?
It was about to delivering when he made the labels change, the time was closed,
why? (take the change on Feb.9, on Feb.10, go to RR)
The packer didn't make the label change in time when the inspection controller
informed him about this, why?
He was not serious about working and nobody went to track it.

Action ③ Ⅰ: Ask the packer and CEO take a communication with the relevant people
