This document summarizes deficiencies found during a survey of the Safire Rehabilitation of Southtown nursing home. Surveyors found that the facility had not made the results of their most recent survey available for residents to view, in violation of regulations. Additionally, the facility failed to obtain stool samples as ordered by a physician for one resident, potentially harming their health. Finally, for one resident with a healing pressure ulcer, the facility did not fully implement a physician's order to apply skin protectant to the wound area, putting the resident at risk of further skin breakdown.
