NAME CHANGE POLICY
NAME CHANGE POLICY
All requested name change will be processed within fourteen (14) working days.
Distributors/clients may request for name change if:
1. Account is inactive for six (6) to twelve (12) months.
2. An authorization letter from the owner of the account is submitted to the office
confirming that he/she allows you to change his/her name.
3. The distributor is willing to pay the prescribed service charge amounting to five
hundred (500.00) pesos.
MEMORANDUM

MEMORANDUM OF AGREEMENT (SHARES)
Agreed Upon that:
1. He shall receive FIVE (5) SHARES from the company accounts on Level 3 and FIVE (5) SHARES
from the company accounts on Level 4 in the amount of TWO HUNDRED (Php200.00) PESOS
maintenance per SHARE.
2. He shall maintain a total of TEN SHARES equivalent to Php2,000.00 worth of products every month
before he/she shall be given his profit share from the company.
3. Failure to maintain shall entitle him to be excluded for the profit sharing for that specific month.
4. If he/she failed to maintain for at least two (2) consecutive months, he shall receive warning letter of
termination of shares from the company.
5. Failure to maintain for three (3) consecutive months, shares shall be forfeited.
6. Profit Sharing will effect three (3) months after the date of purchase.
7. Shareholders will receive 50% CASH and 50% Products of choice with points value (PV).
THIS AGREEMENT made as of this 15th day of January in the year 2018 is designed for
BREAKTHROUGH MICRO-NUTRIENTS WELLNESS (BMW) CORPORATION by and between
RONALD G. DALUMPINES President/ Chief Executive Officer, representing the corporation under the laws
of the Province of North Cotabato, and having its principal place of business at Ground Floor, Fajardo Bldg.,
Jose Abad Santos St., Kidapawan City, and______________________ to be known as The
SHAREHOLDER.
