This document summarizes a scoping literature review on image-based information systems designed for and used by dermatologists. Branch: An interactive, web-based tool for building decision tree classifiers
A crucial task in modern biology is the prediction of complex phenotypes, such as breast cancer prognosis, from genome-wide measurements. Machine learning algorithms can sometimes infer predictive patterns, but there is rarely enough data to train and test them effectively and the patterns that they identify are often expressed in forms (e.g. support vector machines, neural networks, random forests composed of 10s of thousands of trees) that are highly difficult to understand. In addition, it is generally unclear how to include prior knowledge in the course of their construction.
Decision trees provide an intuitive visual form that can capture complex interactions between multiple variables. Effective methods exist for inferring decision trees automatically but it has been shown that these techniques can be improved upon via the manual interventions of experts. Here, we introduce Branch, a new Web-based tool for the interactive construction of decision trees from genomic datasets. Branch offers the ability to: (1) upload and share datasets intended for classification tasks (in progress), (2) construct decision trees by manually selecting features such as genes for a gene expression dataset, (3) collaboratively edit decision trees, (4) create feature functions that aggregate content from multiple independent features into single decision nodes (e.g. pathways) and (5) evaluate decision tree classifiers in terms of precision and recall. The tool is optimized for genomic use cases through the inclusion of gene and pathway-based search functions.
Branch enables expert biologists to easily engage directly with high-throughput datasets without the need for a team of bioinformaticians. The tree building process allows researchers to rapidly test hypotheses about interactions between biological variables and phenotypes in ways that would otherwise require extensive computational sophistication. In so doing, this tool can both inform biological research and help to produce more accurate, more meaningful classifiers.

The document compares data from five different devices - clinical and non-clinical - to assess their reliability for accelerometer-based gait analysis. Participants wore the devices on their waist and walked on a treadmill at speeds of 0.9, 1.1, and 1.3 m/s. Preliminary results found the Fitbit step counts were consistently and significantly lower than observed counts, especially at slower speeds. Further analysis of all device data will compare step counts against manual counts and each other to identify strengths, limitations, and sources of limitations for using consumer devices for gait analysis. The aim is to provide baseline data on device reliability for at-home gait testing.

The researchers developed a taxonomy with 42 CDS elements across 4 axes and interviewed vendors and customers to assess availability in the 9 systems. Results showed systems varied in their CDS capabilities, with the best system missing only 1 capability and the worst missing 18 capabilities. The study aimed to quantify CDS capabilities of commercial EHRs but had limitations such as relying on self-reports and not considering feature usability.

An increasing attention has been given to personal health data (PHD) research over the last years. The rise of researchers’ interest could be attributed to the increasing amount of PHD that are stored across various databases, as a result of individuals’ rapidly- evolving digital life. Accessing and sharing PHD is essential to create personalized health services and to involve patients in the design process of these services. This paper conducts a survey of literature to present an overview of literature about accessing and sharing of PHD. This study aims to identify limitations in research and propose future directions. Sixteen studies were selected from various bibliographic databases and were classified according to three criteria: research type, empirical type and contribution type. The results provide a preliminary review with respect to access and sharing of PHD, addressing a need for more research about PHD accessibility and for solution proposals for both topics.

Have you ever watched a movie in which the same scene was shown several times but as viewed by different individuals? Or, have you watched a detective show in which the witnesses all had differing accounts? The same can hold true for conducting an evaluation of a health information technology project. How you plan and conduct the evaluation is largely dependent on the viewpoint you assume and the perspective with which you approach the evaluation.
Consider a new patient discharge protocol at a small hospital. Do you want to know how the patient feels about the process? Do you want to gather the opinions of nurses who are using this process? Perhaps you want to determine if it is saving the hospital money by freeing up bed space in a more timely fashion. Obtaining each of these viewpoints would require a different approach. Depending on the goal of your evaluation, the model and viewpoint you opt to use will likely vary.
In this Discussion, determine which evaluation model would be most effective for evaluating the health information technology described in one of the scenarios below. Your Instructor will assign a specific scenario by Day 1 of this week.
Scenario 1:
You have recently provided a training program to help nurses and physicians become proficient in the use of a new bedside medication verification (BMV) system.
Scenario 2:
The Chief Medical Officer at your hospital is interested in finding out the impact of a new decision support system on the number of adverse events occurring in the past year.
Scenario 3:
You are helping with the design of a new outpatient surgery center to be built adjacent to the hospital. You are tasked with evaluating the opinions of physicians, nurses, and the general public toward this facility.
To prepare:
Review the information on the types of evaluation models covered in this week’s Learning Resources.
Determine which model would be most appropriate to use for evaluation in the scenario to which you were assigned.
Consider why the viewpoint of the scenario or situation would impact the model used.
View the scenario from a different viewpoint, and consider how a different model might be used.
Reflect on the importance of basing an evaluation on a model.

Technology increases human effectiveness. Using a lever, you can move an object several times your size. In an airplane, you can move exponentially faster than on foot. Using the Internet, you can access information much more quickly than at a library. What possibilities like this exist in the nursing field? What health information technologies can amplify your impact as a nurse far more than ever before? In this Evaluation Project, you will have the opportunity to answer these questions.
Because of the great differences between HIT systems and different goals of an evaluation, there is no one-size-fits-all evaluation plan. Different technologies require different evaluation methods. Consequently, in this part of your Evaluation Project, you will conduct research on how system implementations similar to the one you select have been previously evaluated. After exploring similar system implementations, you will select one research goal and viewpoint to use in the evaluation.
Read the following three scenarios, and select the one that is of most interest to you:
Scenario 1:
Your hospital is implementing a new unified acute and ambulatory Electronic Health Record (EHR) system through which patient care documentation will occur. Interdisciplinary assessment forms (including nursing), clinical decision support, and medical notes will be documented in this system. The implementation of the system is anticipated to improve the hospital’s performance in a multitude of areas. In particular, it is hoped that the use of the EHR system will reduce the rate of patient safety events, improve the quality of care, deter sentinel events, reduce patient readmissions, and impact spending. The implementation of the EHR system is also intended to fulfill the “Meaningful Use” requirements stipulated in the Health Information Technology for Economic and Clinical Health (HITECH) Act. As the hospital’s lead nurse informaticist, you have been tasked with planning the evaluation of the EHR implementation.
Scenario 2:
As the lead nurse informaticist in your hospital, you have been given the task of planning an evaluation for a soon-to-be launched computerized provider order entry (CPOE) system. The CPOE system is designed to replace conventional methods of placing medication, laboratory, admission, referral, and radiology orders. CPOE systems enable health care providers to electronically specify orders, rather than rely on paper prescriptions, telephone calls, and faxes. The intended goal of a CPOE system is to improve safety by ensuring that orders are easily comprehensible through the use of evidence-based order sets. In addition, the CPOE system has the potential for improving workflow by avoiding duplicate orders and reducing the steps between those who place medical orders and their recipients.

Risk shrink: exploring the psychology of risk

Eradication of Malaria from Sri Lanka
Sri Lanka has made extraordinary progress in eliminating malaria, reducing cases from over 500,000 in the early 20th century to just 17 reported cases in 2015. However, malaria elimination faces ongoing challenges, as imported malaria from travelers and migrants is increasing. Sustaining malaria elimination requires continued vigilance, such as ensuring all cases are properly treated according to guidelines, conducting case investigation and response, and preparedness for potential outbreaks. Cooperation with neighboring countries is also important to address the ongoing risk of imported malaria hampering elimination efforts.

Accessing and Sharing Electronic Personal Health Data.
An increasing attention has been given to personal health data (PHD) research over the last years. The rise of researchers’ interest could be attributed to the increasing amount of PHD that are stored across various databases, as a result of individuals’ rapidly- evolving digital life. Accessing and sharing PHD is essential to create personalized health services and to involve patients in the design process of these services. This paper conducts a survey of literature to present an overview of literature about accessing and sharing of PHD. This study aims to identify limitations in research and propose future directions. Sixteen studies were selected from various bibliographic databases and were classified according to three criteria: research type, empirical type and contribution type. The results provide a preliminary review with respect to access and sharing of PHD, addressing a need for more research about PHD accessibility and for solution proposals for both topics.

Have you ever watched a movie in which the same scene was shown several times but as viewed by different individuals? Or, have you watched a detective show in which the witnesses all had differing accounts? The same can hold true for conducting an evaluation of a health information technology project. How you plan and conduct the evaluation is largely dependent on the viewpoint you assume and the perspective with which you approach the evaluation.
Consider a new patient discharge protocol at a small hospital. Do you want to know how the patient feels about the process? Do you want to gather the opinions of nurses who are using this process? Perhaps you want to determine if it is saving the hospital money by freeing up bed space in a more timely fashion. Obtaining each of these viewpoints would require a different approach. Depending on the goal of your evaluation, the model and viewpoint you opt to use will likely vary.
In this Discussion, determine which evaluation model would be most effective for evaluating the health information technology described in one of the scenarios below. Your Instructor will assign a specific scenario by Day 1 of this week.
Scenario 1:
You have recently provided a training program to help nurses and physicians become proficient in the use of a new bedside medication verification (BMV) system.
Scenario 2:
The Chief Medical Officer at your hospital is interested in finding out the impact of a new decision support system on the number of adverse events occurring in the past year.
Scenario 3:
You are helping with the design of a new outpatient surgery center to be built adjacent to the hospital. You are tasked with evaluating the opinions of physicians, nurses, and the general public toward this facility.
To prepare:
Review the information on the types of evaluation models covered in this week’s Learning Resources.
Determine which model would be most appropriate to use for evaluation in the scenario to which you were assigned.
Consider why the viewpoint of the scenario or situation would impact the model used.
View the scenario from a different viewpoint, and consider how a different model might be used.
Reflect on the importance of basing an evaluation on a model.

Technology increases human effectiveness. Using a lever, you can move an object several times your size. In an airplane, you can move exponentially faster than on foot. Using the Internet, you can access information much more quickly than at a library. What possibilities like this exist in the nursing field? What health information technologies can amplify your impact as a nurse far more than ever before? In this Evaluation Project, you will have the opportunity to answer these questions.
Because of the great differences between HIT systems and different goals of an evaluation, there is no one-size-fits-all evaluation plan. Different technologies require different evaluation methods. Consequently, in this part of your Evaluation Project, you will conduct research on how system implementations similar to the one you select have been previously evaluated. After exploring similar system implementations, you will select one research goal and viewpoint to use in the evaluation.
Read the following three scenarios, and select the one that is of most interest to you:
Scenario 1:
Your hospital is implementing a new unified acute and ambulatory Electronic Health Record (EHR) system through which patient care documentation will occur. Interdisciplinary assessment forms (including nursing), clinical decision support, and medical notes will be documented in this system. The implementation of the system is anticipated to improve the hospital’s performance in a multitude of areas. In particular, it is hoped that the use of the EHR system will reduce the rate of patient safety events, improve the quality of care, deter sentinel events, reduce patient readmissions, and impact spending. The implementation of the EHR system is also intended to fulfill the “Meaningful Use” requirements stipulated in the Health Information Technology for Economic and Clinical Health (HITECH) Act. As the hospital’s lead nurse informaticist, you have been tasked with planning the evaluation of the EHR implementation.
Scenario 2:
As the lead nurse informaticist in your hospital, you have been given the task of planning an evaluation for a soon-to-be launched computerized provider order entry (CPOE) system. The CPOE system is designed to replace conventional methods of placing medication, laboratory, admission, referral, and radiology orders. CPOE systems enable health care providers to electronically specify orders, rather than rely on paper prescriptions, telephone calls, and faxes. The intended goal of a CPOE system is to improve safety by ensuring that orders are easily comprehensible through the use of evidence-based order sets. In addition, the CPOE system has the potential for improving workflow by avoiding duplicate orders and reducing the steps between those who place medical orders and their recipients.

The amount of new information and data, and the number of available technologies are growing at an ever-accelerating rate. Did you know that during any given 24 hours, humanity generates enough new information to fill the Library of Congress 70 times (Smolan & Erwitt, 2012)? As a nurse informaticist, it is important to keep current on new developments in the field, but with the rapid pace of change, that effort can be overwhelming. It is easier to keep current with key trends if nurse informaticists focus on selected issues.
In this Discussion, you consider key topics in the field of health care technology. You then consider the different approaches you could take when designing an evaluation in these areas. For example, if you are interested in usability, your goal could be to determine if a system is user friendly from the viewpoint of a nurse. A different goal might be to determine if the location of the system facilitates ease of use from the viewpoint of physicians.

This Discussion serves as practice for the first part of your Evaluation Project. What you derive from your Discussion with colleagues will likely inform the work that you do in Part 1 of the Evaluation Project.

The Learning Health System: Thinking and Acting Across Scales
A Learning Health System (LHS) can be defined as an environment in which knowledge generation processes are embedded into daily clinical practice in order to continually improve the quality, safety, and outcomes of healthcare delivery. While still largely an aspirational goal, the promise of the LHS is a future in which every patient encounter is an opportunity to learn and improve that patient’s care, as well as the care their family and broader community receives. The foundation for building such an LHS can and should be the Electronic Health Record (EHR), which provides the basis for the comprehensive instrumentation and measurement of clinical phenotypes, as well as a means of delivering new evidence at the patient- and population levels. In this presentation, we will explore the ways in which such EHR-derived phenotypes can be combined with complementary data across a spectrum from biomolecules to population level trends, to both generate insights and deliver such knowledge in the right time, place, and format, ultimately improving clinical outcomes and value.

This document presents the Concept Definition Generator (CDG), an open source tool for defining healthcare concepts using multilevel modeling specifications. The CDG allows domain experts to graphically represent healthcare concepts and automatically generate associated XML schemas. It was developed in Python with a wxPython graphical interface to run cross-platform. The CDG addresses the significant challenges of knowledge representation for semantic interoperability of electronic health records. Future work includes further standardizing healthcare terminologies and developing proper modeling tools.

This document discusses using Web 2.0 social technologies to build an online community for an interdisciplinary biomedical research group. It describes conducting a study to select the best Web 2.0 platform to use. The study examined strengths, weaknesses and criteria such as hosting, pricing, and security requirements. Initial results found the group used the selected platform, PBworks, to share knowledge and documents, though some faced barriers like time constraints and technical experience.

Examples of needs we have solved include helping clients organize large amounts of emails, disseminate project updates across organizations, provide just-in-time access to information, improve project management and communication between departments, and overcome reluctance to adopt new technologies through training and emphasis on low investment costs.

This document describes a pilot program at an urban community clinic to provide point-of-care patient education (POCPE) to asthma patients using social media and portable media devices. The program aimed to educate patients on proper use of metered dose inhalers. Videos were created and uploaded to a private YouTube channel to demonstrate inhaler use. Surveys found that using these innovative tools improved patient understanding compared to traditional methods. Lessons learned can help other clinics develop similar POCPE programs to better educate patients.

This document discusses a pilot study that investigated using Web 2.0 social technologies like wikis to enhance collaboration for a community-based participatory research project. The study found that deploying a wiki helped the team construct an iterative digital knowledge repository and enhanced project management. Surveys found wikis superior to email for organizing information. Interviews revealed that project needs, sustained use, organizational culture and active participation affect wiki usage. The York methodology was used to select and chart articles about current image-based information systems designed for and used by dermatologists around the world. A grounded theory analysis identified four key concepts embedded in system development. These included users, system type, system purpose, and access/security concerns. Results suggested implications for system developers and library/information science professionals. Knowledge about user experience (UX) and usability testing is recommended prior to designing an image-based information system for dermatologists.
