Mobile phones, sensors, patients, hospitals, researchers, providers and organizations are nowadays generating huge amounts of healthcare data. The real challenge in healthcare systems is how to find, collect, analyze and manage information to make people's lives healthier and easier
By contributing not only to understand new diseases and therapies but also to predict outcomes at earlier stages and make real-time decisions.
IOT is revolutionizing various industries and we cannot ignore the huge transformation it can bring to the health care industry. IOT combined with the analytics and cloud can provide unprecedented capabilities in healthcare – virtual healthcare, predictive diagnostics, preventive care and targeted care to name a few. It can help in improving effectiveness and quality of patient care at the same time help the healthcare organizations to improve efficiency and improve their financial and administrative performance.
Patents are a good information resource for obtaining IoT (Internet of Things) technology development status. Followings illustrate some examples of patents that provide current status of the IoT healthcare/medical devices technology development: smart health monitoring devices, smart haptic medical devices, patient movement monitoring devices, healthcare robots and medical kiosk.
According to a report from MarketResearch.com, millions of new Internet of Medical Things (IoMT) will be added to health systems and the market segment is poised to hit $117 billion by 2020. Medical device manufacturers have traditionally focused on patient safety and time to market rather than security. Long FDA approval cycles mean that approved devices are often running outdated operating systems versions with known vulnerabilities and limited or no patching ability. This lack of adequate security in IoT and IoMT is why Gartner is predicting that by 2020 25% of all enterprise breaches will involve IoT. Securing IoMT requires close collaboration between biomedical and IT teams and a plan to address three core areas of IoMT security – physical, connection and data. This session will focus on practical steps to improving IoMT security without expensive infrastructure upgrades or wholesale legacy medical device replacements.
Healthcare ecosystem is witnessing a huge transformation lately propelled by improved care and patient outcomes as the critical drivers, in addition mobile technology and IoT are leading the way for innovation in the healthcare industry, by integration sensor network such as Beacons, NFC, RFID etc.. that will play an important roles to improvise the quality of services in Intelligent Healthcare sectors .

Antibiotic Stewardship by Anushri Srivastava.pptxAnushriSrivastav
Stewardship is the act of taking good care of something.
Antimicrobial stewardship is a coordinated program that promotes the appropriate use of antimicrobials (including antibiotics), improves patient outcomes, reduces microbial resistance, and decreases the spread of infections caused by multidrug-resistant organisms.
WHO launched the Global Antimicrobial Resistance and Use Surveillance System (GLASS) in 2015 to fill knowledge gaps and inform strategies at all levels.
ACCORDING TO apic.org,
Antimicrobial stewardship is a coordinated program that promotes the appropriate use of antimicrobials (including antibiotics), improves patient outcomes, reduces microbial resistance, and decreases the spread of infections caused by multidrug-resistant organisms.
ACCORDING TO pewtrusts.org,
Antibiotic stewardship refers to efforts in doctors’ offices, hospitals, long term care facilities, and other health care settings to ensure that antibiotics are used only when necessary and appropriate
According to WHO,
Antimicrobial stewardship is a systematic approach to educate and support health care professionals to follow evidence-based guidelines for prescribing and administering antimicrobials
In 1996, John McGowan and Dale Gerding first applied the term antimicrobial stewardship, where they suggested a causal association between antimicrobial agent use and resistance. They also focused on the urgency of large-scale controlled trials of antimicrobial-use regulation employing sophisticated epidemiologic methods, molecular typing, and precise resistance mechanism analysis.
Antimicrobial Stewardship(AMS) refers to the optimal selection, dosing, and duration of antimicrobial treatment resulting in the best clinical outcome with minimal side effects to the patients and minimal impact on subsequent resistance.
According to the 2019 report, in the US, more than 2.8 million antibiotic-resistant infections occur each year, and more than 35000 people die. In addition to this, it also mentioned that 223,900 cases of Clostridoides difficile occurred in 2017, of which 12800 people died. The report did not include viruses or parasites

Defecation
Normal defecation begins with movement in the left colon, moving stool toward the anus. When stool reaches the rectum, the distention causes relaxation of the internal sphincter and an awareness of the need to defecate. At the time of defecation, the external sphincter relaxes, and abdominal muscles contract, increasing intrarectal pressure and forcing the stool out
The Valsalva maneuver exerts pressure to expel faeces through a voluntary contraction of the abdominal muscles while maintaining forced expiration against a closed airway. Patients with cardiovascular disease, glaucoma, increased intracranial pressure, or a new surgical wound are at greater risk for cardiac dysrhythmias and elevated blood pressure with the Valsalva maneuver and need to avoid straining to pass the stool.
Normal defecation is painless, resulting in passage of soft, formed stool
CONSTIPATION
Constipation is a symptom, not a disease. Improper diet, reduced fluid intake, lack of exercise, and certain medications can cause constipation. For example, patients receiving opiates for pain after surgery often require a stool softener or laxative to prevent constipation. The signs of constipation include infrequent bowel movements (less than every 3 days), difficulty passing stools, excessive straining, inability to defecate at will, and hard feaces
IMPACTION
Fecal impaction results from unrelieved constipation. It is a collection of hardened feces wedged in the rectum that a person cannot expel. In cases of severe impaction the mass extends up into the sigmoid colon.
DIARRHEA
Diarrhea is an increase in the number of stools and the passage of liquid, unformed feces. It is associated with disorders affecting digestion, absorption, and secretion in the GI tract. Intestinal contents pass through the small and large intestine too quickly to allow for the usual absorption of fluid and nutrients. Irritation within the colon results in increased mucus secretion. As a result, feces become watery, and the patient is unable to control the urge to defecate. Normally an anal bag is safe and effective in long-term treatment of patients with fecal incontinence at home, in hospice, or in the hospital. Fecal incontinence is expensive and a potentially dangerous condition in terms of contamination and risk of skin ulceration
HEMORRHOIDS
Hemorrhoids are dilated, engorged veins in the lining of the rectum. They are either external or internal.
FLATULENCE
As gas accumulates in the lumen of the intestines, the bowel wall stretches and distends (flatulence). It is a common cause of abdominal fullness, pain, and cramping. Normally intestinal gas escapes through the mouth (belching) or the anus (passing of flatus)
FECAL INCONTINENCE
Fecal incontinence is the inability to control passage of feces and gas from the anus. Incontinence harms a patient’s body image

How does the BHW receive medical diagnosis by the Doctor? through SMS?
Printout?2
The BHW receives the medical diagnoses through The Finder Mobile App. She then
sends it to patient via SMS or F2F consultation
The Finder Mobile App can work without any Internet connection. Once The Finder
Mobile App detects an Internet connection, then it will automatically upload the data to
the Cloud database.
How is the limited to poor WiFi and cell phone signal in rural areas addressed?1
Will the BHW have enough time to do all that on top what he/she is already
doing?3
This app will save BHW time to do this. Instead of double entry writing Philhealth
reports, they will just be typing the data on their mobile phone once.
20. 5
It will be unlikely that the BHW will be able to contact the Specialists directly, so
where will all these data be forwarded to? There will be a lot of information sifting
and categorizing -- Who will be in charge of that?
5
The BHW will be trained to use The Finder Mobile App to refer to the Doctor who in turn
will refer to the Specialists in the area through The Finder Web App. All data will be
stored on the Cloud, which is accessed by the medical group, who decides to use The
Finder for patient treatment.
The trainors training will be done by ATI in LGU test sites. The BHWs, Doctors, and
Laboratory Personnel will be trained on how to use The Finder Mobile and Web
Applications.
Who will be conducting the training for the users, and how is a support network
strategized, especially in the most far flung areas in the country?4
21. 5
How can it be guaranteed that the specialist will be looking into the patient's
information with a sense of urgency?6
Before the name of the doctor appears in The Finder Mobile App, he/she would have
already agreed in joining/using the innovation. The doctor should already be aware that he
will be receiving messages from The Finder Mobile App. There will also be an expiration
period prompting the BHW to contact someone else if the doctor does not respond within
the allotted period.
22. 5

The problem causes preventable morbidity and mortality
Access to healthcare services is crucial to good health, yet a majority of rural residents face a variety of barriers. Moreover, there is a lack of medical specialists in the Philippines, which is especially true in the rural areas. In a population of 106 million Filipinos, there are only 60,000 active doctors and most specialists practice in urban areas.
Because of this unfortunate plight of the rural populace, Ascendant Technologies, Inc., conceptualized and developed The Finder which enhances the capability of Barangay Health Workers, Medical Doctors, Nurses, and Midwives to detect early signs of diseases like cancer, particularly Hodgkin’s Lymphoma and Multiple Myeloma. They will be able to request for confirmatory laboratory and diagnostic tests and refer their patients to appropriate specialists, thereby enabling early intervention and better medical management and treatment outcomes.

Ascendant Technologies, Inc., aims to solve the problem of poor access to healthcare systems for people living in the rural areas by its healthcare software innovation, The Finder.
The Finder is a mobile application that can be used by Barangay Health Workers as an Electronic Medical Record for patients. It will provide the Barangay Health Worker links to laboratories and diagnostic facilities as well as disease specialists within the area of practice. The Finder has a web platform which laboratory or diagnostic facilities and specialists can access to encode and review laboratory test or diagnostic study results for further diagnostic and medical management.
